Post-transplant lymphoproliferative disorders presenting as gingival overgrowth in patients immunosuppressed with ciclosporin. A report of two cases.

BACKGROUND: Post-transplant lymphoproliferative disorder (PTLD) can occur in patients maintained on immunosuppressive therapy following transplantation. This paper describes two cases of PTLD occurring in gingival tissues, in patients receiving ciclosporin following cardiac transplantation. TREATMENT: The lesions were localised to gingival tissues, mimicking ciclosporin-induced gingival overgrowth. They were removed surgically and the ciclosporin dose reduced to help prevent recurrence. CONCLUSION: The importance of histopathological examination of all tissue removed during routine gingivect

The apically repositioned flap in tooth exposure.

Methods of exposing impacted teeth in order to bring them into the line of the arch include gingivectomy, the apically repositioned flap and closed eruption techniques. These procedures aim to facilitate the eruption of the impacted tooth with a minimum of disruption or damage to the tooth itself or adjacent structures. The aim of this paper is to discuss the various surgical methods of exposing impacted teeth and to help to identify where the use of the apically repositioned flap is indicated. Clinical examples are presented and a surgical method for carrying out this procedure recommended.

Primary extramedullary plasmacytoma in cyclosporine-induced gingival overgrowth. A case report.

An unusual solitary extramedullary plasmacytoma developed in the gingiva of a 28 year-old male renal transplant patient with cyclosporine (CyA)-induced gingival overgrowth. On initial presentation, there was no suspicion of a tumor within the lesion. One year later, however, the lesion had increased progressively in size, and its surface was ulcerated. At this time, the lesion was excised by gingivectomy, and histopathological examination revealed plasmacytoma. CyA was discontinued and the patient has been closely observed for 4 years with no recurrence of the gingival lesion.

Plasma Cell Gingivitis With Cheilitis in an Adolescent: A Case Report.

Plasma cell gingivitis (PCG) is an uncommon plasma-cell-rich inflammatory disorder of the gingiva that may clinically resemble plaque-associated enlargement, granulomatous disease, hematologic disease, or plasma-cell neoplasia. Its association with cheilitis is rare, and diagnosis requires clinicopathologic correlation. An 18-year-old female presented with a 1-year history of progressive gingival enlargement and upper-lip swelling. Intraoral examination showed generalized erythematous marginal and papillary gingival enlargement with loss of stippling, bleeding on probing, pseudopocketing, and

Local Mucosal Flap for the Treatment of Gingival Defect After Gingival Fibromatosis Excision.

Gingival fibromatosis (GF) is a rare condition of fibrous enlargement of the gingivae causing functional or aesthetic problems. We report a case of localized GF in a 51-year-old healthy male patient who presented in our department with localized gingival enlargement. We performed gingivectomy and restored the defect with a novel local transpositional mucosal flap with excellent functional and aesthetic results. This type of intervention is accompanied by short surgical time, provides predictable results, and should be considered in adult patients with large defects from sizable lesions.

Management of idiopathic gingival fibromatosis: report of a case and literature review.

Gingival hyperplasia is a rare condition and is of importance for cosmetic and mechanical reasons. Idiopathic gingival fibromatosis, a benign, slow-growing proliferation of the gingival tissues, is genetically heterogeneous. The enlargement is most intense during the eruption of the primary and permanent teeth, and minimal or nondetectable growth is observed in adults. The genetic aspect, clinical feature, histopathology, immunohistochemistry, and treatment aspects are reviewed. The purpose of this paper was to report a case of idiopathic gingival fibromatosis in a 13-year-old female who had a

Soft-tissue plastic surgery.

Soft-tissue plastic surgery was previously referred to as mucogingival surgery by Friedman (Tex Dent J 1957, 75:358-362). This term referred to any surgery "designated to preserve attached gingiva, to remove frena or muscle attachment, and to increase the depth of the vestibule." The aim of this type of surgery was to maintain an adequate mucogingival complex, focusing on the importance of the amount of attached gingiva. Soft tissue plastic surgery is important not only for reasons of health, but also for cosmetic purposes.

[Optimization of treatment hard tissue defects of the tooth located near gingiva].

Defects near gingiva--a common pathology of dental hard tissue, which adversely affects the health of marginal periodontium. Subgingival tooth cavity is the retention factor for parodontopatogenic microorganisms. Chronic inflammation in the marginal periodontium in response to microbial invasion may lead to local periodontitis (periodontal pocket/gingival recession). Treatment of defects near gingiva should include the removal of the main etiological factors of disease: supraocclusion, bad habits, normalization of individual oral hygiene. Substitution therapy is the defect filling. The most su

Establishing gingival symmetry when placing anterior direct nanohybrid composite restorations.

Restoring the beauty of patients' smiles requires clinicians to incorporate both tooth and gingival esthetics. When gingivectomies and crown-lengthening procedures are required, opting for direct nanohybrid composite restorations can help enable dentists to achieve harmonious blending of the gingival architecture and the anticipated restoration by allowing for tissue healing and subsequent modifications to the gingival portions of restorations. This article presents two cases in which anterior nanohybrid direct composite restorations provided a means to enhance tooth esthetics simultaneous to

Gingival enlargement unveiling sarcoidosis: Report of a rare case.

Sarcoidosis is classified as an acquired systemic granulomatous disease. Because of the fact that sarcoidosis affects multiple tissues and organs, it is characterized by many potential signs and symptoms, as well as by the presence of non-caseating granulomas in the organs involved. Although oral sarcoidosis is relatively rare, it may however, present in the oral cavity. This report presents a rare case of sarcoidosis with the initial presenting symptom as severe generalized gingival enlargement. The gingival enlargement was treated by gingivectomy. Tissue reaction involving an intraoral skin graft and CP titanium abutments: a clinical report.

Focal areas of gingival hyperplasia surrounding the transmucosal portions of titanium implants have been attributed to poor hygiene, lack of attached gingival tissues, and titanium allergy. Following mandibular vestibuloplasty and placement of a split-thickness skin graft, two of five patients developed persistent proliferation of the epithelial tissue surrounding endosseous CP titanium dental implants. In both circumstances, traditional gingivectomy procedures, chemotherapeutic agents, and aggressive oral hygiene measures failed to adequately control the hyperplastic response. Following repla

Hypoplasminogenemia with ligneous periodontitis: a failed local therapeutic approach.

BACKGROUND: Hypoplasminogenemia is a rare condition that is associated with ligneous conjunctivitis, a form of chronic conjunctivitis characterized by firm, fibrin-rich, pseudomembranous lesions on the tarsal conjunctivae and oral lesions. Pseudomembranes may develop on the gingivae, and there may be periodontal involvement. METHODS: Several therapeutic approaches have been developed to treat such patients, but they have had limited effect. We used gingivectomies, topical heparin, and corticosteroids to treat periodontal lesions in an 18-year-old girl. RESULTS: This approach had no benefit.

Generalised Gingival Fibromatosis and Hypertrichosis: A Rare Case of Syndromic Presentation.

Gingival fibromatosis is a benign oral condition characterised by a slow progressive, non-haemorrhagic, non-painful, local or generalised enlargement of the gingival tissues. Gingival fibromatosis may have a genetic basis and is frequently associated with systemic or extraoral findings. Hypertrichosis is a clinical condition characterised by excessive hair growth without androgenic stimulation. The present case report describes a case of a 16-year-old girl who presented with severe generalised gingival fibromatosis along with hypertrichosis, which was treated by gingivectomy, and uneventful po

Management of Chronic Inflammatory Gingival Enlargement: A Short Review and Case Report.

Inflammatory gingival enlargement, sometimes referred to as gingival hyperplasia or gingival hypertrophy, is an abnormal proliferation of gingival tissues caused by underlying inflammation. It might also be related to long-term periodontitis. Herein, we discuss the case of a young, otherwise healthy male patient wherein the anterior regions of both the upper and lower arches were affected by long-standing gingival growth. The overgrowth was removed, and an excellent aesthetic outcome was achieved, using a surgical procedure termed gingivectomy. After a 15-day follow-up period, the healing proc
