Gingival pedicle with split-thickness tunnel technique: A retrospective case series for single mandibular anterior recessions.

INTRODUCTION: Gingival recessions in the mandibular anterior sextant are a common clinical finding, but mucogingival treatment in this location is particularly challenging, due to several anatomical and surgical difficulties. In the present case series, a novel technique, called gingival pedicle with split-thickness tunnel (GPST), was retrospectively evaluated. CASE SERIES: Fifteen patients presenting with a single buccal RT1 or RT2 gingival recession of a depth of ≥3 mm in the mandibular anterior sextant were treated by means of the GPST technique. Clinical periodontal parameters at baseline

Clinical applications of mucogingival therapies utilizing adjunctive autologous blood products.

BACKGROUND: Mucogingival deformities are prevalent oral conditions and can result in esthetic compromise, dentinal hypersensitivity, and an increase in radicular caries rates. Mucogingival treatments address thin periodontal phenotype and/or gingival recession defects. Although many of these grafting procedures are predictable in improving soft-tissue quality and quantity around teeth and dental implants, patients often complain of discomfort at both the donor and recipient sites. Free gingival grafts and coronally advanced flaps alone or in combination with subepithelial connective tissue gra

Double papilla repositioned flap for the treatment of isolated recession - A case report.

Root coverage is achieved by many procedures like free gingival autografts, connective tissue grafts and pedicle grafts. Several studies state that root coverage using connective tissue grafts have high success rates but have disadvantages like creation of second surgical site and post-operative color harmony is less. Although Cohen and Ross reported more than 85% success in covering denuded roots, the degree of success varies among other clinicians. The double papilla pedicle graft has limited usefulness. The double papillae pedicle graft is most appropriate in those cases where esthetics dem

Double laterally-rotated bilayer flap operation for treatment of gingival recession.

BACKGROUND AND PURPOSE: Many root coverage techniques, such as lateral pedicle flap, double papilla flap, free gingival graft, connective tissue graft, and guided tissue regeneration (GTR) procedure have been widely applied in clinical dental practice. Although they all provide various degrees of successful root coverage, some procedures are technique-sensitive and others require a 2-stage operation. METHODS: In this study, we describe a new technique with a double laterally-rotated bilayer flap for root coverage. Briefly, 2 partial-thickness flaps were prepared, one on each side of the gingi

A promising periodontal procedure for the treatment of adjacent gingival recession defects.

Various clinical reports on the reconstruction of gingival recession defects have been published in the past decade. Several techniques have been used to achieve root coverage, including creation of free gingival grafts, laterally positioned flaps or semilunar coronally positioned flaps, as well as guided tissue regeneration and connective tissue grafting. This article focuses on the importance of connective tissue grafting, combined with a recent approach known as the tunnel procedure, in managing gingival recession defects with a single operation. This procedure originated in 1985 with an en

Decision Tree for the Treatment of Multiple Gingival Recession Defects When Utilizing MCAT or MCAF Based on Evidence and Clinical Experience.

The multiple coronally advanced flap (MCAF) and the modified coronally advanced tunnel technique (MCAT) are the most commonly used methods for treating multiple gingival recessions. However, treating multiple defects is very complex due to various biologic and anatomical factors, and there is no clear guideline on the major or minor determinants that influence surgical decisions. The aim of the present article is to discuss a decision tree to suggest to clinicians the most relevant anatomical factors to consider when evaluating the choice between an MCAT and an MCAF. In the proposed decision-m

Indication and timing of soft tissue augmentation at maxillary and mandibular incisors in orthodontic patients. A systematic review.

OBJECTIVE: To assess the indication and timing of soft tissue augmentation for prevention or treatment of gingival recession when a change in the inclination of the incisors is planned during orthodontic treatment. MATERIALS AND METHODS: Electronic database searches of literature were performed. The following electronic databases with no restrictions were searched: MEDLINE, EMBASE, Cochrane, and CENTRAL. Two authors performed data extraction independently using data collection forms. RESULTS: No randomized controlled trial was identified. Two studies of low-to-moderate level of evidence were

Covering of gingival recessions with a modified microsurgical tunnel technique: case report.

Recent advances in periodontal plastic surgery have encouraged reassessment of the envelope technique and its modified version, the tunnel technique, for root coverage. This article describes alterations in the technique-sensitive tunnel approach, which include an undermining split flap preparation of the buccal tissues with newly developed instruments to minimize trauma and ensure a better blood supply for the connective tissue graft. The use of a microsurgical concept, including microsurgical blades and suture material, improves wound healing and establishes a better esthetic result. Because

Modified semilunar coronally advanced flap.

BACKGROUND: Correction of mucogingival recession deformities with a variety of periodontal plastic surgery procedures has been described, each demonstrating a variable degree of success. A modified semilunar coronally advanced flap is described for the treatment of recession defects on multiple adjacent teeth. METHODS: Semilunar incisions were made apical to the recession defects, starting within mucosa and extended mesio-distally, arching more coronally to terminate apical to the papillae mesial and distal to the teeth exhibiting the defects. The papilla between the teeth with recession was

The subepithelial connective tissue graft palatal donor site: anatomic considerations for surgeons.

Surgeons must become completely familiar with the anatomy of the palatal donor site to feel confident in providing the subepithelial connective tissue graft procedure. Variations in the size and shape of the hard palate affect the dimensions of donor tissue harvested, as well as the location of the greater palatine neurovascular bundle. This article classifies palatal vaults according to height as high, average, and shallow. Illustrations and cadaver dissection are utilized to demonstrate that surgeons can gain substantial donor tissue specimens without encountering the neurovascular bundle. A

Surgical coverage of exposed root surfaces: a review of available techniques and applications.

Exposed root surfaces often create esthetic problems with less-than- satisfactory restorative solutions. Gingival recession may result in thermal and tactile sensitivity that is unresponsive to treatment with desensitizing agents. Surgical procedures for the coverage of exposed root surfaces have provided successful and highly predictable results even in deep, wide areas of recession. This article reviews the available procedures and discusses the indications for periodontal plastic surgery. Clinical cases are presented that demonstrate solutions to commonly encountered clinical problems that
