Total Artificial Heart as a Bridge to Transplantation in Cardiogenic Shock Due to Ruptured Cardiac Pseudoaneurysm.

BACKGROUND: Cardiogenic shock (CS) due to contained cardiac rupture is a life-threatening condition that severely limits access to advanced heart failure therapies. CASE SUMMARY: A 53-year-old man with dilated cardiomyopathy and recent mechanical aortic valve replacement for infective endocarditis presented in CS. Imaging identified a large apical pseudoaneurysm, consistent with contained apical rupture. The multidisciplinary shock team opted for implantation of a new-generation total artificial heart as a bridge to transplantation. Postoperative recovery included a period of transient renal

Successful Treatment of Cardiac Tamponade due to Rupture of the Heart Performing an Open-chest Pericardiotomy.

A 78-year-old woman with mild dementia was found unconscious by her family. She was transported by an ambulance to our emergency room (ER). Initially, she was comatose and in a state of shock. The echocardiographic findings suggested cardiac tamponade by hematoma. Computed tomography also showed tamponade without aortic dissection. After imaging, she went into cardiac arrest, was returned to the ER, and tracheal intubation and left thoracotomy for pericardiotomy were performed. A return of spontaneous circulation was obtained by following this procedure. Bleeding from a rupture of the left car

Delayed presentation of pericardial rupture with luxation of the heart following blunt trauma: a case report.

OBJECTIVE: To present an unusual case of delayed presentation of pericardial rupture with luxation of the heart 6 months after the initial injury. DESIGN: Diagnosis, outcome, associated injuries, and treatment are discussed. METHODS: A high index of suspicion is required to make the diagnosis; no single diagnostic test can be used as a screening tool. CONCLUSION: Once recognized, treatment of pericardial rupture is relatively simple.

Myocardial rupture: III. Chamber pressure, rate of distention, and ventricular disruption in isolated hearts.

We have developed an in vitro technique for producing myocardial rupture in lamb hearts, which relates tensile strength to a variety of conditions which can prevail in normal and infarcted human hearts. Retrograde perfusion of saline solution and inflation of the left ventricle was used to apply progressive stress to the left ventricular wall. Three separate sites of myocardial rupture were observed and occurred with the frequency of 54% at the papillary muscle, 30% at the interventricular septum, and 16% at the free wall of the left ventricle. The distribution and configuration of the experim

New insights into mitral heart valve prolapse after chordae rupture through fluid-structure interaction computational modeling.

Mitral valve (MV) dynamics depends on a force balance across the mitral leaflets, the chordae tendineae, the mitral annulus, the papillary muscles and the adjacent ventricular wall. Chordae rupture disrupts the link between the MV and the left ventricle (LV), causing mitral regurgitation (MR), the most common valvular disease. In this study, a fluid-structure interaction (FSI) modeling framework is implemented to investigate the impact of chordae rupture on the left heart (LH) dynamics and severity of MR. A control and seven chordae rupture LH models were developed to simulate a pathological p

Case report: Heart transplant for persistent right heart failure after complete surgical repair and percutaneous closure of post-myocardial infarction ventricular septal rupture.

The incidence of post-acute myocardial infarction ventricular septal rupture (post-AMI VSR) has decreased; however, mortality after surgical repair of post-AMI VSR remains high. Patients who are not surgical candidates can be managed by heart transplant with a good outcome. A heart transplant in a patient after successful repair of VSR has never been reported. We report a patient who had persistent right heart failure after the successful repair of VSR and underwent a heart transplant with a good outcome.

Ruptured Sinus of Valsalva Aneurysm: Silent and Rare Cause of Heart Failure in Young Adults.

BACKGROUND: Sinus of Valsalva aneurysm is a rare condition that can cause acute heart failure once it develops a fistula. CASE SUMMARY: A previously healthy 28-year-old man without history of intravenous drug use initially presents with an acute onset of heart failure symptoms. Transthoracic echocardiogram transesophageal echocardiogram revealed an aorto-atrial fistula causing significant left-to-right shunting. Blood cultures were negative for any infectious process. The patient initially required critical care with inotropic support and aggressive diuresis before going to the operating room

Vitamin A deficiency attenuates cardiac rupture in Stra6-deficient hearts following ischemic injury.

BACKGROUND: Stimulated by retinoic acid gene 6 (STRA6) is a cell surface receptor that regulates cellular uptake of vitamin A metabolites and cardiac development. We hypothesized that Stra6 expression attenuates ischemic injury-induced heart failure following myocardial infarction (MI) by vitamin A-dependent mechanisms. METHODS: MI was induced in mice with Stra6 germline deletion, vitamin A deficiency (VitAD) by combined lecithin-retinol acyltransferase (Lrat) germline deletion and feeding with a vitamin A-deficient diet. Contractile function was determined by transthoracic echocardiography,

Unusual presentation of ruptured non coronary sinus of valsalva with new onset heart failure.

Rupture of sinus of Valsalva is a rare cardiac condition which is associated with severe left to right shunting. Symptoms may include breathlessness, chest pain and fatigue or even cardiogenic shock and when untreated, this condition carries a grave prognosis. We report a case of a 57-year-old gentleman without any past comorbidities who presented to our hospital with features of acute heart failure due to rupture of sinus of Valsalva which was diagnosed by echocardiogram and further confirmed by cardiac computed tomography scan. Patient underwent successful patch closure of the defect and mad

Aortic dissection and ruptures in adult congenital heart disease in Texas from 2009 to 2019.

OBJECTIVES: Acute thoracic aortic dissection and rupture (TADR) has an incidence of 5-7 per 100 000-person years. Today, most children with congenital heart disease (CHD) survive to become adults with congenital heart disease (ACHD). This study evaluates TADR in patients with ACHD in a large, hospitalized patient population over 11 years to evaluate the incidence, risk factors and outcomes associated with TADR. METHODS: This was a retrospective review of the Texas Inpatient Discharge Data Set from 1 January 2009 to 31 December 2019. All non-trauma discharges of patients ≥18 years were include

Acute congestive heart failure due to ruptured mitral chordae tendineae in late pregnancy.

A 31-year-old woman complained of dyspnea and orthopnea at 38 weeks of gestation. A grade 3/6 pansystolic murmur was heard, and echocardiography revealed severe mitral regurgitation with a hyperechoic obstacle on the posterior mitral valve leaflet, consistent with a diagnosis of acute heart failure due to a ruptured chordae tendineae or an infectious endocarditis. An emergency cesarean section was performed under general anesthesia. A male infant was born weighing 2928 g with Apgar scores of 7 and 8 at 1 and 5 min, respectively. The patient was managed in the intensive care unit and underwent

Ruptured pseudoaneurysm of aortic root graft into the pulmonary artery as a cause of severe heart failure.

A 52-year-old man presented with heart failure of 1 month duration. He had undergone aortic valve and root replacement 30 months before admission. A continuous murmur was heard in the second intercostal space at the parasternal border. Aortography showed a pseudoaneurysm surrounding the aorta, whereas color Doppler study revealed flow from the central aorta to the pseudoaneurysm and flow from the pseudoaneurysm to the pulmonary artery trunk through a fistulous communication between them. Thus, fistulous communication with pulmonary artery causing heart failure is a complication of pseudoaneury

Combined blunt traumatic rupture of the heart and aorta: two case reports and review of the literature.

Blunt chest trauma resulting in combined aortic disruption and cardiac rupture, although a common autopsy finding, was found reported only once previously in a surviving patient. We report two cases repaired through a left posterolateral thoracotomy in which the cardiac injury was unsuspected and presented as an intraoperative finding of hemopericardium. With improved emergency resuscitation in the field and faster transport of these cases to tertiary care centers, this combination of lesions may be seen more frequently. Suggestions for their diagnosis and management are presented.

Bespoke valve sizing avoids annular rupture in patients treated with a balloon-expandable transcatheter heart valve.

BACKGROUND: Annular and left ventricular outflow tract (LVOT) calcification increase the risk of annular rupture following transcatheter aortic valve replacement (TAVR). The outcomes of a strategy of routine use of a balloon-expandable valve (BEV) for all patients irrespective of annular or LVOT calcium is unknown. OBJECTIVES: We evaluated the impact of bespoke sizing on annular rupture in patients treated with a BEV. METHODS: All consecutive patients undergoing TAVR at a single centre (February 2020-February 2022) were treated only with a BEV. No other valve design was used. Annular/LVOT ca

Ventricular Fibrillation Arrest and Pulmonary Homograft Rupture During Transcatheter Pulmonary Valve: Successful Rescue by Heart Team.

We describe a 31-year-old woman with pulmonary homograft rupture and ventricular fibrillation arrest complicating a transcatheter pulmonary valve (TPV) procedure. She underwent extracorporeal membrane oxygenation (ECMO) with immediate surgical repair including bioprosthetic pulmonary valve replacement. She had difficulty weaning off ECMO due to hyperacute failure of the valve and ultimately underwent successful hybrid TPV with complete recovery. This case illustrates the importance of the heart team approach during catheter and surgical interventions in adult congenital heart disease.

[Postoperative rupture of abdominal aortic aneurysm in patients with ischemic heart disease and other organ disease].

We experienced two cases of rupture of an abdominal aortic aneurysm during the early postoperative period of coronary artery bypass grafting (CABG). A 71-year-old man on hemodialysis (HD) was diagnosed with ischemic heart disease (IHD) and abdominal aortic aneurysm (AAA) of 70 mm in size. After CABG, he developed symptoms of acute pancreatitis and died of rupture of AAA on the 12th postoperative day. A 74-year-old man with early gastric cancer was diagnosed with IHD and AAA of 70 mm. After CABG and gastrectomy, he died of rupture of AAA due to anticoagulant therapy on the 3rd postoperative day

Heart Transplantation in post-infarction ventricular septal rupture: Contemporary outcomes from the 2016-2021 National Inpatient Database.

INTRODUCTION: Ventricular septal rupture (VSR) is a devastating complication of myocardial infarction (MI), with high mortality, particularly in cardiogenic shock (CS). Heart transplantation (HT) has emerged as a potential alternative to surgery or transcatheter closure (TCC). This study evaluates contemporary trends and outcomes of HT in post-MI VSR using the National Inpatient Sample (NIS) database. OBJECTIVES: To assess in-hospital mortality and resource utilization of HT compared to surgical repair or TCC for post-MI VSR with CS. METHODS: We analyzed NIS data (2016-2021) for MI-VSR hospi
