A Randomized Controlled Trial to Assess the Impact of Ceramic Water Filters on Prevention of Diarrhea and Cryptosporidiosis in Infants and Young Children-Western Kenya, 2013.

Cryptosporidium is a leading cause of diarrhea among Kenyan infants. Ceramic water filters (CWFs) are used for household water treatment. We assessed the impact of CWFs on diarrhea, cryptosporidiosis prevention, and water quality in rural western Kenya. A randomized, controlled intervention trial was conducted in 240 households with infants 4-10 months old. Twenty-six weekly household surveys assessed infant diarrhea and health facility visits. Stool specimens from infants with diarrhea were examined for Cryptosporidium. Source water, filtered water, and filter retentate were tested for Crypto

Gay and lesbian perceptions of discrimination in retirement care facilities.

Much research on older gay, lesbian, bisexual and transgender (GLBT) adults has focused on refuting the widely held misconceptions people have about GLBT lifestyles. To date, however, few studies on older GLBTs have examined their social and health care needs. Further, most studies have collected survey samples of older GLBT adults in large metropolitan areas and have not specifically addressed discrimination or bias in retirement care facilities. In the current exploratory study on perceptions of discrimination and bias in retirement care facilities, we surveyed a wide age range GLBT adults i

Coverage with Influenza, Respiratory Syncytial Virus, and COVID-19 Vaccines Among Nursing Home Residents - National Healthcare Safety Network, United States, November 2024.

Nursing home residents are at elevated risk for severe complications from respiratory viruses, including SARS-CoV-2, influenza, and respiratory syncytial virus (RSV). Nursing homes are required to report COVID-19 vaccination coverage and can voluntarily report influenza and RSV vaccination coverage among residents to CDC's National Healthcare Safety Network. The purpose of this study was to assess COVID-19, influenza, and RSV vaccination coverage among nursing home residents early in the 2024-25 respiratory virus season. As of November 10, 2024, 29.7% of nursing home residents had received a 2

Health care workers' ability and willingness to report to duty during catastrophic disasters.

Catastrophic disasters create surge capacity needs for health care systems. This is especially true in the urban setting because the high population density and reliance on complex urban infrastructures (e.g., mass transit systems and high rise buildings) could adversely affect the ability to meet surge capacity needs. To better understand responsiveness in this setting, we conducted a survey of health care workers (HCWs) (N =6,428) from 47 health care facilities in New York City and the surrounding metropolitan region to determine their ability and willingness to report to work during various

Determinants of Age-Appropriate Childhood Immunization Uptake: A Cross-Sectional Study of Caregiver, Child, Knowledge, Access, and Attitudinal Factors.

BACKGROUND: Childhood immunization is among the most cost-effective public health strategies for reducing under-five morbidity and mortality. However, full immunization coverage for age remains suboptimal in many low- and middle-income countries due to factors related to caregivers, children, health systems, and sociocultural contexts. The study aimed to assess the prevalence of full immunization for age and identify its associated determinants across multiple domains. METHODS: A community-based cross-sectional study was conducted among 364 caregiver-child pairs. Full immunization for age was

Factors associated with postnatal care for newborns in Zambia: analysis of the 2013-14 Zambia demographic and health survey.

BACKGROUND: The importance of postnatal care cannot be overemphasised. Various studies undertaken worldwide have found that PNC is critical for the survival of newborns. However, in Zambia, despite much emphasis by the government and various international Organisations on the need for PNC, coverage continues to be low. This study attempted to assess the demographic and socio-economic factors associated with newborns' receipt of PNC and the timing of first PNC in Zambia. METHODS: Based on data from the 2013-14 Zambia Demographic and Health Survey (ZDHS), this study used bivariate, stepwise bin

How mental health providers spend their time: a survey of 10 Veterans Health Administration mental health services.

BACKGROUND: Allocation of provider time across clinical, administrative, educational, and research activities may influence job satisfaction, productivity, and quality of care, yet we know little about what determines time allocation. AIMS: To investigate factors associated with time allocation, we surveyed all mental health providers in one Veterans Health Administration (VHA) network. We hypothesized that both facility characteristics (academic affiliation, type of organization of services, serving as a hub for treatment of severely mentally ill, facility size) and individual provider chara

Good results with the Ponseti method: a multicenter study of 162 clubfeet followed for 2-5 years.

BACKGROUND AND PURPOSE: In 2002-2003, several hospitals in Norway introduced the Ponseti method for treating clubfoot. The present multicenter study was conducted to evaluate the initial results of this method, and to compare them to the good results reported in the literature. PATIENTS AND METHODS: 116 children with 162 congenital idiopathic clubfeet who were born between 2004 and 2006 were treated with the Ponseti method at 8 hospitals in Norway. All children were prospectively registered at birth, and 116 feet were assessed according to Pirani before treatment was started. 63% used a stand

Effect of hospital volume of thyroidectomies on outcomes following substernal thyroidectomy.

BACKGROUND: Substernal thyroidectomy (ST), as compared to conventional, cervical thyroidectomy, is a technically demanding procedure that is associated with increased morbidity and mortality. We tested the hypothesis that outcomes following ST are improved at centers that perform a high volume of thyroidectomies. METHODS: Patients who underwent ST from 1998 to 2004 were extracted from the New York State Statewide Planning and Research Cooperative System database. Hospital volume of thyroidectomies was divided into low (<33 per year), middle (33-99 per year), and high (>or=100 per year) volume

Characterization of infectious aerosols in health care facilities: an aid to effective engineering controls and preventive strategies.

Assessment of strategies for engineering controls for the prevention of airborne infectious disease transmission to patients and to health care and related workers requires consideration of the factors relevant to aerosol characterization. These factors include aerosol generation, particle size and concentrations, organism viability, infectivity and virulence, airflow and climate, and environmental sampling and analysis. The major focus on attention to engineering controls comes from recent increases in tuberculosis, particularly the multidrug-resistant varieties in the general hospital popula

Predictors of hospital transfer and associated risks of mortality in acute pancreatitis.

BACKGROUND: There is limited research in prognosticators of hospital transfer in acute pancreatitis (AP). Hence, we sought to determine the predictors of hospital transfer from small/medium-sized hospitals and outcomes following transfer to large acute-care hospitals. METHODS: Using the 2010-2013 Nationwide Inpatient Sample (NIS), patients ≥18 years of age with a primary diagnosis of AP were identified. Hospital size was classified using standard NIS Definitions. Multivariable analyses were performed for predictors of "transfer-out" from small/medium-sized hospitals and mortality in large acu

Fifteen-Year Trends in Incidence of Cardiogenic Shock Hospitalization and In-Hospital Mortality in the United States.

Background There is a lack of contemporary data on cardiogenic shock (CS) in-hospital mortality trends. Methods and Results Patients with CS admitted January 1, 2004 to December 31, 2018, were identified from the US National Inpatient Sample. We reported the crude and adjusted trends of in-hospital mortality among the overall population and selected subgroups. Among a total of 563 949 644 hospitalizations during the period from January 1, 2004, to December 30, 2018, 1 254 358 (0.2%) were attributed to CS. There has been a steady increase in hospitalizations attributed to CS from 122 per 100 00

A network analysis of housing quality indicators and depression in women.

Numerous studies have detected associations between poor housing quality and increased risk for mental illness. However, it currently remains unclear in associations between poor housing quality and increased risk for women's mental illness which housing quality indicators drive this association and hence which specific indicators should be prioritised in housing quality assessments or improvements. In a sample of up to 9,669 pregnant women, we used a network analysis to investigate cross-sectional associations between poor housing quality indicators (e.g., house size, facilities, leaks or con
