Evidence of end-effector based gait machines in gait rehabilitation after CNS lesion.

BACKGROUND: A task-specific repetitive approach in gait rehabilitation after CNS lesion is well accepted nowadays. To ease the therapists' and patients' physical effort, the past two decades have seen the introduction of gait machines to intensify the amount of gait practice. Two principles have emerged, an exoskeleton- and an endeffector-based approach. Both systems share the harness and the body weight support. With the end-effector-based devices, the patients' feet are positioned on two foot plates, whose movements simulate stance and swing phase. OBJECTIVE: This article provides an overvi

Gait pathology assessed with Gillette Gait Index in patients after CNS tumour treatment.

Brain tumour is the third leading cause of death in children and adolescents younger than 16 years of age. The increasing survival rate of these patients makes their follow-up and quality of life assessment an important task. This study evaluated the gait pathology of the patients after the combined treatment for central nervous system (CNS) tumours. It assessed if the severity of gait deviation depended on the tumour site or age of illness onset. Gait analysis was performed on patients who completed the treatment (neurosurgery, chemo- and radiotherapy) and were disease-free at the time of the

Gait in thoracolumbar/lumbar adolescent idiopathic scoliosis: effect of surgery on gait mechanisms.

For patients whose scoliosis progresses, surgery remains the ultimate way to correct and stabilise the deformity while maintaining as many mobile spinal segments as possible. In thoracolumbar/lumbar adolescent idiopathic scoliosis (AIS), the spinal fusion has to be extended to the lumbar spine. The use of anterior spinal fusion (ASF) instead of the classic posterior fusion (PSF) may preserve more distal spinal levels in attempt to limit the consequences of surgery on trunk mobility. The effects of surgery on body shape, pain and the decompensation phenomenon have all been well evaluated. Very

Linking executive dysfunction to gait initiation deficits in Parkinson's disease with freezing of gait.

INTRODUCTION: Freezing of gait (FoG) is a disabling symptom of Parkinson's disease (PD) that often emerges during gait initiation and is linked to executive dysfunction. We investigated whether cognitive inhibition and processing speed are associated with gait initiation deficits in people with PD with and without FoG. METHODS: Seventy-nine individuals with PD (50 PD + FoG, 29 PD-FoG) and 37 healthy older adults were evaluated. Executive functions were assessed using the Victoria Stroop test and the Trail Making Test, with scores converted to age- and education-adjusted z-scores. Gait initiat

Topological Gait Analysis: A New Framework and Its Application to the Study of Human Gait.

OBJECTIVE: This study introduces a physiologically driven topological gait analysis (TGA) framework to gain insights into pathological gait. METHODS: A publicly available gait dataset consisting of four groups: healthy adults, people with Parkinson's disease (PD), Huntington's disease (HD), and amyotrophic lateral sclerosis (ALS) was used. The topological properties of the configuration space of three gait parameters were studied by approximating the underlying distribution through a Gaussian kernel-based density estimation technique. Thereafter, sublevel sets of the density estimate were ana

Gait speed and individual characteristics are related to specific gait metrics in neurotypical adults.

Gait biofeedback is a well-studied strategy to reduce gait impairments such as propulsion deficits or asymmetric step lengths. With biofeedback, participants alter their walking to reach the desired magnitude of a specific parameter (the biofeedback target) with each step. Biofeedback of anterior ground reaction force and step length is commonly used in post-stroke gait training as these variables are associated with self-selected gait speed, fall risk, and the energy cost of walking. However, biofeedback targets are often set as a function of an individual's baseline walking pattern, which ma

Gait stability in peripheral artery disease: a phase-dependent analysis using gait tube methodology.

INTRODUCTION: Peripheral Artery Disease (PAD) affects over eight million U.S. adults, impairing mobility, quality of life, and increasing fall and cardiovascular risks. PAD reduces lower limb blood flow and contributes to neuromuscular dysfunction, leading to unstable gait. Traditional stability metrics often miss phase-specific changes. Gait Tube Stability (GTS) is a phase-dependent, three-dimensional method that analyzes center of mass (COM) velocity using ellipsoidal variability to detect directional instability. This study examined whether GTS can identify phase-specific gait deficits in P

A Multicenter Standardized Gait Database from the ORITEL Network for Cerebral Palsy Gait Analysis.

Gait analysis is a cornerstone of clinical decision-making in cerebral palsy (CP), yet multicenter variability limits comparability and translation. This work reports the creation of a standardized, multicenter gait database and a machine learning (ML) classifier to distinguish gait patterns of CP hemiplegia, CP diplegia, and typically developing group to validate the database. Data is contributed by eight ORITEL motion analysis laboratories (156 sessions, 582 trials) using two systems (Vicon/BTS), with unified nomenclature, unit normalization, and resampling. For classification, 95 sessions (

Monitoring Gait Recovery After Total Knee Arthroplasty Using Wearable Sensors: Responsiveness of Gait Accelerations.

Functional recovery after total knee arthroplasty (TKA) varies widely among individuals, and traditional assessments often fail to detect subtle changes in real-world walking ability. Wearable sensors offer continuous and objective tracking of gait outside of clinical settings. In this prospective, longitudinal study, thirty-one patients undergoing unilateral TKA wore thigh-mounted accelerometers continuously from 2 weeks before surgery through 90 days postoperatively. We analyzed temporal gait features (stride time and duty factor) and frequency-domain metrics (power of the first five frequen

Nigral stimulation for freezing of gait: kinematic gait parameters inform optimization of stimulation frequency.

Innovative technology allows for personalization of stimulation frequency in dual-site deep brain stimulation (DBS), offering promise for challenging symptoms in advanced Parkinson's disease (PD), particularly freezing of gait (FoG). Early results suggest that combining standard subthalamic nucleus (STN) stimulation with substantia nigra pars reticulata (SNr) stimulation may improve FoG outcomes. However, patient response and the optimal SNr stimulation frequency vary. Our randomized controlled trial studied patients with advanced PD and persistent FoG, while recording gait kinematics through

A novel gait quality measure for characterizing pathological gait based on Hidden Markov Models.

This study addresses the characterization of normal gait and pathological deviations caused by neurological diseases. We focus on the angular knee kinematics in the sagittal plane and we propose to exploit Hidden Markov Models to build a statistical model of normal gait. Such model provides a log-likelihood score that quantifies gait quality. Hence allowing to assess deviations of pathological cycles from normal gait. Our approach allows a refined characterization of motor impairments of three different patients' groups. In particular, it detects the affected lower limb in Hemiparetic patients

Which Gait Tasks Produce Reliable Outcome Measures of Freezing of Gait in Parkinson's Disease?

BACKGROUND: Measurement of freezing of gait (FOG) relies on the sensitivity and reliability of tasks to provoke FOG. It is currently unclear which tasks provide the best outcomes and how medication state plays into this. OBJECTIVE: To establish the sensitivity and test-retest reliability of various FOG-provoking tasks for presence and severity of FOG, with (ON) and without (OFF) dopaminergic medication. METHODS: FOG-presence and percentage time frozen (% TF) were derived from video annotations of a home-based FOG-provoking protocol performed in OFF and ON. This included: the four meter walk

Overground Robotic Gait Trainer mTPAD Improves Gait Symmetry and Weight Bearing in Stroke Survivors.

Stroke is a leading cause of disability, impairing the ability to generate propulsive forces and causing significant lateral gait asymmetry. We aim to improve stroke survivors' gaits by promoting weight-bearing during affected limb stance. External forces can encourage this; e.g., vertical forces can augment the gravitational force requiring higher ground reaction forces, or lateral forces can shift the center of mass over the stance foot, altering the lateral placement of the center of pressure. With our novel design of a mobile Tethered Pelvic Assist Device (mTPAD) paired with the DeepSole s
