Dorsal Root Ganglion Stimulation for the Management of Inflammatory Bowel Disease: A Case Report.

This case report presents the successful use of dorsal root ganglion stimulation (DRGS) in a 30-year-old female patient with Crohn's disease. Despite extensive treatments, the patient experienced chronic abdominal pain, diarrhea, bloating, cramping, fatigue, and other debilitating symptoms. After a successful DRGS trial with leads placed on the right T6 and T10, she was implanted with a permanent system. At 18 months she continues to experience significant improvement in symptoms, including reduced abdominal pain, decreased defecation frequency, better stool consistency, less pain with eating

Dorsal Root Ganglion Stimulation to Treat Chronic Shoulder Pain: A Case Report.

A 67-year-old man presented with severe 9 of 10 intractable pain of the left shoulder joint after arthroplasty and revision surgeries, with associated weakness, atrophy, and limited range of motion in all directions. Dorsal root ganglion stimulation (DRG-S) at the left C4, C5, and C6 levels was used after failed conservative and interventional measures, resulting in significant improvement in pain, function, and quality of life measures through 6 months postimplantation. Larger studies should examine if DRG-S is effective in treating chronic arthritic joint pain as well as chronic postsurgical

Dorsal root ganglion stimulation lead fractures: potential mechanisms and ways to avoid.

Dorsal root ganglion stimulation (DRGS) therapy is a rapidly emerging tool being used by pain physicians in the treatment of chronic pain. Complex regional pain syndrome (CRPS), a debilitating disease whose mechanism is still has yet to be fully elucidated, is a common pathology targeted by DRGS therapy, often better results than traditional spinal cord stimulation. DRGS therapy, however, is not bereft of complications. Lead migration and fracture are two examples in particular that are among the most common of these complications. The authors report an unusual case of lost efficacy due to lea

Effect of electroacupuncture on porcine cardiac excitability induced by left stellate ganglion stimulation.

Augmentation of cardiac sympathetic tone has been shown to induce ventricular arrhythmias. Acupuncture has been clinically used to treat hypertension, angina pectoris, and atrial arrhythmias. However, the effects of acupuncture on ventricular electrophysiology and autonomic tone remain unknown. We hypothesized that acupuncture attenuates cardiac excitability and corrects the imbalance of autonomic tone during sympathetic hyperactivity. Fourteen Yorkshire pigs were randomized to electroacupuncture (EA, 2 Hz, 0.3-0.5 mA, 0.5 ms duration) or control (without EA) groups. Animals were sedated with

Stimulation strategies for selective activation of retinal ganglion cell soma and threshold reduction.

OBJECTIVE: Retinal prosthetic implants restore partial vision to patients blinded due to outer retinal degeneration, using a camera-guided multielectrode array (MEA) that electrically stimulates surviving retinal neurons. Commercial epi-retinal prostheses use millisecond-scale charge-balanced, symmetric, cathodic-first biphasic pulses to depolarize retinal ganglion cells (RGCs) and bipolar cells (BCs), frequently creating oblong perceptions of light related to axonal activation of RGCs. Stimulation strategies that avoid axonal stimulation and decrease the threshold of targeted neurons may sign

A 12-month prospective study of gasserian ganglion stimulation for trigeminal neuropathic pain.

AIMS: Trigeminal neuropathic pain is a broad diagnostic category that includes pain of several etiologies and excludes trigeminal neuralgia. The authors report a prospective series of percutaneous gasserian ganglion stimulation for trigeminal neuropathic pain. METHODS: Patients who experienced >50% reduction in pain from a 7- to 10-day trial period underwent permanent implantation and were prospectively followed. RESULTS: Eight of 10 trialed patients received a permanent implant. At the 12-month follow-up, 2 patients had been explanted and 1 was lost to follow-up. Three (all working at that

Stimulation of somatostatin expression in developing ciliary ganglion neurons by cells of the choroid layer.

An important component of neuronal development is the matching of neurotransmitter expression with the appropriate target cell. We have examined how peptide transmitter expression is controlled in a simple model system, the avian ciliary ganglion (CG). This parasympathetic ganglion contains 2 distinct types of neurons: choroid neurons, which project to vasculature in the eye's choroid layer and use somatostatin as a co-transmitter with ACh, and ciliary neurons, which innervate the ciliary body and iris and use ACh but no known peptide co-transmitter. We have found that the earliest development

Alleviation of atypical trigeminal pain by stimulation of the Gasserian ganglion via an implanted electrode.

Facial pain associated with disturbances of sensitivity as a sign of injury of the trigeminal nerve is difficult to treat. Drug therapy as well as blocking procedures are generally ineffective. Transcutaneous nerve stimulation offers a new possibility for pain alleviation but is inconvenient for the patient. A method for stimulation of the gasserian ganglion via an implanted electrode has been developed. Five cases are reported on and they all experienced excellent or good pain relief. Follow-up is 6-21 months.

Effect of tilting on the pressor responses to McN-A-343, a muscarinic sympathetic ganglion stimulant.

1 In anaesthetized cats and dogs treated with mecamylamine, the pressor response to McN-A-343 was increased when the animals were changed from a supine to a head-up, tilted position. This potentiation was not seen in rats. 2 The potentiation of the McN-A-343 pressor response was not affected by propranolol, destruction of the brain, or removal of the intestines, spleen or adrenal glands. It was promptly abolished by applying pressure to the lower half of the tilted animal. No increase in the pressor response to McN-A-343 occurred when cats were tilted head down. The potentiated response in til

Inhibition by transcutaneous electrical stimulation of noxious heat elicited in human Gasserian ganglion.

During percutaneous thermocoagulation of the Gasserian ganglion for trigeminal pain, the pain perception threshold was established in a series of 25 awake patients by measuring the temperature at the electrode tip within the ganglion during slow increase of the radiofrequency current. Between 42 and 48 degress C pain perception was regularly reported by the patients. A cutaneous-stimulating system was then applied to the trigeminal area which was reported to be painful during the heating. The external stimulation attempted to preferentially stimulate the A afferent nerve fibres. When external

Studies on the tachyphylaxis to nicotinic stimulant drugs in the perfused hypogastric ganglion of the guinea-pig.

1 The guinea-pig hypogastric ganglion, perfused through its vasculature with physiological saline solutions, will respond repeatedly to nicotinic stimulant drugs injected at 4 min intervals into the perfusion stream. Stimulation is indicated by contraction of the vas deferens. After a varying number of doses of nicotinic stimulants, complete tachyphylaxis usually develops, and normally this persists for at least 1 hour. During this time, however, electrical stimulation of the hypogastric nerve is still fully effective.2 Tachyphylaxis may be reversed for varying periods of time by all of the fo

Very Low Frequencies Maintain Pain Relief From Dorsal Root Ganglion Stimulation: An Evaluation of Dorsal Root Ganglion Neurostimulation Frequency Tapering.

BACKGROUND: Dorsal root ganglion neurostimulation (DRG-S) is effective in treating various refractory chronic pain syndromes. In preclinical studies, DRG-S at very low frequencies (<5 Hz) reduces excitatory output in the superficial dorsal horn. Clinically, we have also observed the effectiveness of DRG-S at low frequencies. We conducted a case series to describe the effect of very low-frequency DRG-S stimulation on clinical outcomes. MATERIALS AND METHODS: DRG-S for refractory low back pain was initiated at parameters consistent with published values. Thereafter, the stimulation frequency of

Piezoelectric Nanofibrous Scaffolds for Promoting the Growth of Spiral Ganglion Neurons via Acoustic Stimulation.

The loss of spiral ganglion neurons (SGNs) and hair cells often leads to sensorineural hearing loss (SNHL), whose therapeutic effect depends on the quality and functional integrity of the residual SGNs. Compared with traditional pharmacological therapies, tissue engineering methods have been developed to provide a biomimetic environment for SGN regeneration, but static strategies and non-responsive scaffolds still limit the therapy efficiency. Here, we present a composite piezoelectric nanofibrous scaffold responsive to acoustic stimulation to promote the growth of SGNs. The oriented nanofibro

Dorsal Root Ganglion Stimulation for Postherpetic Neuralgia: A Pilot Scoping Review of the Current Evidence.

This review synthesizes current evidence regarding the use of dorsal root ganglion (DRG) stimulation, a form of pain neuromodulation, for managing postherpetic neuralgia (PHN), a debilitating chronic neuropathic pain syndrome arising from herpes zoster virus infection. Comprehensive searches across multiple databases (MEDLINE, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library) for studies published from 2010 onward were supplemented by manual reference checks and gray literature review. Inclusion criteria encompassed adults with PHN (pain persisting ≥

New-Onset Tinnitus After Dorsal Root Ganglion Stimulator Implantation: A Case Report.

Dorsal column (dcSCS) and dorsal root ganglion stimulation (DRG-S) complications are similar, typically related to placement and device failure. We present the first case of tinnitus after DRG-S implantation. The patient presented with complex regional pain syndrome (CRPS) type 2. After previous failed treatments, she had a lumbosacral DRG-S trial, which provided relief; however, she briefly noted ringing in her ears. After permanent implantation, she reported persistent, intolerable left-sided tinnitus. Tinnitus can be modulated by secondary somatosensory inputs to the cochlear nucleus from t

Dorsal Root Ganglion Stimulation as a Treatment for Chronic Refractory Groin Pain: A Case Series.

BACKGROUND: Complex regional pain syndrome (CRPS) can be a debilitating source of pain and is characterized by a combination of sensory, motor, vasomotor, and autonomic abnormalities. Neuromodulation can serve as a therapeutic intervention for groin CRPS. CASE REPORT: Two patients with refractory CRPS in the groin and perineum underwent bilateral dorsal root ganglion (DRG) stimulation at L1 and S2. Both patients had reduction (> 70%) in their pain with lasting (> 6 months) relief. CONCLUSIONS: CRPS can lead to changes in phenotype and function of the DRG. Stimulation of the DRG can block the
