Specific properties of shRNA-mediated CCR5 downregulation that enhance the inhibition of HIV-1 infection in combination with shRNA targeting HIV-1 rev.

Treatment with RNAi against HIV-1 transcripts efficiently inhibits viral replication but induces selection of escape mutants; therefore, the CCR5 coreceptor was suggested as an additional target. Blocking viral and host transcripts improved the antiviral effect. We have used short hairpin RNA (shRNA) targeting the human CCR5 (shCCR5) or the HIV-1 rev (shRev) transcripts to demonstrate distinctive properties of anti-CCR5 shRNA: shCCR5 induced more sustained protection than shRev; partial reduction in CCR5 expression substantially decreased HIV-1 infection, and shCCR5 performed better than shRev

Enhancing linkage to HIV care in the "Universal Test and Treat" era: Barriers and enablers to HIV care among adults in a high HIV burdened district in KwaZulu-Natal, South Africa.

Ending AIDS by 2030 would depend on how successful health systems are in linking people living with HIV (PLHIV) into care. The World Health Organization recommended the 'Universal Test and Treat' (UTT) strategy - initiating all individuals testing positive on antiretroviral therapy (ART) irrespective of their CD4 count and clinical staging. This study aimed to explore the enablers and barriers to linkage to HIV care among adults with a new HIV diagnosis in a high-HIV prevalent rural district in South Africa. A qualitative study was undertaken to explore patients' perceptions of enablers and ba

Enhanced B cell priming induces broadly neutralizing HIV-1 apex antibodies.

Efficient priming of B cell precursors is a rate-limiting step in the induction of V2 apex broadly neutralizing antibodies (bnAbs)1,2. Here we describe a germline-targeted HIV-1 Env (CAP256.OPT4) that increases the efficiency of V2 apex bnAb precursor priming by 30-400-fold compared with wild-type HIV-1 Envs and induces, in over 90% of macaques, neutralization breadth that includes Asn130-containing viruses. Using three different delivery platforms-persistently replicating simian human immunodeficiency viruses (SHIVs), protein nanoparticles and mRNA-we show bnAb priming as early as 4 weeks aft

Retinoids enhance NK effector function against HIV-infected CD4 T cells.

Novel approaches to sensitize latently infected cells to apoptosis may provide additional methods to eliminate latent reservoirs. Prior research identified several retinoids as potential drugs that increase the sensitivity of HIV-infected cells to cell death. Retinoids are derivatives of vitamin A that target retinoid receptors causing antiproliferative and proapoptotic activity. Several are FDA-approved or in clinical trials. The aim of this study was to evaluate the ability of vitamin A, three of its natural metabolites, and nine synthetic derivatives to sensitize HIV-infected CD4 T cells to

A broad antibody with enhanced HIV-1 neutralization via bispecific antibody-mediated prepositioning.

Antibodies targeting the highly conserved prehairpin intermediate (PHI) of class I viral membrane-fusion proteins are generally weakly neutralizing and are not considered viable therapeutic agents. We previously demonstrated that antibodies targeting the gp41 N-heptad repeat (NHR), which is transiently exposed in the HIV-1 PHI, exhibit enhanced broad neutralization in cells expressing the Fc receptor, FcγRI. To enhance neutralization in cells lacking FcγRI, we here develop a bispecific antibody (bsAb) by fusing an NHR-targeting antibody to an antibody against CD4, the HIV-1 receptor on T cells

African Immigrants and the End of the HIV Epidemic in the United States: Actionable Recommendations to Enhance Equity.

African immigrants in the United States are disproportionately affected by HIV yet remain overlooked in public health strategies such as the Ending the HIV Epidemic (EHE) initiative. Many reside in EHE-prioritized counties, but their categorization under the broad "Black/African American" label obscures their distinct pathways to HIV acquisition and barriers to care. We explore how African immigrants can be more effectively prioritized in the EHE framework. We examine key challenges, including stigma, cultural and linguistic barriers, and structural inequities, that limit access to prevention

Enhancing holistic patient care of black individuals diagnosed with HIV and comorbid chronic conditions.

BACKGROUND: Pharmacists can play a crucial role in delivering the holistic care model. Research suggests that HIV-positive African American patients particularly benefit from this approach compared to other communities. Typically, pharmacists in community pharmacy settings are limited in their access to a patient's medical record, preventing holistic care delivery. OBJECTIVES: This study addressed the impact of access to medical records on a holistic patient care approach among pharmacists engaged in medication therapy management. METHODS: This was a sub-analysis of a randomized controlled t

Finetuning Type I Interferon Signaling to Enhance T Cell Immunity in HIV Infection.

Type I interferons (IFN-Is) play a dual role in the immune response to HIV-1, providing early antiviral defense while driving immune dysfunction in the chronic phase. During acute infection, robust IFN signaling is critical in controlling viral replication, activating innate immunity, and limiting reservoir establishment. However, sustained IFN-I activation during chronic infection fuels systemic inflammation, immune exhaustion, and fibrosis, particularly in lymphoid tissues such as gut-associated lymphoid tissue (GALT). Prolonged IFN-I exposure upregulates inhibitory receptors on T cells, imp

Intravaginal delivery of mRNA-encoded antibodies with enhanced breadth and potency for SHIV/HIV protection.

Broadly neutralizing antibodies (bnAbs) prevent HIV infection but face administration and cost challenges. We present single-chain mRNA-encoded bnAbs that improve heavy-light chain assembly and enable three enhancement approaches: co-expression, isotype selection, and engineered nanobodies. We observe enhanced in vitro neutralization through co-expression of PGT121 and VRC07 (targeting V3-glycan and CD4-binding site, respectively) and by replacing IgG constant heavy chain (IgG-CH) with IgA-CH or incorporating an IgM tailpiece into IgG-CH. While IgG versions fail to neutralize several SHIV/HIV

The role of mycobacteremia screening in enhancing non-tuberculous mycobacteria detection in hospitalized persons with HIV.

Among hospitalized severely immunocompromised persons with HIV (PWH), non-tuberculous mycobacteria (NTM) may be under-diagnosed due to non-specific symptoms undifferentiable with other opportunistic infections. To evaluate the prevalence of NTM mycobacteremia and how screening for mycobacteremia assistant with identification of NTM infections, this study retrospectively analyzed 1,136 hospitalized PWH with CD4 counts <200 cells/μL, who underwent mycobacteremia screening at a tertiary hospital in Nanjing, China, between July 2018 and December 2023. The prevalence of non-tuberculous mycobacterem

TNF-α inhibition in the setting of undiagnosed HIV infection: a call for enhanced screening guidelines.

BACKGROUND: Despite the risks of immunosuppression, recommendations regarding screening for HIV infection prior to initiation of biologic therapies targeting common rheumatologic disorders, including inflammatory bowel disease (IBD) and inflammatory arthritides, are limited. Few cases of patients started on biologics while living with undiagnosed HIV infection have been reported. METHODS: We report three cases of patients initiated on biologics in the absence of recent or concurrent HIV screening who developed refractory disease or unanticipated complications and were later found to have undi

Chlamydia trachomatis Infection of Endocervical Epithelial Cells Enhances Early HIV Transmission Events.

Chlamydia trachomatis causes a predominantly asymptomatic, but generally inflammatory, genital infection that is associated with an increased risk for HIV acquisition. Endocervical epithelial cells provide the major niche for this obligate intracellular bacterium in women, and the endocervix is also a tissue in which HIV transmission can occur. The mechanism by which CT infection enhances HIV susceptibility at this site, however, is not well understood. Utilizing the A2EN immortalized endocervical epithelial cell line grown on cell culture inserts, we evaluated the direct role that CT-infected

HIV-1 Tat Protein Enhances Expression and Function of Breast Cancer Resistance Protein.

ATP binding cassette (ABC) transporters can transfer a variety of antiviral agents from the cytoplasm to body fluid, which results in a reduced intracellular concentration of the drugs. Proteins of HIV-1, e.g., Tat and gp120, altered some types of ABC transporter expression in brain microvascular endothelial cells and astrocytes. However, the effect of Tat on ABC transporters in T lymphocytes is unclear. In this study the status of breast cancer resistance protein (BCRP) in Tat expressing cell lines was examined with real-time PCR and flow cytometry. It was found that HIV-1 Tat protein upregul

Rapid and Sensitive Determination of HIV-1 Virus Based on Surface Enhanced Raman Spectroscopy.

In this study, rapid and sensitive detection of human immunodeficiency virus (HIV)-1 was performed based on immunoreactions with an Au nanodot fabricated indium tin oxide (ITO) substrate using surface-enhanced Raman spectroscopy (SERS). Highly ordered Au nanodots (ca. 20 nm) were electrochemically fabricated over a large surface area (20 mm x 10 mm) of an ITO substrate using a simple deposition method with Triton X-100. On the Au nanodot surface, monoclonal antibody fragments against gp120 were selectively bound by gold-sulfur interactions. Various concentrations (35 fg/mL to 350 pg/mL) of HIV

Cauda Equina Enhancing Lesion in an HIV-Infected Patient. Case Report and Literature Review.

We report the case of an HIV-infected young men with neuro-toxoplasmosis localized in the spinal cord. The patient received chemotherapy and immunotherapy for Burkitt lymphoma one year before. At the time of the diagnosis of toxoplasmosis, he was on prophylaxis with trimethoprim and sulfamethoxazole and in complete remission of Burkitt lymphoma. The CD4+ T cell count was 270/μl and the HIV viremia was undetectable. These findings suggest that in this patient, the immunodeficiency promoting the neurologic toxoplasmosis arose more from previous immuno-chemotherapy than from the HIV-infection its

Enhanced diagnosis of HIV-1-associated tuberculosis by relating T-SPOT.TB and CD4 counts.

The sensitivity of the tuberculin skin test is impaired in HIV-1-infected persons. Enzyme-linked immunospot-based detection of immune sensitisation may be less affected. Furthermore, the quantitative response can be related to the CD4 count, potentially improving specificity for active disease. The T-SPOT.TB assay was performed on HIV-1-infected participants, 85 with active tuberculosis (TB) and 81 healthy patients (non-TB). The ratio of the sum of the 6-kDa early secretory antigenic target and culture filtrate protein 10 response to the CD4 count (spot-forming cell (SFC)/CD4) was calculated.

System-wide investments generate health and enhance HIV, TB and malaria control in Malawi.

Global health initiatives have expanded access to HIV, tuberculosis, and malaria services in low-income countries, yet these programmes often operate within fragile health systems where shortages of healthcare workers and essential medicines limit their effectiveness and sustainability. Evaluating investments in health system capacity remains challenging because their benefits are difficult to attribute. Here, using a dynamic microsimulation model of Malawi's health system, we show that health system strengthening investments alone could avert 11.4% of disability-adjusted life years between 20

HIV-1-encoded circular RNA enhances viral transcription through Tat binding.

Circular RNAs (circRNAs) are single-stranded RNAs that form a covalently closed loop without 5' and 3' ends. From their own genomes, RNA viruses can encode circRNAs that play important roles during infection, but whether this is the case for HIV remains unclear. Here we detect an HIV-1-encoded viral circRNA (termed circHIV) in plasma from 18 people living with HIV and in infected primary cells and T-cell lines using RT-qPCR and northern blot. We find that this viral circRNA is packaged within HIV-1 virions and binds to the HIV-1 Tat protein upon expression in cells, which enhances transcriptio

SAMHD1 enhances HIV-1-induced apoptosis in monocytic cells via the mitochondrial pathway.

Sterile alpha motif (SAM) and histidine-aspartate (HD) domain-containing protein 1 (SAMHD1) inhibits HIV-1 replication in non-dividing cells by reducing the intracellular dNTP pool. While SAMHD1 is known to promote spontaneous apoptosis, its role in HIV-1-induced apoptosis and the underlying mechanisms remain unclear. In this study, we identify a novel mechanism by which SAMHD1 enhances HIV-1-induced apoptosis in monocytic cells via the mitochondrial pathway. We demonstrate that SAMHD1 enhances apoptosis induced by HIV-1 infection in dividing monocytic THP-1 and U937 cell lines, but not in dif
