Anti-glomerular basement membrane (anti-GBM) disease is a rare entity. The classic presentation includes circulating autoantibodies and rapidly progressive glomerulonephritis. However, there are atypical variants, which constitute 5% to 10% of cases. These variants are characterized by the absence of circulating antibodies, a more indolent clinical course, and the potential to recur in a kidney transplant. We present the case of a woman with chronic kidney disease initially diagnosed as unclassified glomerulonephritis. After the first transplant, a biopsy revealed linear IgG deposits with lamb

A 68-year-old male patient presented with a 2-week history of malaise and anuria. Renal replacement therapy with haemodialysis was begun for acute kidney injury. His anti-glomerular basement membrane (anti-GBM) antibody titre was 3060 U/ml. Based on this finding, anti-GBM disease was diagnosed. Plasmapheresis and high-dose glucocorticoid therapy were begun, but his haemolytic anaemia and thrombocytopenia progressed. A disintegrin and metalloprotease with thrombospondin type 1 motif, 13 (ADAMTS-13) activity decreased to 33%, but no inhibitor was detected. Secondary thrombotic microangiopathy wa

Objective: To describe a case of anti-glomerular basement membrane (GBM) nephritis that occurred shortly after initiation of nebulized tobramycin (TOB) therapy using intravenous solution, suggesting an association with the inhalation therapy and the disease onset. Background: With the emergence of antimicrobial resistance, clinical importance of aminoglycosides that usually remain susceptibility against gram-negative organisms is increasingly acknowledged. Despite the growing number of evidence supporting the effectiveness of aminoglycoside inhalation therapy for respiratory tract infections,

Prior reports have associated the unusual presentations of anti-glomerular basement membrane (anti-GBM) disease and pulmonary aspergillosis in an immunocompetent host. We present the case of a previously healthy 24-year-old man who presented with symptoms of hemoptysis, cough, and dyspnea. His environmental risk factors included tobacco use (smoking), hydrocarbon exposure, and an unidentified mold present in his home. Laboratory evaluation revealed positive serum anti-GBM autoantibodies and positive detection of galactomannan in the bronchoalveolar lavage fluid. The diagnosis of anti-GBM disea

In 1961, based on results obtained with the particulate tracer ferritin, Farquhar, Wissig and Palade [15] proposed a functional model for the glomerulus and defined a role for each of its components in the filtration process: a) the basement membrane as the main filter; b) the endothelium as a valve, which by the number and size of its fenestrae, controls access to the filter; c) the epithelium as a monitor which partially recovers proteins that leak through the filter; and d) the mesangium which serves to recondition and unclog the filter by incorporating and disposing of filtration residues

Thin basement membrane disease (TBMD) typically presents with persistent microscopic hematuria, and is usually defined as a glomerular basement membrane (GBM) thickness < 250 nm. Previous studies showed that neither the degree of thinning nor the extent of the abnormality correlate with the patient's clinical presentation or prognosis. To further define this, we enrolled a study group of 41 patients with isolated microscopic hematuria and a normal renal biopsy, except those with a GBM thickness of 250-320 nm, and compared them with 33 patients with traditional TBMD. We found no difference in b

The non-collagenous domain 1 of the α3 chain of type IV collagen (α3(IV)NC1) is the primary autoantigen in anti-glomerular basement membrane (anti-GBM) disease. We previously developed a modified antigen-specific peptide, m-P14, derived from the nephritogenic epitope α3127-148, which ameliorated experimental anti-GBM nephritis. However, its short half-life limits clinical translation. This study evaluated a butyrate-conjugated derivative (m-P14-BA) to improve pharmacokinetic properties while preserving therapeutic efficacy. M-P14-BA and m-P14 were administered to α3127-148 immunized Wistar Kyo

A woman in her 20s was referred to renal clinic for investigation regarding positive anti-glomerular basement membrane (anti-GBM) antibody results. Autoimmune screening was performed to investigate an episode of fever, cough and haemoptysis which presented alongside the formation of a maculopapular rash and mild proteinuria (1+) on urine reagent dipstick.Despite repeated positive anti-GBM results obtained through BioPlex 2200 multiple antibody (multiplex) immunoassay, examination four months later was unremarkable, all signs and symptoms being resolved. Urinalysis, imaging and renal function t

BACKGROUND: Thin basement membrane nephropathy (TBMN) is a disorder characterized by ultrastructural abnormalities of the glomerular basement membrane (GBM), representing a spectrum of genetic and clinical phenotypes ranging from benign hematuria to proteinuria and chronic kidney disease. Recent studies have shown that a significant percentage of patients who initially present with hematuria later develop proteinuria and worsening renal function.
MATERIALS AND METHODS: We retrospectively analyzed records of patients diagnosed with TBMN, including their clinical, laboratory, and histological f

Anti-glomerular basement membrane (anti-GBM) glomerulonephritis is a rare disease caused by autoantibodies against the glomerular basement membrane. Atypical anti-GBM nephritis is clinically less aggressive and characterized by the absence of circulating autoantibodies to the basement membrane. A previously healthy 53-year-old white woman presented with a rising creatinine over a short observation period. Renal biopsy, urinary sediment, and laboratory testing confirmed the diagnosis of atypical anti-GBM disease. She received plasmapheresis, steroids, and cyclophosphamide. She developed hemorrh

Classic anti-glomerular basement membrane (GBM) disease presents with rapidly progressive glomerulonephritis (GN) with or without pulmonary hemorrhage. On biopsy typical disease displays bright polytypic linear GBM staining for IgG by immunofluorescence and diffuse crescentic/necrotizing GN on light microscopy. Here, we studied 20 patients with atypical anti-GBM nephritis typified by bright linear GBM staining for immunoglobulins but without a diffuse crescentic phenotype. Patients had hematuria, proteinuria, and mild renal insufficiency, without pulmonary hemorrhage. Light microscopy showed e

Fifteen renal biopsy specimens from adult patients with minimal change disease (MCD) for whom both light and electron microscopy as well as immunofluorescence microscopy and full clinical data were available were examined quantitatively and compared with six normal controls. The electron micrographs of the glomeruli were scanned in Primax flatbed A4 scanner and then morphometric investigations were performed by means of a computer image analysis system to compare glomerular basement membrane (GBM) thickness and to study whether this parameter could correlate with the degree of proteinuria. In this report, the authors describe a case of a 2-year, 11-month-old girl with glomerulonephritis and no family history of renal diseases and deafness. Immunofluorescent studies in the renal biopsy specimens with the use of anti-sera against human glomerular basement membrane (GBM) and P3 antigen (prepared from bovine GBM and inducible of Steblay's type nephritis in rats) demonstrated focal and segmental distribution of the GBM antigen(s). Electron microscopic examination revealed splitting and thinning of the GBM. Indirect immunofluorescence showed that there was no binding of Goodpasture's

To clarify the mechanism of hexadimethrine-induced proteinuria, we studied the changes in stainable glomerular basement membrane anions and renal hemodynamics during hexadimethrine (HDM) infusion. To determine whether glomerular anions were neutralized in vivo, animals infused with HDM received cationized ferritin intravenously, or their kidneys were perfused in situ with lysozyme. During its infusion, HDM bound heavily within the glomerular basement membrane, and binding of the cationic probes was virtually abolished. During recovery after HDM infusion, HDM deposits diminished and the binding
