Costs of broad-spectrum antibiotic use for acute sinusitis, chronic bronchitis, and pneumonia in a managed care population.

Respiratory infections place a heavy burden on patients, providers, employers, and health care systems. The prescribing of antibiotics is common, despite the fact that many respiratory conditions are caused by viruses. The economic effect of treating respiratory tract infections with broad-spectrum antibiotics was retrospectively analyzed by means of health care claims data from six managed care health plans affiliated with a large national insurer. A regression model was used to adjust for factors that can influence treatment costs, such as age, baseline cost, retreatment, and drug cost. The

Pandemic influenza preparedness in the WHO South-East Asia Region: a model for planning regional preparedness for other priority high-threat pathogens.

Pandemic influenza preparedness has contributed significantly to building, strengthening and maintaining countries' core capacities to prepare for health emergencies. The Pandemic influenza preparedness framework for the sharing of influenza viruses and access to vaccines and other benefits (the PIP framework) was adopted by the World Health Assembly in 2011. The experiences and lessons learnt from the implementation of the PIP framework have provided insights that can be used to strengthen preparedness for epidemics of other priority high-threat pathogens in the World Health Organization (WHO

Surgical health policy 2025-35: strengthening essential services for tomorrow's needs.

Progress towards The Lancet Commission on Global Surgery's 2030 targets has been too slow and too patchy, particularly in low-income and middle-income countries. The unmet need for surgery has continued to grow, reaching at least 160 million operations per year. Ensuring high-quality surgical care remains a crucial global challenge, with 3·5 million adults dying after surgery each year. The COVID-19 pandemic exposed the fragility of surgical services long undermined by chronic underfunding, workforce shortages, and under-resourced infrastructure. However, The Lancet Commission on Global Surger

Once BITTEN, Twice Shy: An Applied Trauma-Informed Healthcare Model.

Nurses need a pragmatic theory to understand and respond to the impact of vulnerable patients' previous healthcare experiences, as these are likely to influence response and adherence to treatment plans. The authors of this paper present the new BITTEN (Betrayal history by health-related institutions, Indicator for healthcare engagement, Traumas related to healthcare, Trust in healthcare providers, patient Expectations and Needs) Model of Trauma-Informed Healthcare. BITTEN identifies patients' current healthcare expectations and needs as a function of their previous betrayal by healthcare syst

Lessons learned from supporting local planning for extreme heat in North Carolina.

The impacts of extreme heat are broad and systemic, affecting not only human health and well-being, but also infrastructure, energy systems, and public services. Many of the best strategies to address these impacts are implemented at the local level, by local governments and their partners. This paper covers the approaches taken by the North Carolina State Resilience Office (SRO), the North Carolina State Climate Office (SCO) and their partners to support the development of heat action plans by local governments in their state. These agencies collaborated to develop planning support resources

The impact of COVID-19 on adolescents with eating disorders: Increased need for medical stabilization and decreased access to care.

OBJECTIVE: We aimed to identify trends of patients with eating disorders (EDs) requiring hospitalization before and during the pandemic at a children's hospital in the southeastern United States. METHOD: A retrospective chart review was completed for 71 adolescents and young adults (ages 10-21 years; M = 14.61, SD = 2.121). RESULTS: Results indicated a 188% increase in ED hospital admissions since the pandemic, with patients presenting with increased rates of comorbid mental health diagnoses (p = .009). During COVID-19, the development of temporary outpatient multidisciplinary discharge plan

'I Wish the School Had a Better Understanding of the Diagnosis': parent perspectives on educational needs of students with sex chromosome aneuploidies.

Students with sex chromosome aneuploidies (SCAs) are at increased risk for learning disabilities and often require individualized supports in the school setting. Parents of students with rare disorders such as SCAs possess a unique understanding of their child's educational experiences and play a crucial role in the development of successful school support plans. This international survey study aimed to inductively capture parent perspectives on educational needs and supports for students with SCAs. Parents of youth with SCAs ages 5-21 years (n = 305) provided free-text responses to open-ended

Family planning in Viet Nam: a vigorous approach.

Viet Nam, a country in transition, is vigorously implementing its population and family planning policies and plans, despite the fact that increased government expenditure for family planning only amounts to some US $0.15 per capita, far short of the minimum $0.60 required. An increased range of family planning methods will be offered to the population through a "cafeteria" approach. Genital tract infections will be reduced, as well as the incidence of abortion. Research continues on injectable and implanted contraceptives, and the use of quinacrine nonsurgical female sterilization. In the lon

Expanding the Vision for Differentiated Service Delivery: A Call for More Inclusive and Truly Patient-Centered Care for People Living With HIV.

BACKGROUND: Simplifying antiretroviral therapy for clinically stable people living with HIV (PLHIV) is important but insufficient to meet their health care needs, including prevention and treatment of tuberculosis and noncommunicable diseases, routine primary care, and family planning. Integrating these services into differentiated service delivery (DSD) platforms is a promising avenue to achieve such coverage. We propose a transition from an HIV-focused "DSD 1.0" to a patient-centered "DSD 2.0" that is inclusive of additional chronic care services for PLHIV. DISCUSSION: The lack of coordinat

To what extent is equity entrenched in HIV/AIDS-related policy documents in Ethiopia? A policy content analysis.

INTRODUCTION: It is important to assess the extent of mentions of equity in policy and strategic documents for several reasons. First, it helps us understand the root causes of inequities, revealing both failures and successes. Second, it shows the readiness of stakeholders and leaders to take actions towards equity. Third, it identifies areas of improvement to plan strategies towards equity. The objective of this study was to assess the degree to which equity is incorporated in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS)-related policy documents in Ethiopia.

The Employer-Led Health Care Revolution.

To tame its soaring health care costs, intel tried many popular approaches: "consumer-driven health care" offerings such as high-deductible/low-premium plans, on-site clinics and employee wellness programs. But by 2009 intel realized that those programs alone would not enable the company to solve the problem, because they didn't affect its root cause: the steadily rising cost of the care employees and their families were receiving. Intel projected that its health care expenditures would hit a whopping $1 billion by 2012. So the company decided to try a novel approach. As a large purchaser of h

The monkeypox virus: A public health challenge threatening Africa.

On 23 July 2022, the World Health Organization reported monkeypox cases in over 75 countries and, as a result, declared the virus a Public Health Emergency of International Concern (PHEIC). Despite Africa being the centre of the zoonotic disease evolution, its healthcare systems have not provided optimum attention to the problem. The African healthcare system is under the threat of a repeat of the situation that transpired during the COVID-19 pandemonium if stringent measures are not implemented immediately. Lessons learned from the COVID-19 pandemic should inform emergency preparedness and re

Developing and expanding contributions of the global laboratory network for poliomyelitis eradication, 1997-1999.

In 1988, the World Health Assembly resolved to eradicate poliomyelitis globally by 2000. Substantial progress toward achieving this goal has been reported from all countries where polio is endemic, and three regions of the World Health Organization (WHO) (American Region, European Region, and Western Pacific Region) appear to be free of indigenous wild poliovirus transmission. One key strategy for polio eradication is establishing sensitive surveillance systems for polio (through notification of acute flaccid paralysis [AFP] cases) and poliovirus. To ensure that specimens from AFP cases underg

Managing health systems under armed conflict: a scoping review of international evidence and a document review of Iranian experiences.

Wars and armed conflicts are among the most critical crises that can severely impact a country's vital infrastructures, particularly the health system. During wartime, health systems face major challenges, including attacks on healthcare facilities and personnel, which disrupt access to services and undermine public health. This study aims to conduct a scoping review to assess reported international strategies for wartime health system management, focusing on resource allocation, workforce distribution, financing, and access to essential supplies, followed by a document review of Iranian exper

What slows the progress of health systems strengthening at subnational level? A political economy analysis of three districts in Uganda.

There is increasing recognition that without stronger health systems, efforts to improve global health and Universal Health Coverage cannot be achieved. Over the last three decades, initiatives to strengthen health systems in low-income countries have attracted huge investments in the context of achieving the Millennium Development Goals, the Sustainable Development Goals, as well as Universal Health Coverage. Yet, health system inadequacies persist, especially at the subnational level. Our paper presents a political economy analysis featuring a three-district case study in Uganda, where distr

Crisis Standards of Care Implementation at the State Level in the United States.

INTRODUCTION: In 2009, the Institute of Medicine published guidelines for implementation of Crisis Standards of Care (CSC) at the state level in the United States (US). Based in part on the then concern for H1N1 pandemic, there was a recognized need for additional planning at the state level to maintain health system preparedness and conventional care standards when available resources become scarce. Despite the availability of this framework, in the years since and despite repeated large-scale domestic events, implementation remains mixed. PROBLEM: Coronavirus disease 2019 (COVID-19) rejuven

Uncertainty and operational considerations in mass prophylaxis workforce planning.

BACKGROUND: The public health response to an influenza pandemic or other large-scale health emergency may include mass prophylaxis using multiple points of dispensing (PODs) to deliver countermeasures rapidly to affected populations. Computer models created to date to determine "optimal" staffing levels at PODs typically assume stable patient demand for service. The authors investigated POD function under dynamic and uncertain operational environments. METHODS: The authors constructed a Monte Carlo simulation model of mass prophylaxis (the Dynamic POD Simulator, or D-PODS) to assess the conse

Born Too Soon: learning from the past to accelerate action in the next decade.

PROGRESS: This paper is a narrative review that takes stock of the progress in addressing preterm birth over the past decade - notably on policies, national plans, innovation, evidence, social mobilisation, and community engagement - to inform future progress on preterm birth. At the global policy level, many countries have strongly supported collective initiatives and resolutions on maternal and newborn health relevant to preterm birth in multilateral fora, most recently through a World Health Assembly resolution calling for a revival amongst the global community on stalled progress for mater
