Rat Model of Closed-Head Mild Traumatic Injury and its Validation.

Animal models are crucial for advancing our understanding of mild traumatic brain injury (mTBI) and guiding clinical research. To achieve meaningful insights, developing a stable and reproducible animal model is essential. In this study, we report a detailed description of a closed-head mTBI model and a representative validation method using Sprague-Dawley rats to verify the modeling effect. The model involves dropping a 550 g mass weight from a height of 100 cm directly onto the head of a rat on a destructible surface, followed by a 180-degree turn. To assess the injury, rats underwent a seri

Posttraumatic Parinaud's syndrome as a contrecoup injury in a case of closed head trauma.

The purpose of the study was to present a rare case of post-traumatic Parinaud's syndrome with a history of closed head injury. The clinical characteristics, examination, and management are presented in a 12-year-old boy who was a victim of physical violence at the hands of a young boy who punched him in his chin with his fists, with associated paralysis of the upward gaze of the left eyeball, and convergence nystagmus with pupillary involvement. CT examination indicated posttraumatic lesions in the dorsal midbrain and in the cavity of third ventricle, suggestive of acute hemorrhages. Posttrau

Neurobehavioral functional deficits following closed head injury in the neonatal pig.

Neurobehavioral deficits in higher cortical systems have not been described previously in a large animal model of diffuse brain injury. Anesthetized 3-5 day old piglets were subjected to either mild (142 rad/s) or moderate (188 rad/s) rapid non-impact axial rotations of the head. Multiple domains of cortical function were evaluated 5 times during the 12 day post-injury period using tests of neurobehavioral function devised for piglets. There were no observed differences in neurobehavioral outcomes between mild injury pigs (N=8) and instrumented shams (N=4). Moderately injured piglets (N=7) had

Altered electrodermal response to facial expression after closed head injury.

UNLABELLED: The objective was to determine whether diffuse damage to orbital and ventromedial regions of the prefrontal cortex usually associated with moderate-to-severe closed head injury (CHI) would affect the ability to perceive and respond to socially relevant information. METHODS: Participants with CHI and age-matched non-injured controls were presented with faces that varied with respect to emotional expression while electrodermal activity (EDA) was monitored. Cognitive and general adaptive functioning was also assessed. RESULTS: CHI was associated with a failure to increase EDA in res

An examination of the impact of participation in a conversation group for individuals with a closed head injury.

This study describes the impact of participation in a conversational group for individuals with chronic closed head injury in the Department of Communication Pathology at the University of Pretoria over the period 1995-2000. The information was obtained through a combination of clinical observations by the writers; the examination of data from Pragmatic measures; and a Quality of Life Scale that was compiled and administered (to both the subjects and their significant others) examining the perceived effects of group therapy over time. The results showed that despite the plateauing of pragmatic

Damage to intracranial optic pathways in fatal closed head injury in man.

Head injury is a leading cause of visual impairment. This is partly due to direct trauma to the eye and optic nerve but much of the damage involves the intracranial optic pathways. We have studied the frequency, distribution and nature of the intracranial lesions of the optic pathways at autopsy in 45 cases of severe closed head injury, and examined the correlation between these post-mortem lesions and the ante-mortem clinical findings. Twenty-four of the patients had been involved in road traffic accidents. The ages ranged from 9 to 88 years (mean 46.4), the Glasgow Coma Score (GCS) on admiss

Sixth cranial nerve palsy following closed head injury in a child.

A five year old female had an isolated abducens nerve palsy following closed head injury. There was no associated skull fracture, haematoma, or other cranial nerve injury. The significance, frequency, and differential diagnosis of traumatic sixth cranial nerve injury is discussed, particularly in paediatric patients. Management is symptomatic; occlusion with an eye pad may be used if diplopia is significant. In young children alternate day occlusion of each eye will help prevent amblyopia. Most cases improve within three months and many resolve by six months. Residual palsy at six months is li

Closed head injury and the treatment of sequelae after a motor vehicle accident.

Approximately 2 million closed head injuries (CHIs) occur yearly in the United States. Twenty-five percent of these injuries require hospitalization, and 70,000 to 90,000 of those hospitalized suffer long-term disability. This case conference details one such case of CHI in which the patient ultimately died. Close attention is given to the pathophysiology and treatment of this process. Commonly accepted, as well as investigational, modalities of therapy are discussed.

Somatosensory evoked potential monitoring following severe closed head injury.

This paper describes the results of somatosensory evoked potential (SSEP) monitoring in 65 patients with severe head injury. Intracranial pressure (ICP) monitoring data were available for 63 patients, and arterial-jugular oxygen content (AVDO2) data for 52 patients. Eighty-nine percent of patients with no SSEP activity beyond 50 msec post-stimulus in either hemisphere died or were vegetative survivors (3 month Glasgow Outcome Score). All 17 patients with a good or moderate outcome had long latency cortical activity (i.e. > or = 70 msec post-stimulus) in both hemispheres. Among patients with ab

Blinded comparison of cranial CT and MR in closed head injury evaluation.

PURPOSE: To compare CT and MR in the evaluation of acute head injury. METHODS: One hundred seven consecutive patients who were referred to the emergency department and underwent both MR and CT cranial examinations within 48 hours were retrospectively reviewed. The films were interpreted by two neuroradiologists blinded to all patient information. RESULTS: The sensitivity of MR was significantly higher than that of CT for the detection of contusion, shearing injury, subdural and epidural hematoma, and sinus involvement. The sensitivity of CT was significantly higher than that of MR for the ev

Transcranial Doppler ultrasound following closed head injury: vasospasm or vasoparalysis?

Nine patients suffering closed head injury were studied using transcranial Doppler ultrasound recording of the basal cerebral arteries. On admission, six patients had Glasgow Coma Scores of 7 or less, while three had initial scores of 8-10. Eight of the nine patients (82%) developed abnormally high mean velocities (greater than or equal to 110 cm/s) in one or more vessels. The onset, duration, and amplitude of the altered flow velocities were somewhat different from those that are seen with vasospasm after aneurysmal subarachnoid hemorrhage, which tend to occur later. Cerebral angiography in o

Memory function after closed head injury: a review of the quantitative research.

Studies which provide quantitative analyses of memory function following closed head injury were reviewed. Specific issues covered include criteria for assessing post-traumatic amnesia (PTA), PTA duration, relation of PTA duration to later memory impairment, memory functions within PTA, and comparisons between PTA and other amnesic conditions. Issues associated with the assessment of retrograde amnesia were also reviewed. With regard to memory function after PTA has terminated, we described and discussed normative data, the time course of recovery, and issues related to the nature of the later

Brainstem and Cortical Spreading Depolarization in a Closed Head Injury Rat Model.

Traumatic brain injury (TBI) is the leading cause of death in young individuals, and is a major health concern that often leads to long-lasting complications. However, the electrophysiological events that occur immediately after traumatic brain injury, and may underlie impact outcomes, have not been fully elucidated. To investigate the electrophysiological events that immediately follow traumatic brain injury, a weight-drop model of traumatic brain injury was used in rats pre-implanted with epidural and intracerebral electrodes. Electrophysiological (near-direct current) recordings and simulta

Coping and Health-Related Quality of Life after Closed Head Injury.

OBJECTIVES: In the present study the relationship between illness coping and health-related quality of life (HRQOL) in patients after closed head injury (CHI) was analyzed. Furthermore, the study was performed to assess the relative significance of clinical, neuroradiological, psychosocial variables and coping activities after CHI. We hypothesized that the effect of a depressive coping style is significantly stronger than that of all other variables considered. PATIENTS AND METHODS: This cross-sectional study took place at the outpatient clinic of the Department of Neurosurgery of the Univers

[Venous sinus thrombosis after closed head injury: case report].

Dural sinus thrombosis after closed head injury is seen very rarely and its pathophysiological mechanisms are not yet understood. An 18-year-old man who had been crushed in an automobile had closed head injury with Glasgow Coma Score 4. Computerized tomography showed diffuse cerebral edema and right occipital linear fracture extending to the internal jugular foramen. Because of high- level intracranial pressure, he underwent bilateral frontotemporoparietal craniectomy. After surgery, magnetic resonance scan with venography showed that the right internal jugular vein was obliterated from the to

Prevalence of sleep disturbance in closed head injury patients in a rehabilitation unit.

UNLABELLED: Traumatic brain injury (TBI) is a leading cause of disability in young people in the United States. Disorders of arousal and attention are common in closed head injury (CHI). Daytime drowsiness impairs participation in rehabilitation, whereas nighttime wakefulness leads to falls and behavioral disturbances. Sleep disturbances in TBI reported in the literature have included excessive daytime somnolence, sleep phase cycle disturbance, narcolepsy, and sleep apnea. Although well known to the clinician treating these patients, the extent and prevalence of disrupted sleep in patients in

Procalcitonin as a diagnostic marker in patients with aspiration after closed head injury.

BACKGROUND: Aspiration of gastric content frequently induces early onset of pneumonia in patients with impaired consciousness after closed head injury and thus worsens the prognosis. Early detection of aspiration and appropriate therapy are essential. The purpose of the study was to assess the diagnostic value of procalcitonin (ProCT) in aspiration of gastric content and to evaluate its prognostic impact in patients with closed head injury. METHODS: Twenty-three patients with isolated closed head injury (Glasgow Coma Scale [GCS] score < or = 8) were studied. Bronchoscopy was done on admission
