BACKGROUND: The introduction in many countries of conjugate vaccines against Haemophilus influenzae type b, Streptococcus pneumoniae, and Neisseria meningitidis has led to significant reductions in acute bacterial meningitis (ABM) in children. However, recent population-based data on ABM in sub-Saharan Africa are limited.
METHODS: Population-based surveillance for meningitis was carried out in a rural area of The Gambia under demographic surveillance from 2008 to 2017, using standardised criteria for referral, diagnosis and investigation. We calculated incidence using population denominators.

UNLABELLED: In West Africa, kidney diseases are frequently seen, but diagnostic and therapeutic options are poor due to limited access to specialized facilities. To unravel the etiology and develop clinical guidelines, we collected clinical data and results of kidney biopsies in 121 pediatric and mostly young adult patients with edema and proteinuria in The Gambia. Workup included clinical examination, urine and serum analysis, and kidney biopsy findings. Selected cases were treated with steroids.
RESULTS: The median age was 14.9 years (range 1.8-52.0) at presentation. The most frequent under

The clinical relevance of pulmonary non-tuberculous mycobacteria (PNTM) in The Gambia is unknown. The aim of this study was to estimate the prevalence of non-tuberculous mycobacteria (NTM) in colonisation, and the burden of clinically relevant pulmonary NTM (PNTM) disease in The Gambia. This was a cross-sectional study of the prevalence of NTM in participants aged ≥ 15 years, in a nationwide tuberculosis (TB) prevalence survey between December 2011 and January 2013. We enrolled 903 participants with suspected NTM and NTM cultures were confirmed by 16S rRNA gene sequencing analyses. We applied

BACKGROUND: The Gambia is a popular holiday destination that is classified as a high-risk area for rabies exposure by the WHO.
METHODS: This was a retrospective study of 49 subjects with rabies exposure attending The Medical Research Council Clinical Service, The Gambia, for treatment. Their demographics, details of exposure and treatment delivery were evaluated.
RESULTS: An estimated 16% (8/49) of all exposures were likely to be from rabid animals. Moreover, 24% (12/49) of patients were tourists, none of whom had received pre-exposure prophylaxis.
CONCLUSION: Rabies exposure is a serious r

Mortality from childhood cerebral malaria remains unacceptably high in endemic regions. This survey was conducted between June and December 2001 among 69 primary caregivers of children admitted for cerebral malaria in Bansang Hospital, Central River Division (CRD), The Gambia to describe decision-making process at the family level that could have impact on malaria mortality. Thirty two percent of children presented in coma after 24 h of onset of illness. The eldest person in the compound or the father was responsible for taking decision on when hospital treatment was necessary in 85% of the ca

PURPOSE: Trachoma is the leading infectious cause of blindness. However, there are few data on the natural history of trachomatous trichiasis to guide program planning or that investigate its pathogenesis.
METHODS: A cohort of Gambians with trichiasis in one or both eyes who had declined surgery was observed. Clinical examinations were performed at baseline and 4 years later. Conjunctival swab samples were collected for Chlamydia trachomatis PCR and bacteriology.
RESULTS: One hundred fifty-four people were examined at baseline and 4 years later (241 nonsurgical eyes). At baseline 124 (52%) e

This study aimed to describe the quality and costs of sexually transmitted disease (STD) case management in urban pharmacies in The Gambia, and explore pharmacy workers' (PWs) willingness to improve the STD care they provide. PWs from 24 registered pharmacies were interviewed in order to collect information on their knowledge and practices regarding management of STDs. The same pharmacies were visited by a male 'simulated client' (SC) to ascertain how urethral discharge syndrome (UDS) cases were managed in practice. Fifteen (63%) pharmacies were equipped for treatment of UDS, pelvic inflammato

Cannabis abuse is a major public health problem in The Gambia and other parts of West Africa, and the rise in the incidence of psychotic illness reflects the increased background use of cannabis by the local population. A case-control study was performed to determine the association between psychosis and cannabis abuse in The Gambia and the importance of other risk factors. Out of 234 patients admitted to Campama Psychiatric Unit over 12 months, 210 (90%) were enrolled in a case-control study. Urine was tested for cannabinoid substances and 38% were positive compared with 12% of matched non-ps

A population based survey of blindness and eye disease has been conducted throughout the whole country of The Gambia, and 8174 people were examined. The prevalence of blindness (best acuity less than 3/60) was 0.7% and low vision (6/24-3/60) 1.4%. The causes of blindness were cataract (55%), non-trachomatous corneal opacity/phthisis (20%), and trachoma (17%). An estimated 5500 people in The Gambia require cataract surgery, 4600 eyelid surgery for entropion, and 4600 people spectacles to correct a refractive error which causes a visual acuity of less than 6/18. More than half the current burden

Plasmodium, the causative agents of malaria, are obtained by mosquitoes from an infected human. Following Plasmodium acquisition by Anopheles gambiae, mosquito gamma-interferon-inducible lysosomal thiol reductase (mosGILT) plays a critical role in its subsequent sporogony in the mosquito. A critical location for this development is the midgut, a tissue we show expresses mosGILT. Using membrane-feeding and murine infection models, we demonstrate that antibodies against mosGILT reduce the number of P. falciparum and P. berghei oocysts in the midgut and the infection prevalence of both species in

BACKGROUND: Mosquitoes in the Anopheles (An.) gambiae species complex are major vectors of Plasmodium falciparum malaria. One reason for this is the high anthropophily of the constituent species An. coluzzii, An. gambiae sensu stricto, and An. arabiensis. In contrast, their sister species An. quadriannulatus is highly zoophilic. Anopheles mosquitoes largely rely on chemical cues for host-seeking, which are primarily detected by four chemosensory gene families: olfactory receptors (Ors), ionotropic receptors (Irs), gustatory receptors (Grs), and odorant binding proteins (Obps). Genes from these

OBJECTIVE: Paragonimiasis is a foodborne trematode infection of the lungs caused by Paragonimus spp., presenting clinically with similar symptoms to active tuberculosis (TB). Worldwide, an estimated 20.7 million people are infected with paragonimiasis, but relatively little epidemiological data exists for Africa. Given a recently reported case, we sought to establish whether paragonimiasis should be considered as an important differential diagnosis for human TB in The Gambia, West Africa.
RESULTS: We developed a novel PCR-based diagnostic test for Paragonimus species known to be found in West

BACKGROUND: Agent-based models (ABMs) have been used to model the behaviour of individual mosquitoes and other aspects of malaria. In this paper, a conceptual entomological model of the population dynamics of Anopheles gambiae and the agent-based implementations derived from it are described. Hypothetical vector control interventions (HVCIs) are implemented to target specific activities in the mosquito life cycle, and their impacts are evaluated.
METHODS: The core model is described in terms of the complete An. gambiae mosquito life cycle. Primary features include the development and mortalit

A technique for the rapid field assessment of African animal trypanosomosis (AAT) was developed during studies in the Gambia. This involved gathering indigenous information from rapid-appraisal questionnaires addressed to local informants, the results of single tsetse surveys and evaluations of the prevalence of trypanosome infections in village cattle. Local informants included livestock owners and herdsmen and trained personnel such as livestock assistants. The answers to the questionnaires were weighted in order to translate them into semi-quantitative ranked estimates (zero, low, medium, h

The polyubiquitin gene from the mosquito Anopheles gambiae has been cloned and sequenced, and its structure is reported along with sequence analysis results. The gene consists of approximately seven tandem head-to-tail repeat units of the seventy-six amino acid-coding ubiquitin monomer. It is expressed constitutively in larvae, pupae and adults of An. gambiae, as well as in a cell line derived from this mosquito species. A probe made from a DNA fragment containing the coding region of the gene recognizes transcripts of approximately 3.6 kb and 4.4 kb in RNA isolated from all mosquito developme

The G-3 strain of Anopheles gambiae was compared with 2 other strains of An. gambiae and An. freeborni, An. stephensi, and An. dirus for susceptibility to infection with 7 different geographic strains of Plasmodium vivax. Ratios of infection varied, indicating that certain strains of P. vivax were more infectious to the G-3 strain of An. gambiae than to other anopheline species/strains. Based on the comparative number of oocysts per mosquito, the relationships between the 3 strains of An. gambiae were closer than between the G-3 strain of An. gambiae and the 3 other species of Anopheles. Alternative regimes for the treatment of Schistosoma haematobium infection were compared in two trials. Praziquantel at a dose of 40 mg kg-1 appeared to cure 63% of a random sample of heavily infected subjects; significantly more than the 18% cured by three fortnightly doses of metrifonate at 10 mg kg-1. However, praziquantel led to a greater incidence of mild, transient side-effects. A single dose of metrifonate was found to be an inadequate treatment in the same group of subjects as it left 53% with an egg count of at least 100 ova/10 ml. A combination of 10 mg kg-1 of metrifonate and 25 mg
