Immediate radiographic reduction loss but preserved clinical outcomes after hardware removal in lateral clavicle fractures treated with plate fixation and coracoclavicular augmentation.

BACKGROUND: Locking plate fixation combined with coracoclavicular (CC) augmentation using a suspensory fixation system is a widely accepted technique for unstable lateral clavicle fractures. Elective hardware removal is commonly performed due to mechanical irritation or discomfort, yet data on radiographic and clinical outcomes following implant removal remain limited. The purpose of this study was to evaluate immediate radiographic loss of reduction and long-term clinical outcomes after removal of the locking plate and CC augmentation in patients with lateral clavicle fractures. It was hypoth

The effect of using patient-specific guides for total knee replacement without hardware removal in complex post-traumatic arthritis: an in-vitro study.

BACKGROUND: Post-traumatic knee replacement (PTKR) is frequently complicated by the presence of retained metallic hardware around the joint, which limits the use of intramedullary alignment guides. Consequently, extramedullary jigs are often required, although they may increase radiation exposure and reduce alignment precision. Patient-specific guides (PSGs), generated from medical imaging and produced via 3D printing, offer a potential alternative for improving accuracy in complex surgical scenarios. This study aimed to assess the accuracy of PSGs in PTKR using in-vitro knee models with and w

Tubular versus anatomically contoured low-profile plate fixation of Weber B fibula fractures: A threshold-cost analysis based on rates of hardware removal.

BACKGROUND: While anatomically contoured low-profile plates (ACLPPs) are more expensive when compared to tubular plates for ankle fracture fixation, this may not capture the entirety of relevant costs. We aimed to conduct a threshold-cost analysis to determine the frequency of hardware removal that would make ACLPPs cost-effective despite increased implant cost. METHODS: In this threshold-cost analysis study, three orthopaedic implant companies provided list prices of ACLPPs, tubular 7-hole plates, 3.5 mm non-locking screws, and 3.5 mm locking screws. The constructs that were compared were: a

Low Rates of Hardware Removal and Tendon Rupture for the Acu-Loc 2 Volar Distal Radius Plate: A Minimum One-Year Follow-Up Study.

AIM: Volar locking plates designed for far distal radius fracture fixation can have a significant hardware removal rate and risk of tendon rupture. Plate design has a role in the rate of complications. This study assessed the hardware removal and tendon rupture rate of the Acu-Loc 2 volar distal radius (VDR) plate often used in the treatment of far distal radial fractures. METHOD: We searched our electronic healthcare records system for all patients who had undergone fixation with an Acu-Loc 2 VDR plate (Acumed, Hillsboro, OR, USA) at a tertiary center between January 2017 and December 2021.

Buttress Plating for Type 3-4-5 Jersey Finger Fractures: Without Bone Fragment Disruption and With a Challenging Rate of Hardware Removal-A Case Series.

PURPOSE: The use of plates in the treatment of Leddy-Packer type 3-5 flexor digitorum profundus tendon avulsion injuries is becoming increasingly prevalent; however, potential complications, including disruption of the avulsed bone fragment and damage to the flexor digitorum profundus tendon, may be encountered. The objective of this study was to ascertain whether the buttress plate technique, without screw placement in the avulsed fragment, can be employed as a treatment option for patients with type 3, 4, and 5 flexor digitorum profundus tendon avulsion injuries. METHODS: Type 3, 4, and 5 f

Can Hardware Removal be Avoided Using Bioresorbable Mg-Zn-Ca Screws After Medial Malleolar Fracture Fixation? Mid-Term Results of a First-In-Human Study.

UNLABELLED: Ankle is the most common site of hardware removal, mainly performed within 12 months of the primary surgery. The prominence of the metallic hardware is a frequent cause of pain after fracture fixation. Over the last decade, the development of bioresorbable materials based on magnesium (Mg) has increased. Bioresorbable metals aim to avoid a second surgery for hardware removal. METHODS: Twenty patients with isolated, bimalleolar, or trimalleolar ankle fractures were treated with bioresorbable screws made of Mg, 0.45wt% calcium (Ca) and 0.45wt% zinc (Zn) (ZX00). Patient-reported outc

Graft-Preserving Arthroscopic Debridement With Hardware Removal Is Effective for Septic Arthritis After Anterior Cruciate Ligament Reconstruction: A Clinical, Arthrometric, and Magnetic Resonance Imaging Evaluation.

BACKGROUND: Arthroscopic debridement with graft preservation has been advocated as the treatment of choice for septic arthritis after anterior cruciate ligament (ACL) reconstruction, but no previous studies have investigated if hardware removal, while retaining the graft in situ, improves the success rate. Moreover, it is unclear whether the premature removal of fixation devices may affect graft integration and knee stability. PURPOSE/HYPOTHESIS: The purpose was to assess the clinical and functional outcomes of patients with septic arthritis after ACL reconstruction who underwent arthroscopic

The boundary between microbial detection and implant-related infection: a structured narrative review of analytical credibility, biological persistence, host response, and clinical causality.

BACKGROUND: Unexpected microbial signals are frequently encountered in patients with orthopaedic implants who do not satisfy validated criteria for confirmed infection. The central interpretive problem is not limited to periprosthetic joint infection (PJI); it also arises in fracture-fixation and spinal instrumentation, and it involves conventional culture, sonication, histopathology, broad-range polymerase chain reaction (PCR), 16 S rRNA sequencing, and metagenomic next-generation sequencing (mNGS). METHODS: We performed a structured narrative review of literature identified in PubMed, Embas

Challenging Removal of Intramedullary Nails: Description of 2 Techniques With Proposal to Classify Nail Extraction Methods.

CASE: We present 2 techniques for removing intramedullary nails. In the first case, an intact, incarcerated short proximal femoral nail was removed without making an additional bone window and underwent hip arthroplasty in the same sitting. The other case had a small distal fragment of broken femoral nail removed through the nonunion site, and exchange nailing was performed. A classification system for various techniques is also proposed, which may help orthopedic surgeons best prepare in the event of failure to remove the nail using the standard technique. CONCLUSION: Removal of an intramedu

A microsurgical approach to implant mechanical rescue.

The purpose of this paper is to describe and illustrate the value of using a microsurgical approach when resolving an implant mechanical rescue case (internally retained implant component fragments). Four cases involving screw retrieval issues are reviewed to demonstrate how microscopic visualization clarifies a diagnostic algorithm to determine correct treatment protocols. Microscopic visualization then continues to contribute an extreme advantage for guidance and predictability during treatment. When accompanied with appropriate tooling choices, the combination offers improved consistency an

Flap Use for Wound Coverage After Spinal Hardware Infection: A Systematic Review of the Literature.

BACKGROUND: Reconstructing spinal defects complicated by surgical site infection (SSI) is challenging, with SSIs occurring in 1%-4% of spine surgeries, often involving hardware exposure. These infections increase hospital stays, costs, and risks of poor outcomes. Effective management strategies are crucial for addressing infected spinal wounds. METHODS: We conducted a systematic review following PRISMA-P guidelines across six databases. Included studies reported flap coverage for spinal reconstructions with or without hardware. Outcomes of interest included flap loss, partial flap necrosis, w

Low rate of secondary surgery and implant removal following superior, precontoured plating of midshaft clavicle fractures.

BACKGROUND: Surgical fixation of midshaft clavicle fractures with a single 3.5 mm superior clavicular plate has been associated with a high rate of hardware removal due to symptomatic hardware. Because of this, dual-plating techniques with lower-profile implants have been proposed. However, dual-plating systems have disadvantages, including increased cost and increased surgical morbidity. The aim of this study was to define the rate of symptomatic hardware removal for all midshaft clavicle fractures. METHODS: We retrospectively reviewed information on all patients from 2014 to 2018 at a singl

Robotic-assisted total knee arthroplasty in post-traumatic and previously operated knees with retained hardware: Practical decision-making workflow.

Total knee arthroplasty (TKA) after periarticular fracture fixation, malunion, high tibial osteotomy, or other previous knee surgery differs substantially from routine primary TKA. Retained implants, extra-articular deformity, distorted landmarks, bone defects, scarred soft tissues, and infection risk may all compromise standard instrumentation and implant positioning. Robotic-assisted TKA (RA-TKA) is increasingly used in selected non-standard primary cases because computed tomography -based or imageless planning can define deformity, anticipate implant-hardware conflicts, guide bone resection
