Mucoadhesive hyaluronic acid composites for prolonged gastric retention.

The interaction between mucoadhesive materials and the mucin layer is a critical factor in the development of advanced drug delivery systems designed to enhance site-specific targeting and prolong gastrointestinal residence time. Among these materials, hyaluronic acid (HA), a naturally occurring, linear glycosaminoglycan composed of repeating disaccharide units of β(1 → 3)-D-glucuronic acid and β(1 → 4)-N-acetyl-D-glucosamine, has emerged as a promising mucoadhesive agent due to its unique physicochemical properties. Despite the inherent electrostatic repulsion between the anionic carboxylate

Potent gastric acid inhibition in Helicobacter pylori eradication.

At present, antisecretory drugs--foremost among them the proton pump inhibitors (PPIs)--represent a keystone in Helicobacter pylori eradication therapy. The present article shall first compare the role of PPIs as compared with histamine H2 receptor antagonists, both of them in the role of antibiotic-associated antisecretory therapy, and shall then address the contribution of each of the various PPIs that have been developed until the present time to the H. pylori eradication therapies. In summary, it may be concluded that PPIs are more effective overall than H2 receptor antagonists when the tw

Gastric acid secretion in cyclooxygenase-1 deficient mice.

BACKGROUND: Constitutive cyclooxygenase-1 enzyme synthesizes prostaglandins which are thought to play an important role in the functional integrity of the stomach gastric mucosa. Recently, it was shown that cyclooxygenase-1 deficient mutant mice did not develop spontaneous gastric pathology and appear less sensitive to indomethacin-induced gastric damage. AIM: To investigate gastric acid secretion in cyclooxygenase-1 deficient mutant mice. METHODS: The basal and histamine or isobutyl methylxanthine-stimulated acid secretion in stomachs of cyclooxygenase-1 deficient homozygous mice and the ef

Helicobacter pylori infection and chronic gastric acid hyposecretion.

BACKGROUND & AIMS: We have identified a subgroup of Helicobacter pylori-infected subjects with low or absent gastric acid output. The aim of this study was to document the morphological and functional abnormalities in these subjects and to assess the effect of eradicating the infection. METHODS: The 16 hypochlorhydric subjects (6 men) had a mean age of 55 years (range, 36-79 years). They underwent a 14C-urea breath test, H. pylori serology, fasting gastrin, gastric autoantibodies, gastroscopy with antral and body biopsies, and measurement of peak acid output to pentagastrin (PAO(PG)). Their h

Gastric acid secretion and enteric infection in Bangladesh.

In developing countries many enteric infections are caused by acid-sensitive pathogens. Failure of the gastric acid barrier to infection has been reported in cholera but gastric acid secretion has been little studied in other enteric infections. We therefore studied basal and stimulated gastric acid in 185 Bangladeshi men admitted to hospital for the treatment of enteric infection. Patients with dysentery (amoebiasis, n = 24 and shigellosis, n = 19) and culture-negative diarrhoea (n = 69) had similar mean gastric acid levels (basal, 3-5 mmol/h; stimulated, 11-17 mmol/h), which remained stable

Sauvagine: inhibition of gastric acid secretion in rats.

Sauvagine (SV) powerfully inhibits gastric acid secretion by both the central and peripheral mechanisms. We examined whether adrenergic mechanisms or prostaglandin pathways might mediate the inhibitory action of SV on acid production in pylorus-ligated rats. Adrenalectomy altered the extent of the SV suppressive effect, suggesting that adrenal-derived substances participate in the action of the peptide. Blockade of adrenergic receptors by propranolol did not modify the antisecretory effect of SV, while the alpha-adrenergic antagonist, phentolamine, and the dopaminergic antagonist, haloperidol,

Gastric acid hypersecretion in refractory gastroesophageal reflux disease.

We prospectively evaluated gastric acid output (mEq/h), gastric volume output (ml/h), ambulatory 24-h esophageal pH monitoring, and the endoscopic appearance of the esophagus in 23 patients undergoing treatment of chronic long-standing pyrosis. Twelve of these 23 individuals (52%) remained symptomatic after 3 mo of standard antisecretory treatment with ranitidine, 150 mg twice daily. When compared with initial responders, those patients who did not experience complete symptomatic relief on therapy had significantly higher basal acid output (p less than 0.001), basal volume output (p less than

Omeprazole: a new approach to gastric acid suppression.

Omeprazole, one of a new group of antisecretory drugs, is a substituted benzimidazole that does not exhibit the anticholinergic or histamine H2 antagonistic properties of drugs such as cimetidine. This agent suppresses gastric acid secretion by inhibiting the proton pump mechanism, thereby blocking the final step of acid secretion. Omeprazole is significantly more effective than the histamine H2 receptor antagonists in eliminating acid secretion; thus, it may be beneficial in patients who are resistant to these agents. Omeprazole is indicated for severe erosive esophagitis, gastroesophageal re

Role of blood flow in gastric acid secretion.

The relationship between gastric acid secretion and blood flow has been investigated with a variety of different blood flow techniques including aminopyrine clearance, hydrogen gas clearance, intravital microscopy, laser-Doppler flowmetry, radioactive microspheres, and the elimination of inert gases. The most commonly used technique, aminopyrine clearance, predicts that increasing acid secretion is accompanied by a parallel increase in blood flow. However, the efficiency of clearance of aminopyrine is low in the nonsecreting stomach and increases as secretion rate increases. This precludes the

Effect of acid inhibition on gastric endocrine cells.

There is increasing evidence that the numbers of antral G cells and of fundic argyrophil (ECL) cells are influenced by agents that inhibit gastric acid secretion. Antral G cells increase in states of achlorhydria in man and animals provided atrophic antral gastritis is absent. In rats, treatment with substituted benzimidazoles like omeprazole and BY 308 increase G-cell densities dose-dependently. Even high doses of histamine H-2 antagonists and antacids increase antral G-cell densities. In contrast to G cells, antral D cells decrease in these instances. Fundic ECL cells increase in all experim

Cerebroventricular bombesin inhibits gastric acid secretion in dogs.

Bolus injections of bombesin into a lateral cerebral ventricle of conscious beagle dogs inhibited pentagastrin-stimulated gastric acid secretion from both the innervated stomach (gastric fistula) and the vagally denervated Heidenhain pouch in a dose dependent manner. Bombesin (300 ng/kg), injected into the lateral ventricle, inhibited acid secretion from the gastric fistula and Heidenhain pouch by 66% +/- 5% and 93% +/- 4%, respectively. Central administration of bombesin also suppressed acid secretion in response to a meal by 52% +/- 15%; postprandial gastrin response was not affected. By con

Gastric acid secretion in patients with typhoid fever.

Gastric secretory studies were carried out in the following groups of patients: group A, 30 Salmonella typhi positive enteric patients, group B, 15 non-enteric fever patients and group C, 20 healthy controls. Patients with typhoid were divided into two subgroups on the basis of the severity of the disease: group A1 (17) consisted of uncomplicated cases of enteric fever and group A2 (13) comprised of those who had associated complications. During convalescence both groups of typhoid patients showed significantly lower basal acid output (BAO) and maximal acid output (MAO) levels compared with co

Inhibitory effect of aprotinin on gastric acid secretion.

Aprotinin was found to inhibit the amplitude of electrically stimulated contractions of the isolated guinea pig ileum with an ID50 of 6.75 X 10(-6) M. Aprotinin inhibited pentagastrin-stimulated gastric acid secretion in the rat, cat, and pig after oral or intraventricular administration. No inhibition of gastric acid secretion was seen after intravenous infusion of aprotinin. When segments of antrum, duodenum, and ileum were incubated in vitro with increasing concentrations of aprotinin, increasing amounts of somatostatin were released from the organs. It is suggested that aprotinin's effects

Fasting serum gastrin and basal gastric acid secretion.

The gastric secretion of acid was examined 30 minutes basally (BAO) and in response to stepwise increasing doses of pentagastrin in subjects with (n = 51) and without (n = 40) peptic ulcer disease. None of them showed basal anacidity. Before insertion of the gastric tube, blood was taken for radioimmunological determination of the serum gastrin concentration (SG). A significant positive correlation was found between SG and BAO in the subjects without ulcer. This was mainly due to a close correlation in 20 healthy young volunteers. When BAO and SG was expressed as proportions of calculated maxi

Alpha 1-acid glycoprotein in gastric cancer juice.

Gastric juice was neutralized (nGJ) in vivo by 80 ml of a phosphate buffer containing radiolabelled vitamin B12 as dilution indicator. Unprocessed nGJ was analyzed in the double gel diffusion technique for the presence of serum proteins using monospecific antisera. Alpha1-Acid glycoprotein (AGP) was found in a high incidence (36 out of 38 subjects) in nGJ of gastric cancer patients. AGP was also observed less frequently in nGJ of patients with Billroth II resections (6/15), metaplasia (11/52), gastric ulcer (3/24), chronic atrophic gastritis (2/26) and chronic gastritis (3/63). AGP was absent

Prostaglandin E2 and gastric acid secretion in man.

1. Extracts of human stomach homogenized in Krebs solution had more PGE(2)-like activity than tissue homogenized in acid/ethanol or in the presence of indomethacin, indicating that the tissue can synthesize PGE(2).2. The distribution and synthesis of PGE(2)-like substance in human stomach was determined by extracting frozen sections cut parallel to the mucosal surface. Peak levels usually occurred at a depth of 0-600 mum in the mucosa.3. Small amounts of a PGE(2)-like substance were present in basal gastric juice, and its concentration was usually even lower in secretion stimulated by pentagas
