Randomized Clinical Trial of Geriatric Comanagement Versus Geriatric Assessment-Guided Supportive Care for Older Patients With Head and Neck Cancer.

BACKGROUND: Geriatric assessment (GA) can be used to identify patient-specific deficits and facilitate interventions to improve patient outcomes. In this randomized pilot clinical trial, we hypothesized that geriatric assessment with or without geriatrician comanagement can reduce the risk of hospitalization for older adults with head and neck cancer receiving concurrent chemoradiotherapy (CRT). PATIENTS AND METHODS: Patients aged ≥65 years with newly diagnosed or postoperative mucosal head and neck squamous cell carcinomas were eligible for enrollment and received 6 to 7 weeks of CRT. All pa

Correlation of the Geriatric Assessment with Overall Survival in Older Patients with Cancer.

AIMS: Global guidelines recommend that all older patients with cancer receiving chemotherapy should undergo a geriatric assessment. However, utilisation of the geriatric assessment is often constrained by its time-intensive nature, which limits its adoption in settings with limited resources and high demand. There is a lack of evidence correlating the results of the geriatric assessment with survival from the Indian subcontinent. Therefore, the aims of the present study were to assess the impact of the geriatric assessment on survival in older Indian patients with cancer and to identify the fa

Using Information Technology in the Assessment and Monitoring of Geriatric Oncology Patients.

Purpose of reviewOlder adults with cancer have complex medical needs and often experience higher rates of treatment-related toxicities compared to their younger counterparts. The advent of health information technologies can address multiple gaps in the care of this population. We review the role of existing and emerging technologies in facilitating the use of comprehensive geriatric assessment (CGA) in routine clinics, promoting symptom reporting, and monitoring medication adherence.Recent findingsIncreasingly, studies demonstrate the feasibility of implementing electronic CGA in routine onco

Geriatric assessment of older patients with cancer in Australia--a multicentre audit.

OBJECTIVE: The aim of this study is to determine the frequency of geriatric assessment in patients aged over 70 years in Australian medical oncology clinics. MATERIAL AND METHODS: This was a multicentre audit in two parts: a retrospective file review of initial consultations with an oncologist and prospective audit of case presentations at multidisciplinary meetings (MDMs). Patients aged over 70 years presenting to a medical oncology clinic or being discussed at an MDM were eligible. Data was collected at six oncology centres in Victoria, NSW and Canberra from October 2009 to March 2010. RES

Assessment of cognitive deficit in geriatric patients. A study of physician behavior.

A crucial factor in promoting a vigorous quality of life in the aging population is assessment and treatment of cognitive deficits. A very high percentage of delirium and at least 20% of dementia is eminently treatable. This study of patients over 65 years of age admitted to a university general hospital reveals that 79% of cognitive deficits were missed by the examining physicians. Furthermore, in 394 examinations of 165 patients, only four mental status examinations were recorded. A clear-cut cognitive deficit on admission was predictive of later acute episodes of confusion. The global techn

Geriatric assessment in older patients with a hematologic malignancy: a systematic review.

The aim of this systematic review is to give an update of all currently available evidence on the relevance of a geriatric assessment in the treatment of older patients with hematologic malignancies. A systematic search in MEDLINE and EMBASE was performed to find studies in which a geriatric assessment was used to detect impaired geriatric domains or to address the association between geriatric assessment and survival or clinical outcome measures. The literature search included 4,629 reports, of which 54 publications from 44 studies were included. Seventy-three percent of the studies were publ

The Effect of A Geriatric Assessment on Treatment Decisions for Patients with Lung Cancer.

BACKGROUND: Decision-making for older patients with lung cancer can be complex and challenging. A geriatric assessment (GA) may be helpful and is increasingly being used since 2005 when SIOG advised to incorporate this in standard work-up for the elderly with cancer. Our aim was to evaluate the value of a geriatric assessment in decision-making for patients with lung cancer. METHODS: Between January 2014 and April 2016, data on patients with lung cancer from two teaching hospitals in the Netherlands were entered in a prospective database. Outcome of geriatric assessment, non-oncologic interve

Preoperative assessment of the older surgical patient: honing in on geriatric syndromes.

Nearly 50% of Americans will have an operation after the age of 65 years. Traditional preoperative anesthesia consultations capture only some of the information needed to identify older patients (defined as ≥65 years of age) undergoing elective surgery who are at increased risk for postoperative complications, prolonged hospital stays, and delayed or hampered functional recovery. As a catalyst to this review, we compared traditional risk scores (eg, cardiac-focused) to geriatric-specific risk measures from two older female patients seen in our preoperative clinic who were scheduled for electiv

[Evaluation and risk assessment of geriatric ankle fractures? : What must be considered?].

The frequency of fractures in old age is steadily increasing including geriatric ankle fractures. These injuries are associated with increased morbidity and prolonged hospitalization. The consequences of courses prone to complications represent a relevant socioeconomic factor and the goal of treatment is to preserve the patient's mobility and quality of life. To ensure optimal perioperative care, the identification of geriatric patients, evaluation of the functional status and existing risk factors with subsequent interdisciplinary treatment are required. Various surgical procedures are availa

Extending geriatric care beyond traditional settings: lessons from a mobile assessment program.

BACKGROUND: As populations age, changing living conditions and declining self-advocacy often create barriers to essential care. Mobile Geriatric Teams (MGTs) offer a potential solution by providing assessments within a patient's regular environment, potentially reaching underdiagnosed and undertreated individuals who struggle to access routine healthcare. The aim of this study was to evaluate the clinical impact and reach of MGTs in the Northern District of Israel. Specifically, we aimed to assess the extent of MGT-initiated Comprehensive Geriatric Assessments and compare therapeutic and diagn

Management of multiple myeloma in older adults: Gaining ground with geriatric assessment.

Multiple myeloma increases in incidence with age. With the aging of the population, the number of cases of multiple myeloma diagnosed in older adults each year will nearly double in the next 20years. The novel therapeutic agents have significantly improved survival in older adults, but their outcomes remain poorer than in younger patients. Older adults may be more vulnerable to toxicity of therapy, resulting in decreased dose intensity and contributing to poorer outcomes. Data are beginning to emerge to aid in identifying which individuals are at greater risk for toxicity of therapy; comorbidi

Screening for cognitive impairment in an Australian aged care assessment team as part of comprehensive geriatric assessment.

Accurate detection of mild cognitive impairment (MCI) is important to stratify and address risk. Yet, few short cognitive screening instruments are validated for this. . In Australia, all clients referred to an Aged Care Assessment Team (ACAT) receive comprehensive geriatric assessment (CGA) including the Standardized Mini-Mental State Examination (SMMSE). We compared the accuracy of the quick mild cognitive impairment (Qmci) screen to the SMMSE in 283 participants: 195 with dementia, 47 with MCI, and 41 with subjective cognitive decline (SCD) in an Australian community-based ACAT. Both had si

The Role of Geriatric Syndromes in Kidney Transplant Decision-Making: A Domain-Based Analysis of Comprehensive Geriatric Assessment in Older Adults.

BACKGROUND: As the population ages, kidney transplantation (KT) is increasingly considered for older adults with advanced chronic kidney disease (CKD). However, frailty, cognitive impairment, and malnutrition complicate transplant decision-making. A Comprehensive Geriatric Assessment (CGA) offers a multidimensional approach, but the relative contribution of individual CGA domains to transplant eligibility and receipt remains unclear. METHODS: We conducted a retrospective observational study of 164 older adults with advanced CKD seeking KT and underwent CGA by a geriatrician at an urban academ

Association between depression scores and comprehensive geriatric assessment and frailty in geriatric outpatients with somatic complaints: an observational cross-sectional study.

BACKGROUND: Depression is the most common psychiatric problem in older individuals. In some countries, the common approach is to ignore psychiatric disorders. This study aimed to reveal the importance of newly diagnosed high depression scores in the geriatric population admitted to outpatient clinics with somatic complaints. METHODS: Patients who did not have a previous diagnosis of a psychiatric disorder and were not receiving treatment were included in the study.
