BACKGROUND: Neovascular glaucoma (NVG) is a serious complication for patients with proliferative diabetic retinopathy (PDR). Bevacizumab is a full-length humanized monoclonal antibody that binds all isoforms of vascular endothelial growth factor (VEGF). Recently, encouraging results regarding the off-label use of intravitreal bevacizumab (IVB) for the treatment of NVG have been reported. BACKGROUND: Neovascular glaucoma (NVG) is a rare but severe complication of hypoperfusion retinopathy. Our objective was to analyse the clinical features of NVG secondary to carotid artery atherosclerosis, moyamoya disease and Takayasu arteritis.
METHODS: Thirty-six cases (36 eyes) with hypoperfusion retinopathy associated with NVG were retrospectively analysed. Each patient had their visual acuity assessed and fundus examined by fundus fluorescein angiography, colour Doppler flow imaging, magnetic resonance angiography, digital subtraction angiography and other techniques.

We compared the effect of seton (White pump shunt) surgery (16 eyes) with that of trabeculectomy and 5-fluorouracil (31 eyes) in treating 38 Asian patients with medically uncontrollable neovascular glaucoma. We found the probability of long-term success (intraocular pressure < or = 26 > or = 5 mm Hg) of seton surgery (53.0% at 3 years) to be similar to that obtained following trabeculectomy with adjunctive 5-fluorouracil (45.4% at 3 years). The patient charts of 81 consecutive vitrectomies performed for complications of proliferative diabetic retinopathy were reviewed. Sixty-five preoperative, intraoperative, and postoperative variables were analyzed. In eyes with completed panretinal photocoagulation, only postoperative retinal detachment had a statistically significant correlation with postvitrectomy development of neovascular glaucoma. A retrospective study of the rate of development of neovascular glaucoma after cataract extraction in 242 eyes of 186 diabetic patients identified neovascular glaucoma in 13 of 146 eyes (8.9%) after intracapsular extraction, in two of 17 eyes (11.8%) after extracapsular extraction with primary capsulotomy, and in zero of 53 eyes after extracapsular extraction without capsulotomy. PURPOSE: To report two cases of neovascular glaucoma associated with Radium-223 infusion.
OBSERVATIONS: Presented are two patients with metastatic prostate cancer who developed uncontrolled intraocular pressure secondary to neovascular glaucoma requiring surgical intervention. Both patients had received six cycles of Radium-223, a calcium mimetic that causes DNA double strand breaks and tumor cell death in bony metastases as part of their treatment regimen for metastatic prostate cancer. One patient had been a prior glaucoma suspect while the other had no significant ocular history.

Three cases of neovascular glaucoma associated with ocular hypoperfusion secondary to carotid artery disease are reported. In two cases, anastomosis of the internal and external carotid artery systems via a superficial temporal artery to the middle cerebral artery (Yasargil procedure) resulted in resolution of the rubeosis, relief of the ocular pain and improvement of the circulation of the globe. The third case, which did not have this procedure, resulted in a blind eye with severe rubeotic glaucoma.
