Comparison of the predictive value of the Helsinki, Rotterdam, and Stockholm CT scores in predicting 6-month outcomes in patients with blunt traumatic brain injuries.

PURPOSE: Despite advances in modern medicine, traumatic brain injuries (TBIs) are still a major medical problem. Early diagnosis of TBI is crucial for clinical decision-making and prognosis. This study aims to compare the predictive value of Helsinki, Rotterdam, and Stockholm CT scores in predicting the 6-month outcomes in blunt TBI patients. METHODS: This cohort study was conducted on blunt TBI patients of 15 years or older. All of them were admitted to the surgical emergency department of Shahid Beheshti Hospital in Kashan, Iran from 2020 to 2021 and had abnormal trauma-related findings on

Relation of visual field defects to neuropsychological outcome after closed head injury.

Neuropsychological outcome within two years after injury was determined in 159 head injured patients who were classified into three groups according to the presence of either unilateral, bilateral, or no visual field defects (VFDs). The VFDs occurred irrespective of injury severity as determined by the Glasgow coma scale, or social outcome as determined by the Glasgow outcome scale. Differences among the three visual field groups were obtained for several neuropsychological functions: intelligence, memory, learning, acquired verbal skills, visuospatial skills, and visuomotor speed. Patients wi

Association between the fresh frozen plasma-to-red blood cell transfusion ratio and neurological outcomes in patients with severe traumatic brain injury.

OBJECTIVE: To date, no studies have focused on the fresh frozen plasma (FFP)-to-red blood cell (RBC) transfusion ratio in patients with severe traumatic brain injury (TBI). Herein, the authors investigated the relationship between the FFP-to-RBC ratio and neurological outcomes, including mortality, in patients with severe TBI (Glasgow Coma Scale [GCS] score < 9). METHODS: This multicenter, retrospective, observational study used data from the Japan Trauma Data Bank from 2019 to 2023. The study included patients aged ≥ 18 years with severe TBI (defined as a GCS score < 9 and an Abbreviated Inj

Outcome of moderate and severe traumatic brain injury amongst the elderly in Singapore.

INTRODUCTION: In line with other established protocols, our unit has instituted a standardised protocol for the management of moderate and severe traumatic brain injury since 1996 in our neurointensive care unit. MATERIALS AND METHODS: We reviewed the outcomes, at 6 months' post-injury, in an elderly group aged > or = 64 years (73.86 +/- 8.0 years) and compared them to a younger group aged 20 to 40 years (29.2 +/- 5.7 years) in a cohort of 324 patients. Outcome was dichotomised as favourable (mild and moderate disability but independent; Glasgow Outcome Score [GOS] 4 and 5), unfavourable (sev

Time-to-treatment in traumatic brain injury: unraveling the impact of early surgical intervention on patient outcomes.

BACKGROUND: Traumatic brain injury (TBI) remains a major public health concern due to its high morbidity and mortality. The 'golden hour' principle suggests that outcomes improve with rapid access to definitive care. However, the role of prehospital transport time in TBI prognosis remains unclear, particularly in resource-limited settings. This study evaluates the relationship between hospital arrival time and functional prognosis in TBI patients. METHODS: A cross-sectional observational study was conducted in two Ecuadorian trauma centers from 2017 to 2020. Patients were categorized into ear

Sex Differences in Neurological Outcome at 6 and 12 Months Following Severe Traumatic Brain Injury. An Observational Analysis of the OXY-TC Trial.

The effect of sex in outcomes after severe traumatic brain injury (TBI) remains uncertain. We explored whether outcomes differed between women and men after standardized care management during the first 5 days in the intensive care unit (ICU). This study was an observational analysis of the OXY-TC multicenter randomized clinical trial between June 15, 2016 and April 17, 2021. Recruited patients had a pre-hospital Glasgow Coma Scale (GCS) score of 3-8, mechanical ventilation, and intracranial pressure (ICP) with or without brain tissue oxygen pressure (PbtO2) monitoring. Objectives were to main

Clinical outcome of mild head injury with isolated oculomotor nerve palsy.

Isolated oculomotor nerve palsy after mild head injury is rare. Only a few case reports have described the clinical presentation of these patients, and clinical and functional outcome have not been discussed in depth. The outcome of 10 patients with mild head injury in whom imaging studies ruled out other possible causes of oculomotor palsy was assessed during follow-up using the Glasgow Outcome Scale-Extended (GOSE). We suggest that limited eye movement is a major factor that negatively affects functional status after mild head injury.

"The Glasgow effect?"- the result of the geographical patterning of deprived areas?

The aim of this research was to examine whether the excess mortality found in Glasgow, compared to other cities in the UK ("Glasgow effect"), could be attributed to patterns of the distribution of deprived neighbourhoods within the cities. Data on mortality and deprivation at a neighbourhood scale were used to examine the impact of the patterning of neighbourhood deprivation on mortality in Glasgow, Liverpool and Manchester. Analysis using a combination of GIS and statistical approaches, including a Moran׳s I test and Conditional Auto Regressive models to capture residual spatial autocorrelati

Emergency neurosurgery for traumatic brain injury by general surgeons at local hospitals in Sweden: a viable option when time is brain.

BACKGROUND: Timing of surgical evacuation of mass lesions in traumatic brain injury (TBI) is crucial. However, due to geographical variations, transportation time to the nearest neurosurgical department may be long. To save time, general surgeons at a local hospital may perform the operation, despite more limited experience in neurosurgical techniques. This study aimed to determine whether patient outcomes differed between those who had undergone emergency neurosurgery at local hospitals by general surgeons vs. at university hospitals by neurosurgeons. METHODS: A nationwide observational stud

Endoscopic third ventriculostomy for occlusive hydrocephalus caused by cerebellar infarction.

OBJECTIVE: The surgical management of occlusive hydrocephalus caused by massive cerebellar infarction remains controversial. The procedures that are more commonly used to avoid progressive neurological deterioration are based on transient external ventricular drainage or the placement of permanent shunt systems. To our knowledge, this is the first report regarding using endoscopic third ventriculostomy (ETV) in patients with an occlusive hydrocephalus caused by cerebellar ischemic stroke. We report our experience of 10 reviewed cases. METHODS: Between 1997 and 2004, 10 patients with a resulti

Admission Neurological Status, Radiological Mass Effect, and In-Hospital Mortality Among Surgically Treated Patients with Traumatic Acute Subdural Hematoma: A Single-Center Experience from Türkiye.

BACKGROUND: Traumatic acute subdural hematoma (ASDH) carries high morbidity and mortality despite surgery. We evaluated associations of admission neurological status and radiological mass effect with in-hospital mortality. An exploratory analysis examined a composite unfavorable Glasgow Outcome Scale (GOS) outcome, with in-hospital mortality as its largest component. METHODS: This retrospective study included 82 adults treated surgically for traumatic ASDH between January 2020 and December 2025. The Zumkeller index (midline shift minus hematoma thickness) was categorized as <0, 0-3, or >3 mm.
