Clopidogrel resistance is common in patients undergoing vascular and coronary interventions.

OBJECTIVES: "Clopidogrel resistance," also defined as heightened platelet reactivity (HPR) while on clopidogrel therapy, may lead to a sub-optimal antiplatelet effect and a potential thrombotic event. There is limited literature addressing the prevalence of HPR in a large cohort of patients receiving either coronary or endovascular interventions. METHODS: In a large integrated healthcare system, patients with a P2Y12 reaction units (PRU) test were identified. HPR was defined as a PRU ≥ 200 during clopidogrel therapy. Vascular and coronary interventions were identified utilizing CPT codes, HPR

Variations in surgical practice and adverse events following isolated proximal humerus fracture in adults - a comparative longitudinal cohort study of 53,852 patients from Denmark, England, and Sweden.

BACKGROUND: Proximal humeral fractures (PHFs) are common injuries, but their management has remained controversial. Recent high-level evidence from randomized clinical trials has shown that for many PHFs, surgical treatment is not superior to non-surgical treatment. Our primary aim was to conduct a multi-nation assessment of temporal trends and changes in surgical treatment of isolated PHF in Denmark, England, and Sweden. A secondary aim was to estimate the incidence of serious adverse events (SAEs) within 30 days (and death within 90 days) of primary surgery. METHODS: This population-based c

Real-World Cardiotoxicity of Biosimilar versus Reference Trastuzumab in Early HER2-Positive Breast Cancer.

BACKGROUND: Trastuzumab has greatly improved outcomes in patients with human epidermal growth factor receptor 2-positive breast cancer. To reduce costs and increase access, biosimilar products were approved after trials demonstrated short-term safety similar to that of reference trastuzumab. However, real-world data on cardiac safety are limited. OBJECTIVES: We evaluated the adoption of biosimilar trastuzumab and compared heart failure (HF) risk between users of reference and biosimilar trastuzumab. METHODS: Patients aged ≥18 years with breast cancer who received trastuzumab from 2018 to 202

Comparative Analysis of Work Relative Value Units in Surgical Treatment of Common Orthopaedic Fractures: Ankle, Distal Radius, and Proximal Hip: A Propensity Score-Matched Comparative Study.

BACKGROUND: Ankle, distal radius, and proximal femur fractures are among the top three most common fractures. Despite the high prevalence and cost of these fractures, the relative valuation of these common fractures within the current US healthcare system has not been evaluated. This study aims to evaluate whether ankle fracture fixation is adequately compensated for in the hospital setting compared with distal radius and proximal femur fractures, using the National Surgical Quality Improvement Program database. METHODS: Ankle, distal radius, and proximal femur fracture fixations were identif

Unveiling the gap in heart failure: a Brazilian Unified Health System study.

BACKGROUND: Heart failure (HF) decompensation is the leading cause of hospitalisations in developed countries and the third most common cause in Brazil. Underdiagnosis or misdiagnosis thereof remains a critical challenge and with significant implications. At the patient level, it delays appropriate treatment and disease management. At the health system level, it leads to inefficient population health strategies, distorted epidemiological estimates, and suboptimal resource allocation. Addressing this hidden HF population is therefore essential for improving disease surveillance, optimising care

Incidence, time trends and predictors of acute kidney injury in the region of Stockholm, Sweden.

Acute kidney injury (AKI) is a common and potentially serious complication, yet population-level data on its burden and determinants remain limited. We quantified AKI incidence, temporal trends, and risk factors across a complete healthcare system. We conducted a population-based cohort study including all adult residents of Stockholm, Sweden (2017-2021) using the SCREAM project. AKI events requiring hospitalization or occurring during hospital stays were identified using a composite algorithm incorporating laboratory-based KDIGO creatinine criteria, ICD-10 diagnoses, and acute dialysis proced

The lived experience of mothers who give birth after induction of labour in hospital settings in Southern Ethiopia: a phenomenological study.

BACKGROUND: Induction of labor (IOL) is a medical intervention used to initiate labor artificially and has become one of the most common procedures in modern obstetrics to improve maternal and neonatal outcomes. While extensive research has focused on the methods and outcomes of labor induction, limited evidence exists regarding mothers' lived experiences following IOL, particularly in the Ethiopian context. OBJECTIVE: To explore the lived experience of mothers who give birth after induction of labor at Wolaita Sodo Comprehensive Specialized Hospital, Southern Ethiopia, 2024. METHODS: A qual

Survey of normalized CTDIvol values across four major computed tomography vendors for use in the MIRDct software.

BACKGROUND: Computed tomography (CT) is an essential imaging modality for disease diagnosis, treatment efficacy, and image-based guidance of various medical procedures. The locally deposited radiation dose in tissues, as estimated by the computed tomography dose index (CTDI), can vary considerably across exposures delivered by CT scanners from different vendors, even if the scans are performed using similar technique factors, such as tube potential and tube current. The volumetric CTDI (CTDIvol) is a common dose metric that reports an average radiation dose (in mGy) delivered to a specific vol

Temporal trends in colorectal resections for benign colorectal neoplasia: A retrospective cohort study of the ACS-NSQIP 2016-2022.

BACKGROUND: Colorectal cancer (CRC) screening programmes in North America have effectively reduced CRC incidence. Among the nearly six million colonoscopies performed annually in the United States, 4%-11% detect large benign colorectal neoplasms, many of which can be removed endoscopically, minimizing the need for surgery. Endoscopic removal is less invasive, preserves bowel function and lowers healthcare costs. Despite these benefits, segmental colectomy for benign neoplasia remains common, with surgeries increasing between 2000 and 2014. To address this, efforts have focused on improving acc

Out-of-pocket health care spending among patients with acute limb ischemia stratified by income.

OBJECTIVE: Acute limb ischemia (ALI) results in high health care costs due to the near-universal need for surgical procedures and long hospital stays. Beyond the system costs incurred due to ALI, out-of-pocket (OOP) costs have a significant impact on patients' well-being and compliance with care. The objective of this study was to quantify OOP costs among individuals with ALI compared with the general population. METHODS: This was a cross-sectional study of individuals aged 18 years or older in the Agency for Healthcare Research and Quality Medical Expenditure Panel Survey (MEPS) from 2017 to
