Community Care in Action: Scaling Doula Support Through a Health Department-Community Partnership.

INTRODUCTION: Doula support is increasingly recognized as a promising strategy to address racial inequities in maternal health. The New York City Health Department launched the Citywide Doula Initiative (CDI) in March 2022 to expand access to free, community-based doula support in disinvested neighborhoods. METHODS: To evaluate the program's first year of implementation, we conducted semi-structured interviews with 44 clients, doulas, doula-organization staff, and Health Department staff between November 2022 and April 2023. We coded interview notes using template analysis (a thematic analysi

Providing general and preconception health care to low income women in family planning settings: perception of providers and clients.

This study examines both provider and client perceptions of the extent to which general health concerns are addressed in the context of publicly supported family planning care. A mail survey of family planning providers (n = 459) accepting Medicaid-covered clients in Arkansas and Alabama gathered data on reported actions and resource referral availability for ten categories of non-contraceptive health concerns. A telephone survey of recent family planning clients of these providers (n = 1991) gathered data on the presence of 16 health concerns and whether and how they were addressed by the fam

Supporting cardiac patient physical activity: a brief health psychological intervention.

BACKGROUND: One of the most important risk factors for coronary artery disease is physical inactivity. Health psychological research demonstrates the importance of planning for behaviour change success. Consequently, a health action process approach (HAPA) model-based design to support the uptake of physical activity was initiated for the first time in an acute cardiac ward. METHODS: For impact evaluation, a control group (CG) and an intervention group (IG) of coronary artery disease patients were compared in a controlled longitudinal study. Baseline assessment included socio-demographic vari

Family planning for teens: strategies for improving outreach and service delivery in public health settings.

The persistent underuse of family planning services by inner-city, low-income, sexually active youth underscores the importance of testing innovative programs that maximize participation. Presented in this paper is an analysis of a Chicago public health clinic's special program for adolescents that originated from the staff's observations of the scheduling, educational, and support needs of teens seeking family planning services from a traditionally managed public health facility. Between December 1982, when the special program--the Teen Clinic--was implemented, and March 1985, more than 600 a

Society of Family Planning Research Practice Support: Strategies and considerations for addressing race and racism in quantitative family planning studies.

OBJECTIVES: Family planning researchers have not critically engaged with topics of race, racism, and associated concepts like ethnicity. This lack of engagement contributes to the reproduction of research that reifies racial hierarchies rather than illuminate and interrupt the processes by which racism affects health. This Research Practice Support paper lays out considerations and best practices for addressing race and racism in quantitative family planning research. STUDY DESIGN: We are scholars with racialized identities and expertise in racial health equity in family planning research. We

Addressing stroke family caregivers' mental health through person‑ and family‑centred rehabilitation.

PURPOSE: We aim to summarize the literature on family caregiver experiences and support needs following stroke and highlight person- and family-centred care (PFCC) as a potential approach to addressing gaps in caregiver support and mental health. MATERIALS AND METHODS: Literature was reviewed to examine caregiver mental health outcomes, caregiver-focused interventions, challenges integrating caregiver support into patient-centred models of care, and the potential role of PFCC. RESULTS: Caregivers are at increased risk of adverse mental health outcomes, including stress, burden, and psycholog

Classification of Mental Health Establishments in India According to Levels of Care: A Policy Perspective.

The recent classification of mental health establishments under the Mental Healthcare Act, 2017, is based largely on institutional affiliation. This viewpoint proposes a care-level-based classification comprising acute, intermediate, and rehabilitation services. Such an approach may better reflect clinical needs, support service planning, and align with the objectives of the Mental Health Care Act.

The role of evidence in formulating public health programs to prevent oral disease and promote oral health in the United States.

A major goal of federal, state, and local health agencies is to reduce the burden of disease in populations. To obtain sufficient resources to achieve this goal, they must document the importance of the health problems addressed and the impact and efficiency of programs. CDC supports core activities within state and local health departments to promote health and prevent disease. This presentation will focus on the need for evidence to document the effectiveness of these activities that include routine monitoring of oral health, risk behaviors, and other factors; implementing effective populati

Successful proof of concept of family planning and immunization integration in Liberia.

Globally, unmet need for postpartum family planning remains high, while immunization services are among the most wide-reaching and equitable interventions. Given overlapping time frames, integrating these services provides an opportunity to leverage existing health visits to offer women more comprehensive services. From March through November 2012, Liberia's government, with support from the Maternal and Child Health Integrated Program (MCHIP), piloted an integrated family planning and immunization model at 10 health facilities in Bong and Lofa counties. Vaccinators provided mothers bringing i

Social networks and community prevention coalitions.

This study investigates the links between community readiness and the social networks among participants in Communities That Care (CTC), community-based prevention coalitions. The coalitions targeted adolescent behavior problems through community risk factor assessments, prioritization of risk factors, and selection/implementation of corresponding evidence-based family, school, and community programs. Key leaders (n = 219) in 23 new CTC sites completed questionnaires focusing on community readiness to implement CTC and the respondents' personal, work, and social organization links to other key

Association of Formal and Informal Advance Care Planning with Multidimensional Health Literacy in Patients Undergoing Hemodialysis.

KEY POINTS: Higher critical health literacy was associated with informal advance care planning, while higher total health literacy was associated with both informal and formal advance care planning in patients receiving hemodialysis. Patients unsure about treatment preferences had lower total health literacy, and lack of opportunity was the most common barrier to advance care planning engagement. Enhancing health literacy and providing structured opportunities for advance care planning may support patient-centered decision making in dialysis care. BACKGROUND: Although qualitative studies sugg

A randomized controlled trial of a community health worker intervention in a population of patients with multiple chronic diseases: Study design and protocol.

UNLABELLED: Upstream interventions - e.g. housing programs and community health worker interventions- address socioeconomic and behavioral factors that influence health outcomes across diseases. Studying these types of interventions in clinical trials raises a methodological challenge: how should researchers measure the effect of an upstream intervention in a sample of patients with different diseases? This paper addresses this question using an illustrative protocol of a randomized controlled trial of collaborative-goal setting versus goal-setting plus community health worker support among pa
