Prevalence and progression of erosive tooth wear among children and adolescents in a Swedish county, as diagnosed by general practitioners during routine dental practice.

OBJECTIVES: To investigate the prevalence and four-year progression of erosive tooth wear (ETW) recorded in general dental practice, and to evaluate the usefulness of a simplified grading scale. METHODS: Four cohorts (aged 3, 7, 11 and 15 years at baseline; n = 735) were followed from 2008 to 2012 during their routine dental examinations. Grading of ETW was performed on permanent upper incisors and first molars, using the scales of Johansson et al. 1996 and Hasselkvist & Johansson 2010. RESULTS: Valid data were available for 641 individuals, 7-19-years of age, of whom 326 had data allowing a

Caregiver-Reported Preventive Oral Health Practices and Satisfaction With Dental Treatment Under General Anaesthesia Among Adolescents and Adults With Intellectual Disabilities: A Cross-Sectional Study.

BACKGROUND: People with intellectual disabilities depend on family caregivers for daily oral care, yet caregiver-reported practices are rarely examined alongside satisfaction with dental treatment under general anaesthesia. METHOD: Cross-sectional survey of 62 family caregivers of adolescents and adults with intellectual disabilities (median age 24.5 years, range 16-74) treated under general anaesthesia at Gazi University, Ankara, Türkiye. An investigator-developed questionnaire was administered face to face. Scores were analysed using analysis of variance and linear regression. RESULTS: Rep

The effects of a 0.12% chlorhexidine-digluconate-containing mouthrinse versus a placebo on plaque and gingival inflammation over a 3-month period. A multicentre study carried out in general dental practices.

Several previous studies have evaluated the effects of 0.12% chlorhexidine digluconate (ChD) mouthrinses on plaque and gingival inflammation. However, previously, none have been based in general dental practices. The aim of this study was to evaluate the potential to conduct controlled periodontal clinical trials in co-operation with general dental practitioners (gdps). The project took place in 5 general dental practices in the South of England. 121 healthy subjects (24 at 4 sites and 25 at the 5th), aged 18-65 years, mean 35 +/- 12) years participated in a double-blind, randomised study duri

General Dentistry on Life Support: The Overspecialization of the Profession.

ISSUE: General dentists are trained to identify and treat the most common oral conditions and their complications, including dental caries lesions, by restoring tooth structure and function and replacing missing teeth, as well as by treating gingival diseases. However, many senior undergraduate students and junior dentists believe they lack confidence to practice comprehensive clinical dentistry; they may avoid surgical endodontic procedures, orthodontic care, and implant-related cases, for example. The answer, they believe, lies in the imminent need to pursue a specialization to have a succes

How general dentists could manage a patient with oral lichen planus.

BACKGROUND: The literature hardly contains information on how patients suffering from oral lichen planus could be managed by dentists. MATERIAL AND METHODS: Based on the limited available literature and particularly on the long-term clinical and histopathological experience of one of the authors, suggestions on how dentists could manage patients with oral lichen planus have been put forward. RESULTS: In most cases, the dentist should be able to establish a correct diagnosis. Occasionally, the dentist may call upon a specialist, usually an oral medicine specialist or an oral and maxillofacial

Dental management of the elderly patient.

UNLABELLED: The dental management of the elderly individual is becoming more of an issue in both general dental practice and specialist prosthodontic practice. This is because, in general, the population is living longer and often elderly patients are retaining their natural teeth into their old age. In addition, there are often related significant medical, social and oro-dental issues that require consideration. CLINICAL RELEVANCE: The dental management of the elderly patient is an increasing problem. This paper seeks to consider the main relevant issues pertaining to this important subject

Management of patients with reduced oral aperture and mandibular hypomobility (trismus) and implications for operative dentistry.

Reduced oral aperture and mandibular mobility/trismus are relatively common conditions that can be encountered in patients attending general dental practice, community dental practice and district general or dental teaching hospitals. All dental specialties may see patients with these conditions, and regardless of which environment or specialty, both patient and clinician may experience significant problems. The purpose of this opinion-based paper is to identify and review the causes of such conditions, to review the development of problems encountered for patients and clinicians, and to ident

Seeking to achieve periodontal health in practice using the Healthy Gums DO Matter toolkit.

The pressures on the dental team to successfully manage and treat periodontal diseases in general practice are ever-mounting. Dental practitioners have a duty of care to screen, investigate, diagnose, risk assess and appropriately treat periodontal diseases using evidence-based practice. Once patients are stable, they must continue to monitor and risk assess patients in the maintenance phase. Managing periodontal diseases in general practice is challenging and this has been reflected in the rising trend in periodontal litigation. General practitioners see the complete spectrum of patients; fro

Use of the dry angle isolation adjunct in general dentistry: a clinical approach.

The dry angle is a thin, triangle-shaped piece of moisture-absorbing cellulose that is placed over the buccal mucosa. It absorbs parotid gland saliva and some mandibular saliva, and helps to maintain a dry working field. It is an adjunct to rubber dam isolation and can facilitate dry field isolation in ways that cotton rolls cannot. Its thinness allows it to be placed in the thin space between the maxillary second molars and the ramus aspect of the mandible and sometimes it may be the most practical tool for isolating second molars for bonding or cementation procedures. When slightly moistened

The provision of occlusal splints in primary dental care.

UNLABELLED: Occlusal splints (Michigan splints, night/bite guards or bite-raising appliances) can be an effective, inexpensive and reversible treatment for a wide range of dental problems. OBJECTIVE: The aim of this study was to analyse retrospectively the provision of occlusal splint ('Michigan' type) in general dental practice, following a prescription by a restorative dental consultant. METHOD: One hundred patients were recruited from consultant clinics in a department of conservative dentistry during 1995 and 1996. All patients were prescribed a maxillary, full-coverage, heat-cured, acry

Caring for cancer patients in the general dental office.

Modern therapeutic modalities and emphasis on early detection have made oral cancer a treatable, and in many cases, a curable disease. The role of the dentist in cancer patient management is two-fold. Early detection of oral lesions during routine dental examination has been shown to be a significant factor in cancer diagnosis. The dentist's other role comes after cancer treatment, specifically therapeutic radiation. Ionizing radiation can have permanent effects on both hard and soft tissues. Prescription and use of fluoride gel in topical applicators can aid in assuring oral health for post-c

General medicine and surgery for dental practitioners. Part 3 - skin disorders part A.

Skin disorders are potentially important to dentists in diverse ways. The skin disease itself might have oral manifestations, and drugs used to treat skin disorders may impact on dental management. This first paper considering skin disorders examines points to look out for in the history and considers specific groups of conditions. Further conditions will be discussed in part B.

Malignant hyperthermia and its implications in general dentistry.

Malignant hyperthermia (MH) is an uncommon and potentially life-threatening pharmacogenetic disorder. This abnormality in muscle metabolism can be triggered by a variety of agents (particularly general anesthetics and stress), resulting in a rapid heart rate increase, muscle rigidity, acidosis, temperature elevation, rhabdomyolysis, and renal failure. Immediate discontinuing of triggering agents, oxygenation, cooling, and dantrolene are necessary to treat an episode. MH-susceptible patients often indicate a positive family history of experiencing an adverse event during anesthesia. Few diagnos

[Treatment of craniomandibular dysfunction by general practitioners].

A general practitioner diagnoses--after a screening--the patient who suffers from a dysfunction. Then he has to decide whether to advise either a further treatment on the basis of basic knowledge of the most frequent TMJ-dysfunctions or to refer to other disciplines. In the treatment by the general practitioner a careful and thorough screening is of primary importance. Finally, the therapy by means of a 'splint' often yields good results.

Practical implementation of caries disease management in the era of CAMBRA: a guide for the general dentist.

Although the philosophy of caries management by risk assessment (CAMBRA) is widely accepted as an essential part of comprehensive disease management, its implementation in daily clinical practice has historically been hindered by a lack of concise guidance that bridges the gap between scholarly research and the logistics of patient care. With the aim of guiding comprehensive disease management for dental caries by integrating available evidence-based systems such as the American Dental Association Caries Classification System (ADA CCS) and caries risk assessment (CRA), this review presents cli
