Glasgow Coma Scale ≤ 12 at Admission is a Predictor of Poor Functional Outcome (mRS 2-6) at One Year in Patients with Cerebral Venous Thrombosis.

OBJECTIVE: To determine factors at hospitalization of cerebral venous thrombosis (CVT) which determine outcome at one year. METHODS: This was an ambispective study with outcome at one year follow up. Patients angiographically proven as CVT were included in study and functional modified Rankin Scale (mRS) determined at one year. They were dichotomized into "good" outcome (mRS 0-1) and "poor" outcome (mRS 2-6). Variables at admission were compared on univariate and then by cox proportional hazard regression for significance. Complications during follow up period were also compared. RESULTS: On

ED disposition of the Glasgow Coma Scale 13 to 15 traumatic brain injury patient: analysis of the Transforming Research and Clinical Knowledge in TBI study.

OBJECTIVE: Mild traumatic brain injury (mTBI) patients are frequently admitted to high levels of care despite limited evidence suggesting benefit. Such decisions may contribute to the significant cost of caring for mTBI patients. Understanding the factors that drive disposition decision making and how disposition is associated with outcomes is necessary for developing an evidence-base supporting disposition decisions. We evaluated factors associated with emergency department triage of mTBI patients to 1 of 3 levels of care: home, inpatient floor, or intensive care unit (ICU). METHODS: This mu

Association between fresh frozen plasma transfusion and mortality stratified by Glasgow Coma Scale scores in isolated traumatic brain injury: a nationwide cohort study in Japan.

PURPOSE: Fresh frozen plasma (FFP) is widely used to manage trauma-induced coagulopathy. However, its effectiveness in isolated traumatic brain injury (TBI) management remains controversial, particularly when stratified by severity based on the Glasgow Coma Scale (GCS) scores. This study aimed to examine the association between FFP administration within 24 h and mortality in patients with isolated TBI, overall and stratified by GCS on arrival. METHODS: We conducted a multicenter retrospective cohort study using the Japan Trauma Data Bank (2019-2023). Adults with isolated TBI (intracranial AIS

Comparative analysis of Glasgow Coma Scale, quick Sepsis-related Organ Failure Assessment, base excess, and lactate for mortality prediction in critically ill emergency department patients.

OBJECTIVES: It is crucial to promptly identify high-mortality patients in emergency departments and initiate their treatment as soon as possible. Although many parameters have been studied to select patients with high mortality, no comprehensive evaluation exists in previous literature on these parameters in critically ill patients, regardless of patient groups. The aim of this study is to evaluate the Glasgow Coma Scale (GCS), quick Sepsis-related Organ Failure Assessment (qSOFA), blood gas base excess (BE), and blood gas lactate in predicting mortality in critically ill patients admitted to

A Prognostic Model for Predicting One-Month Outcomes among Emergency Department Patients with Mild Traumatic Brain Injury and a Presenting Glasgow Coma Scale of Fifteen.

The lack of well-performing prognostic models for early prognostication of outcomes remains a major barrier to improving the clinical care of patients with mild traumatic brain injury (mTBI). We aimed to derive a prognostic model for predicting incomplete recovery at 1-month in emergency department (ED) patients with mTBI and a presenting Glasgow Coma Scale (GCS) score of 15 who were enrolled in the HeadSMART (Head Injury Serum Markers for Assessing Response to Trauma) study. The derivation cohort included 355 participants with complete baseline (day-of-injury) and follow-up data. The primary

Neurobiomarkers for Traumatic Brain Injury: Comparison of Serum Values Within 24 Hours of Injury With Glasgow Coma Scale (GCS) Scores in a Prospective Cohort Trial.

Objective Traumatic brain injury (TBI) is a significant cause of morbidity, disability, and mortality across all age groups, presenting both health and socioeconomic challenges globally. Neuroimaging techniques are crucial for assessing TBI, but their availability is often limited. Investing in point-of-care blood biomarkers, such as neurofilament light (NFL) protein and S100 calcium-binding protein B (S100B), may offer more accessible and reliable information on neuronal injury, assisting in clinical evaluation without compromising sensitivity. The objective of this study was to correlate the

Aggressive operative neurosurgical management in patients with extra-axial mass lesion and Glasgow Coma Scale of 3 is associated with survival benefit: A propensity matched analysis.

INTRODUCTION: Prognosis in patients with traumatic brain injury (TBI) and Glasgow Coma Scale (GCS) score of 3 is poor, raising concern regarding the utility of aggressive operative neurosurgical management. Our purpose was to describe outcomes in a propensity matched population with TBI and GCS3 treated with operative neurosurgical procedures of craniotomy or craniectomy (CRANI). METHODS: We conducted a five-year, multicenter retrospective cohort study of patients with an ED GCS 3 and a positive head CT identified by ICD-9CM diagnosis codes. Two populations were examined: (1) patients with ex

Mild head injury: differences in prognosis among patients with a Glasgow Coma Scale score of 13 to 15 and analysis of factors associated with abnormal CT findings.

We performed a retrospective study of 2484 consecutive patients with mild head injury (Glasgow Coma Scale score 13-15) who were seen during a period of 18 months. Of these, 2351 (94.6%) patients scored 15 points, 88 (3.5%) scored 14 points and 45 (1.3%) 13 points. A multivariate analysis showed that advanced age, a lower GCS (13-14) and the presence of skull fracture, and focal signs, significantly increased the incidence of abnormal computed tomography (CT) findings. By contrast, the gender, the mechanism of injury, the occurrence of initial loss of consciousness, posttraumatic amnesia and co

Early prediction of outcome following head injury in children: an assessment of the value of Glasgow Coma Scale score trend and abnormal plantar and pupillary light reflexes.

A retrospective study of 95 children less than 15 years of age with significant head injury was made to assess the value of Glasgow Coma Scale (GCS) score trend and plantar and pupillary light reflexes during the first 24 hours after injury, in predicting eventual outcome. GCS score trend or reflexes used alone were significantly correlated to outcome. There was also a statistically significant correlation when these parameters in combination were related to outcome. However, the clinical value of the combined use of GCS score trend and reflexes was only slightly greater than the use of GCS sc

Clinical Profile and Association Between Admission Glasgow Coma Scale and In-Hospital Mortality Among Children With Head Injury at a Tertiary Care Centre in Central India.

Introduction Traumatic brain injury (TBI) is a leading cause of trauma-related morbidity and mortality in children worldwide, and injury patterns in low- and middle-income countries differ from those reported in high-income settings. Data from tertiary centres in central India remain limited. This study describes the clinical profile, management pattern, and in-hospital outcome of children admitted with head injury at a tertiary care centre and evaluates the association between admission Glasgow Coma Scale (GCS) and in-hospital mortality. Methods This retrospective observational study included

Analysis of long-term (median 10.5 years) outcomes in children presenting with traumatic brain injury and an initial Glasgow Coma Scale score of 3 or 4.

OBJECT: Patients with traumatic brain injury (TBI) with low presenting Glasgow Coma Scale (GCS) scores have very high morbidity and mortality rates. Neurosurgeons may be faced with difficult decisions in managing the most severely injured (GCS scores of 3 or 4) patients. The situation may be considered hopeless, with little chance of a functional recovery. Long-term data are limited regarding the clinical outcome of children with severe head injury. The authors evaluate predictor variables and the clinical outcomes at discharge, 1 year, and long term (median 10.5 years) in a cohort of children

Age-adjusted shock index vs. reverse shock index multiplied by Glasgow Coma Scale to evaluate mortality in pediatric trauma: a systematic literature review and meta-analysis.

OBJECTIVE: Early identification of high-risk pediatric trauma patients is essential for timely intervention. The Shock Index, Pediatric Age-adjusted (SIPA) and the reverse Shock Index multiplied by Glasgow Coma Scale (rSIG, including rSIM variants) are commonly studied physiologic scores that have not been systematically compared. This study evaluated and compared the performance of SIPA and rSIG/rSIM for predicting mortality in pediatric trauma, and secondarily, blood product transfusion and other resource-use outcomes. METHODS: This systematic review and meta-analysis followed PRISMA 2020 a

Prior Psychiatric Disorder and Post-Traumatic Stress, Depressive and Anxiety Disorder after Traumatic Brain Injury with Glasgow Coma Scale Score 13-15: A TRACK-TBI Study.

The most consistent risk factor for developing psychiatric problems post-traumatic brain injury (TBI) is a preexisting psychiatric disorder, but many studies have reported that psychiatric disorders can occur de novo following "mild" TBI. The objective of this secondary analysis of a prospective cohort study of patients (n = 1,947) with acute TBI and presenting Glasgow Coma Scale (GCS) score between 13 and 15 was to describe the association of pre-injury psychiatric history with prevalence and risk factors for probable posttraumatic stress disorder (PTSD), major depressive disorder (MDD), and

A prospective study of Glasgow Coma Scale (GCS), age, CSF-neutrophil count, and CSF-protein and glucose levels as prognostic indicators in 100 adult patients with meningitis.

BACKGROUND: The Glasgow coma scale (GCS) is an objective measurement of a patient's level of consciousness and has prognostic implications in traumatic head injuries. Morbidity and mortality of patients with meningitis have been related amongst others to level of consciousness, hypoglycorrhachia, extremes of age, and high CSF protein values. In this prospective study of 100 patients the correlation between the GCS, age, CSF-neutrophil count and CSF-glucose and protein levels and the eventual outcome of the patients was assessed. METHODS: In 100 consecutive patients with meningitis (bacterial,
