Has electronic fetal heart rate monitoring made a difference.

Electronic fetal heart rate monitoring (EFHRM) has revolutionised our understanding of the function of the cardiovascular system of the fetus during labour, and how the fetus responds to hypoxia. However, although it is a sensitive technique for the detection of hypoxia during labour, it is relatively non-specific for detecting the development of acidosis. Moreover, it is highly dependent on accurate interpretation of fetal heart rate (FHR) patterns, which has been shown to be commonly of a low standard in everyday clinical practice. Use of EFHRM has probably reduced the incidence of birth asp

Fetal heart rate observations in the brain-damaged infant.

The objective of this study was to describe the fetal heart rate patterns and underlying pathophysiologic changes in the brain-damaged fetus. Fetuses with brain damage from hypoxic ischemic encephalopathy do not manifest uniform fetal heart rate patterns. However, these fetuses do show distinct fetal heart rate patterns that permit categorization based on their admission heart rate, subsequent changes in their baseline rate; and neonatal findings. Based on the observations of infants brain damaged in utero because of hypoxic ischemic encephalopathy, the intrapartum fetal management will depend

Perioperative assessment of fetal heart rate and uterine activity.

Improvements in surgical techniques and anesthesia allow women the option to schedule needed surgery during pregnancy. However, perioperative monitoring of the fetus and uterine activity remains a matter of controversy. Monitoring may allow rapid improvement of the fetal status or uterine activity when early compromise or contractions are detected. The reassurance and decreased medicolegal risks provided by perioperative monitoring may offset the cost of a perinatal nurse and use of monitoring equipment even though the drug and anesthetic effect on the fetal heart limit the benefits of monitor

Fetal heart rate patterns associated with amniotic fluid embolism.

Two cases with recorded fetal heart rate response during maternal amniotic fluid embolus are reported. Fetal heart rate abnormalities can be associated with the development of amniotic fluid embolus or occur during the acute events of amniotic fluid embolus. Attention to the management of amniotic fluid embolus in the mother must not neglect the fetus. Improvement of maternal oxygenation and perfusion can effect intrauterine resuscitation; however, when this is not successful early delivery of the fetus may be necessary for its survival.

Simultaneous nonstress fetal heart rate testing in twin pregnancy.

Twenty-four sets of twins in the third trimester of pregnancy underwent 120 simultaneously recorded nonstress tests (NSTs). Synchronous patterns of fetal heart rate alterations occurred in 58% of the cases and were associated with single placentas and small weight differences between twins. Reactive NSTs were frequent (77%) and, when they occurred within 1 week of delivery, conferred a good prognosis, ie, no perinatal deaths and low morbidity. Nonreactive NSTs were associated with both fetal deaths and a perinatal morbidity of 28%. simultaneous NSTs for twins are technically feasible in the th

Routine fetal heart rate monitoring in the antepartum period.

A total of 4517 successful antepartum nonstress tests (NST) was performed on 2003 high- (28%) and low- (72%) risk obstetric patients. Most patients were monitored from 32 to 34 weeks' gestation onward. A reactive NST was defined as 2 accelerations in 10 minutes--15 beats/min minimum amplitude; 15 seconds minimum duration. The test was repeated at the next visit in low-risk patients with reactive NST or in 1 week in high-risk patients with this response. Nonreactive tests were followed immediately by a stress test in high-risk patients, but only after repeat nonreactive NST within 24 hours in l

Sinusoidal fetal heart rate pattern associated with alphaprodine administration.

Sinusoidal fetal heart rate patterns were observed in 42.5% of a group of 40 women who had received alphaprodine (Nisentil) for relief of labor pain. Analysis showed that the pattern appeared approximately 19 minutes following alphaprodine administration and persisted for approximately 60 minutes. In a control group of 18 patients who did not receive narcotic analgesia, and intermittent sinusoidal pattern was observed in one patient. All babies with sinusoidal heart rate during labor had 1-minute Apgar scores of 7 or greater except two in the study group. Both these neonates had low Apgar scor

Abrupt fall in the fetal heart rate during labour.

Cardiotocograms of 6044 patients in labour were examined for evidence of an abrupt fall in the fetal heart 50 beats within 3 minutes, without a demonstration of signs of recovery in the following 2 minutes. Only 9 instances could be found -- an incidence of 0,15%. Umbilical cord entanglement possibly played a role in 6 of these instances, and 2 of the infants concerned were stillborn. Intra-uterine growth retardation was associated with deceleration in 3 infants, 1 of whom was stillborn.

THE EFFECTS OF SPINAL ANESTHESIA ON THE FETAL HEART RATE.

The effect of spinal anesthesia on fetal heart rate is due to maternal hypotension and subsequent fetal hypoxia. Maternal hypotension of 80 mm of mercury for five minutes almost always results in hypoxic fetal bradycardia. This bradycardia is gradual in onset, and may be preceded by a short period of fetal tachycardia. There is a lag in the return of fetal heart rate to normal after maternal blood pressure has normalized. Similar bradycardia has been observed in maternal syncope unassociated with anesthesia. Maternal hypotension should be prevented, and if it occurs should be corrected early.

Intrapartum fetal heart rate monitoring: Rationalise, refine or replace?

Monitoring the fetal heartbeat underpins assessment of fetal wellbeing in labour. Although commonly employed in clinical practice, shortcomings remain. A recent review of clinical practice guidelines highlights the variation in definitions of the fetal heart rate that will lead to differences in interpretation. Will intrapartum care be improved by greater consensus around clinical practice guidelines through rationalisation or refinement of guidelines, or will the future see this technique replaced by more accurate forms of fetal monitoring?

Complexity analysis of fetal heart rate preceding intrauterine demise.

BACKGROUND: Visual non-stress test interpretation lacks the optimal specificity and observer-agreement of an ideal screening tool for intrauterine fetal demise (IUFD) syndrome prevention. Computational methods based on traditional heart rate variability have also been of limited value. Complexity analysis probes properties of the dynamics of physiologic signals that are otherwise not accessible and, therefore, might be useful in this context. OBJECTIVE: To explore the association between fetal heart rate (FHR) complexity analysis and subsequent IUFD. Our specific hypothesis is that the comple

Effect of epidural analgesia on the fetal heart rate.

Fetal heart rate (FHR) changes occasionally follow the use of labor epidurals. Most studies of the phenomenon have focused on specific techniques, but similar timing and appearance suggest a common cause. Induction of analgesia may transiently alter the balance between factors encouraging and inhibiting uterine contractions with subsequent changes of FHR patterns. Regardless the etiology, these changes are transient and do not produce maternal or fetal morbidity. However, they must be promptly recognized and treated if necessary.

[Fetal heart rate patterns in response to acoustic stimulation].

Fetal heart rate response to fetal acoustic stimulation (FAST) during the antepartum period after 25 weeks of gestational age was studied in 157 cases of normal pregnancy, 22 cases of fetal distress, 12 cases of SFD, 10 cases of fetal malformation, 9 cases of placental dysfunction, 4 cases of oligohydramnios, 12 cases of toxemia of pregnancy, and 15 cases of SLE. Five different types of response were observed: Type I through type V. 1. FAST was available to evaluate fetal well-being from 27 weeks of gestational age, although a nonstress test (NST) has been used after 31 weeks of gestational ag
