Primary closure versus delayed or no closure for traumatic wounds due to mammalian bite.

BACKGROUND: Mammalian bites are a common presentation in emergency and primary healthcare facilities across the world. The World Health Organization recommends postponing the suturing of a bite wound but this has not been evaluated through a systematic review. OBJECTIVES: To assess the effects of primary closure compared with delayed closure or no closure for mammalian bite wounds. SEARCH METHODS: In July 2019 we searched the Cochrane Wounds Specialised Register; the Cochrane Central Register of Controlled Trials (CENTRAL); Ovid MEDLINE (including In-Process & Other Non-Indexed Citations); O

Physical Activity Patterns and Behavioral Resilience Among Foggia University Students During the COVID-19 Pandemic: A Public Health Perspective.

Background: The (COVID-19) pandemic profoundly disrupted daily routines and physical activity (PA), especially among university students, due to restrictions and limited access to sports facilities. As this group is particularly vulnerable to sedentary lifestyles and mental health issues, understanding their PA patterns is crucial. This study explores overall and domain-specific PA levels and the influence of sociodemographic factors, offering insights for promoting sustainable PA strategies in higher education during and beyond health crises. Methods: A cross-sectional online survey was condu

Mixed Method Evaluation of Louisiana's Prerelease Medicaid Enrollment Program, 2017-2019.

BACKGROUND: Over 600,000 people leave US prisons annually. Many are eligible for Medicaid upon release but may need support to enroll. Carceral facilities in nearly half of states have implemented systems to facilitate Medicaid access for those leaving incarceration, but there is limited information on program implementation models or outcomes. OBJECTIVES: To evaluate implementation and initial outcomes of Louisiana's prison-based Prerelease Medicaid Enrollment Program. METHODS: In this mixed-methods study, we assessed enrollment in Louisiana Medicaid at time of release from prison in the 2

Observational study of the use of infection control interventions for Mycobacterium tuberculosis in pediatric facilities.

INTRODUCTION: Hospital transmission of Mycobacterium tuberculosis (TB) is a problem in US facilities where adults are treated. However, specific guidelines for facilities in which pediatric patients are cared for have never been defined, nor has any study attempted to assess pediatric health care worker (HCW) compliance with TB infection control (IC) guidelines. METHODS: An observational study was performed in two pediatric inpatient hospitals from May, 1996, to December, 1997. A trained observer tallied persons (i.e. professional HCWs, ancillary HCWs and non-HCWs) entering and leaving occupi

"Stakeholder" attitudes over time toward the closing of a state hospital.

For four groups--patients (N = 80), their families or lay caregivers (N = 120), hospital workers (N = 124), and the public (N = 108)--attitudes toward the decision to close the only long-term, urban state hospital are traced over time. Initially, patients were most supportive of the closing decision (65.4%), followed by family members (39.8%), the general public (27.8%), and workers (10.4%). A majority of all groups favored fixing the hospital. Almost half of the clients and more than half of other stakeholders expressed concerns about homelessness. Most attitudes were consistent despite respo

United States aid cut: alternatives for sustainable financing for health.

Recent United States (US) foreign aid cuts, notably through Executive Order 14169 signed on January 20, 2025, have disrupted health programs in low- and middle-income countries (LMICs), threatening progress in combating HIV/AIDS, tuberculosis, and malaria. With 37 LMICs relying on US aid for over 10% of their health budgets, the funding pause has led to closures of critical health facilities, such as 200 in Afghanistan, impacting 1.84 million people. This commentary explores sustainable financing solutions, emphasizing the Debt-to-Health Swap (D2H), where debt is forgiven in exchange for healt

Can regionalization decrease the number of deaths for children who undergo cardiac surgery? A theoretical analysis.

OBJECTIVE: The association between high case volumes and better patient outcomes has been demonstrated for many surgical procedures and medical treatments, including surgery for children with congenital heart disease. To simulate the effects of regionalization of pediatric cardiac surgery, we assessed the impact of reducing the number of pediatric cardiac centers on surgical mortality and patient's travel distance. METHODS: This study used abstracted statewide hospital discharge data from California from 1995 to 1997. Case volume and in-hospital mortality for pediatric cardiac surgeries at ea

New norovirus classified as a recombinant GII.g/GII.1 causes an extended foodborne outbreak at a university hospital in Munich.

BACKGROUND: Noroviruses are among the most prevalent causative agents for gastroenteritis worldwide. The low infectious dose, its stability in the environment, and its genetic variability enable the virus to cause outbreaks, especially in health care facilities and other similar settings. Genotype II.4 has been most prevalent over the last years. OBJECTIVES: To characterize an extended norovirus outbreak at a university hospital in Munich, Germany, molecularly and epidemiologically. STUDY DESIGN: The outbreak affecting more than 100 persons within 3 days was monitored by real time PCR. The r

A qualitative exploration of the downstream impact of a sustained disruption to the methadone supply in Mexico: effects on the mental, financial, and social health of patients with opioid use disorder in Tijuana.

BACKGROUND: Medications for opioid use disorder (MOUD) are considered the gold standard long-term treatment for opioid use disorder (OUD) as they have a range of health and psycho-social benefits. Sudden reduction or interruptions in MOUD can have both immediate and long-term consequences for patients. We sought to qualitatively examine the impact of the prolonged closure of Mexico's main methadone production facility (Psicofarma) in 2023 on MOUD patients in Tijuana, Mexico. METHODS: We conducted semi-structured qualitative interviews with 20 MOUD patients in Tijuana, Mexico, from May to Augu

Evaluating a maternal and child health capacity-building program in Senegal: environment-adjusted district efficiency and gap reduction using DHIS2 data.

BACKGROUND: Many low-and middle-income countries have expanded maternal and child health services, yet district-level efficiency and equity remain concerns, particularly in settings that rely on international assistance for health. Senegal uses the national DHIS2 platform to monitor maternal and child health indicators, but little is known about how capacity-building programs affect district efficiency after accounting for contextual need and remaining inefficiency gaps. METHODS: Data from 2019 and 2023 were analyzed for 27 health districts in central Senegal. Inputs were numbers of doctors,

Embolic Strokes in Paroxysmal Atrial Fibrillation: Anticoagulation Failure or Something Else?

Infective endocarditis (IE) often presents with various signs and/or symptoms. However, at times, IE can present without outstanding clinical evidence but may carry devastating consequences if not detected and treated. We present a case of an 81-year-old female with paroxysmal atrial fibrillation who presented to the emergency department with slurred speech. Her National Institutes of Health Stroke Scale (NIHSS) score was one, and her physical examination was unremarkable. Brain imaging revealed bilateral multiple acute supratentorial and infratentorial infarcts. The patient was fully complian

Obstetric emergency readiness in rural emergency departments: a cross-sectional survey of birthing and non-birthing hospitals in Montana, USA.

BACKGROUND: Rural emergency departments (EDs) increasingly serve as frontline providers of obstetric care due to widespread closures of labour and delivery (L&D) units. Building on a prior study that assessed hospital-level obstetric readiness using the WHO Emergency Obstetric Care (EmOC) Indicators, this study evaluates ED-specific preparedness for obstetric emergencies in a rural US state, with implications for similar settings globally. METHODS: A cross-sectional survey was conducted in Autumn 2024 involving ED medical and nursing staff from both birthing and non-birthing hospitals. The su

Hospital obstetric volume and maternal outcomes: Does hospital size matter?

INTRODUCTION: In recent decades, centralization of health care has resulted in a number of obstetric unit closures. While studies support better infant outcomes in larger facilities, few have investigated maternal outcomes. We investigated obstetric unit closures over time and whether obstetric volume is associated with onset of labor, postpartum hemorrhage (PPH) and obstetric anal sphincter injury (OASIS). MATERIAL AND METHODS: All births registered in Sweden between 1992 and 2019 (Medical Birth Register, N = 2 931 140), linked with data on sociodemographic characteristics and maternal/infan

Veterans Health Administration Pain Management Quality Improvement in a Post-Commission on Accreditation for Rehabilitation Facilities Era.

Tertiary interdisciplinary pain rehabilitation programs (IPRPs) provide integrated, comprehensive pain rehabilitation using an evidence-based biopsychosocial approach delivered by teams offering multi-modal care. A 2009 Veterans Health Administration (VHA) Directive mandated implementation of Commission on Accreditation for Rehabilitation Facilities (CARF) accredited IPRPs in each Department of Veterans Affairs (VA) service network. Commission on Accreditation for Rehabilitation Facilities accreditation included regular program surveys to reinforce continuous quality and improvement standards.
