Routine magnifying endoscopy mode improved the diagnostic efficacy of opportunistic screening for early upper gastrointestinal neoplasms: a prospective, randomized controlled study.

OBJECTIVE: White light endoscopy (WLE) is an important method for upper gastrointestinal cancer screening. However, studies have demonstrated that magnifying endoscopy with image-enhanced endoscopy (ME-IEE) achieves a significantly higher detection rate of early cancer compared with WLE. Current research has primarily focused on evaluating the diagnostic performance of ME-IEE, while evidence regarding its feasibility as a routine opportunistic screening mode for early upper gastrointestinal cancer in daily clinical practice remains limited. METHODS: A prospective, single-center, randomized con

Cavernous Hemangioma of the Anal Canal Mimicking a Neoplasm: A Rare Cause of Lower Gastrointestinal Bleeding Successfully Treated With Transanal Excision.

Cavernous hemangiomas of the anal canal are exceptionally rare vascular malformations that may present with chronic rectal bleeding and are often misdiagnosed as hemorrhoids or neoplastic lesions. We report the case of a 75-year-old female patient with a one-year history of painless rectal bleeding and a soft polypoid lesion located 2 cm from the anal verge, within the anterior wall of the anal canal. Magnetic resonance imaging (MRI) revealed a hyperintense pseudonodular lesion consistent with a vascular malformation. Histopathological examination after biopsy confirmed the diagnosis of cavern

True smooth muscle neoplasms of the gastrointestinal tract: morphological spectrum and classification in a series of 85 cases from a single institute.

BACKGROUND AND AIM: True smooth-muscle neoplasms of the GI tract have been only rarely studied in the KIT era. Their incidence among other GI mesenchymal tumours and their clinicopathological spectrum have not been sufficiently analysed. MATERIALS AND METHODS: We reviewed all GI mesenchymal lesions at the Pathology Institute of the Nuremberg Clinic Centre from 1994 through 2005. RESULTS: Among 262 lesions, there were 142 GISTs (54%) and 85 true smooth muscle neoplasms (32%). Smooth muscle neoplasms comprised 72 polypoid leiomyomas (78%, 5 oesophageal and 67 colorectal), 10 intramural leiomyo

Monoclonal small (well-differentiated) lymphocytic proliferations of the gastrointestinal tract resembling lymphoid hyperplasia: a neoplasm of uncertain malignant potential.

Three histologically benign-appearing or diagnostically equivocal small lymphocytic proliferations of the gastrointestinal tract were examined by fresh-frozen section immunohistologic techniques. In one case, a dense infiltrate in the small intestine, consisting of small lymphocytes with round nuclei, was limited almost entirely to the mucosa. In another case, a localized colonic polyp was formed by mucosal and submucosal lobules of benign-appearing lymphoid aggregates with centrally located germinal centers. The third case, a penetrating gastric ulcer, was surrounded by histologically hyperpl

Efficacy of artificial intelligence-assisted upper gastrointestinal endoscopy for neoplasm detection: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND AND AIMS: EGD remains the primary diagnostic modality for evaluating lesions of the upper gastrointestinal (UGI) tract. This meta-analysis assessed the diagnostic performance of artificial intelligence-EGD (AI-EGD) compared with conventional EGD (Co-EGD) for the detection of UGI neoplasms. METHODS: A systematic search of major bibliographic databases identified randomized controlled trials (RCTs) comparing AI-EGD and Co-EGD in adults undergoing EGD. The primary outcome was neoplasm detection rate (NDR) per patient. Secondary outcomes included NDR per lesion and stratification by le

Endoscopic ultrasound FNA: An illustrated review of spindle cell neoplasms of the upper gastrointestinal tract including a novel case of gastric plexiform fibromyxoma.

Plexiform fibromyxoma (PF) is a recently-described and rare mesenchymal neoplasm of the gastric wall. A few small case series reports of this spindle cell entity exist in the surgical pathology literature, but to our knowledge no prior endoscopic ultrasound guided fine needle aspiration cytology examples have been reported. In clinical practice, mural gastrointestinal (GI) lesions are often initially evaluated by endoscopic ultrasound guided (EUS) fine needle aspiration (FNA). In addition, newer EUS fine needle biopsy techniques also allow for reliable retrieval of core tissue samples with int

Efficacy of a real-time intelligent quality-control system for the detection of early upper gastrointestinal neoplasms: a multicentre, single-blinded, randomised controlled trial.

BACKGROUND: Oesophagogastroduodenoscopy (OGD) quality and identification of the early upper gastrointestinal (UGI) neoplasm play an important role in detecting the UGI neoplasm. However, the optimal method for quality control in daily OGD procedures is currently lacking. We aimed to evaluate the efficacy of a real-time intelligent quality-control system (IQCS), which combines OGD quality control with lesion detection of early UGI neoplasms. METHODS: We performed a multicentre, single-blinded, randomised controlled trial at 6 hospitals in China. Patients aged 40-80 years old who underwent pain

New insights in gastrointestinal "pediatric" neoplasms in adult patients: pancreatoblastoma, hepatoblastoma and embryonal sarcoma of the liver. A practical approach by GIPPI-GIPAD Groups.

Pediatric solid neoplasms are rare and very different from those observed in adults. The majority of them are referred to as embryonal because they arise as a result of alterations in the processes of organogenesis or normal growth and are characterized by proliferation of primitive cells, reproducing the corresponding tissue at various stages of embryonic development. This review will focus on embryonal gastrointestinal pediatric neoplasms in adult patients, including pancreatoblastoma, hepatoblastoma, and embryonal sarcoma of the liver. Although they are classically considered pediatric neop

Early cardia neoplasms in the upper gastrointestinal tract have a higher detection rate under sedation: a retrospective cohort study based on the Chinese population.

BACKGROUND: Esophagogastroduodenoscopy (EGD) is a standard method for upper gastrointestinal (UGI) tumor screening. However, differing opinions persist regarding the impact of sedation on the detection rate of early adenocarcinoma of the gastric cardia (EGCA). This study evaluated the effect of sedation on the detection of EGCA, other upper digestive tract early cancer (UDTEC) and microscopic upper gastrointestinal (UGI) lesion detection. METHODS: We retrospectively analyzed 313,503 symptomatic and screening patients undergoing EGD (2018-2025) at Zhejiang Provincial Hospital of Traditional Ch

Mixed Ovarian Neoplasms With Gastrointestinal-type Mucinous and Mullerian Epithelial Components: A Rare Group of Tumors Demonstrating the Phenotypic Plasticity of the Mullerian Epithelial Cell.

Primary mucinous ovarian neoplasms, gastrointestinal-type (GI-type), are composed of mucin-producing tumor cells resembling intestinal goblet cells or gastric foveolar epithelium. In contrast to seromucinous tumors, which exhibit endocervical-type mucinous differentiation and are thought to be derived from endometriosis, the cell/tissue-of-origin of most GI-type mucinous ovarian tumors is unknown. We identified 8 GI-type mucinous ovarian tumors (cystadenomas, n=4; borderline tumor/carcinoma, n=4) with spatially distinct areas that showed morphologic features of Mullerian-type epithelial differ

Effect of a deep learning-based system on the miss rate of gastric neoplasms during upper gastrointestinal endoscopy: a single-centre, tandem, randomised controlled trial.

BACKGROUND: White light endoscopy is a pivotal first-line tool for the detection of gastric neoplasms. However, gastric neoplasms can be missed during upper gastrointestinal endoscopy due to the subtle nature of these lesions and varying skill among endoscopists. Here, we aimed to evaluate the effect of an artificial intelligence (AI) system designed to detect focal lesions and diagnose gastric neoplasms on reducing the miss rate of gastric neoplasms in clinical practice. METHODS: This single-centre, randomised controlled, tandem trial was done at Renmin Hospital of Wuhan University, China. W

Evolution of Gastrointestinal Inflammatory Diseases and Neoplasm Burden in Super-Elderly Populations: Integrated GBD 2023, CHARLS, and CLHLS Analyses of China and G20 Countries.

BACKGROUND: Rapid population aging has profoundly altered the epidemiological profile of gastrointestinal diseases in individuals aged ≥75 years. Although enteric infections (EI) have been effectively controlled through public health interventions, the burden of gastrointestinal inflammatory diseases (GIID) and neoplasms (GIN) continues to evolve. Systematic comparisons of temporal trends, sex differences, and age-subgroup patterns between China and G20 nations remain limited. METHODS: Using GBD 2023 data, we analyzed incident cases, age-standardized rates (ASR), and Disability-Adjusted Life

Sodium-glucose cotransporter-2 inhibitors and gastrointestinal neoplasm risk in type 2 diabetes: a systematic review and meta-analysis of randomized controlled trials.

The potential carcinogenic effects of sodium-glucose cotransporter 2 (SGLT2) inhibitors in patients with type 2 diabetes mellitus (T2DM) remain controversial, particularly regarding site-specific gastrointestinal (GI) neoplasms. This systematic review and meta-analysis aimed to determine the relationship between SGLT2 inhibitors and the risk of GI neoplasms in patients with T2DM. We searched PubMed, EMBASE, Cochrane CENTRAL, Scopus, and Web of Science through March 17, 2025, for RCTs in T2DM comparing SGLT2 inhibitors with placebo or active comparators. Two reviewers independently screened stu
