Multimodal Management of Complex Congenital Heart Disease: A Case of Atrial and Ventricular Septal Defects With a Bifid Apex.

Congenital heart disease (CHD) is one of the most common congenital anomalies influenced by both genetic and environmental factors. Atrial septal defect (ASD) and ventricular septal defect (VSD) are among the most common types of CHD. ASD is more common in females and often remains asymptomatic during childhood, leading to delayed diagnosis. Both surgical and percutaneous closure techniques are used, with evidence supporting early intervention for improved outcomes. This study presents the case of a female patient with CHD, including ASD, VSD, and a bifid left ventricular apex, who was treated

Management of Late-Presenting Congenital Combined Heart Defect - Bicuspid Aortic Valve and Ventricular Septal Aneurysm.

BACKGROUND Bicuspid aortic valve is the most common congenital heart malformation, encountered in 1-2% of the population, while interventricular septal defect and patent ductus arteriosus are the most common congenital malformations associated with bicuspid aortic valve. Although bicuspid valve can have no clinical manifestation, patients with bicuspid valve are prone to develop vascular abnormalities. Aortic dilatation is the most common of these abnormalities, which in turn can lead to serious complications and often requires surgical treatment. Coexistence of bicuspid aortic valve and inter

Pulmonary artery banding in a cat with a perimembranous ventricular septal defect and left-sided congestive heart failure.

CASE DESCRIPTION A 6-month-old sexually intact male domestic shorthair cat was referred for evaluation of a heart murmur and ventricular septal defect (VSD). CLINICAL FINDINGS Physical examination revealed a grade 5/6 right apical systolic heart murmur. Findings on thoracic radiography were consistent with moderate right and left ventricular enlargement, left atrial enlargement, and enlargement of the pulmonary arteries and veins; an interstitial pulmonary pattern was also evident. Echocardiography revealed a perimembranous VSD with left-to-right shunting combined with trace mitral valve regur

Is the short axis view of the fetal heart useful in improving the diagnostic accuracy of outlet ventricular septal defects?

OBJECTIVES: Outlet ventricular septal defects (VSDs) are usually suspected on the five-chamber view of the fetal heart; however, postnatal confirmation occurs only in a small number of cases. The aim of this study was to evaluate if the systematic assessment of the short axis view may improve the prediction of prenatally detected outlet VSDs. METHODS: Cases where isolated outlet VSD was suspected on the five-chamber view were assessed by using the short axis scanning plane for confirmation of the defect. Postnatal assessment was performed within 2 weeks from birth at our Paediatric Cardiology

Importance of plasma endothelin-1 level in the evaluation of heart failure severity in infants with ventricular septal defect.

BACKGROUND: Haemodynamic disturbances due to ventricular septal defect (VSD) can lead to heart failure (HF) and cause neurohormonal activation. Endothelin-1 (ET-1) clearance takes place mainly in the pulmonary circulation. We hypothesized that increased pulmonary blood flow in children with VSD could influence ET-1 level and reflect haemodynamic disturbances in these patients. AIM: To analyse usefulness of plasma ET-1 level in the evaluation of HF severity in infants with VSD without pulmonary hypertension. METHODS: The study group included 34 children (aged 38-338 days, mean 130 ± 81 days)

Perventricular closure on the beating heart: an effective hybrid approach in managing a postinfarction apical ventricular septal defect.

Postinfarction ventricular septal defect (VSD) is a life-threatening complication of acute myocardial infarction. Surgical repair of this type of VSD is associated with a high mortality rate. We present a technique of using perventricular closure for a postinfarction apical VSD without cardiopulmonary bypass.

Identification of differentially expressed genes in human heart with ventricular septal defect using suppression subtractive hybridization.

Ventricular septal defect (VSD) accounts for the largest number of birth congenital heart defects in human, but the genetic programs that control ventricular septation are poorly understood. To identify differentially expressed genes between ventricular septal defect and normal ventricular septum myocardium, we have undertaken suppression subtractive hybridization (SSH) and generated reciprocal cDNA collections of representative mRNAs specific to human heart with ventricular septal defect versus normal control. Following SSH, 1378 clones were sequenced and found to derive from 551 different ge

Important ECG diagnosis-aiding indices of ventricular septal defect children with or without congestive heart failure.

In this paper we perform a statistical study of the conventional RR intervals and two newly defined PR' and RT intervals of ECG data. A quadratic classification rule is applied to extract several important ECG diagnosis-aiding indices among normal children and children with ventricular septal defect (VSD) with or without congestive heart failure (CHF). The results show that certain statistics computed from PR', RR and RT intervals are important diagnosis-aiding indices. Best classification vectors are searched for pairwise classification. Two methods, minimum distance criterion and a two-stage

Reduction of carotid arterial blood flow in ventricular septal defect associated with severe congestive heart failure.

After diagnosing abnormality of cardiac and carotid-cerebral circulation in an infant with isolated ventricular septal defect (VSD) associated with severe congestive heart failure, the authors measured the carotid arterial blood flow volume (CABF). At 3 months, the patient was not thriving and had dyspnea because of severe congestive heart failure. The authors measured the VSD size/body surface area (BSA) ratio relative to the predicted value of the left ventricular end-diastolic dimension (%LVEDd), left-to-right shunt ratio (Qp/Qs), and the small stroke volume (SV)/BSA using echocardiography

Criss-cross heart--a case with horizontal septum, complete transposition, pulmonary atresia and ventricular septal defect.

Criss-cross heart is a recently described anomaly in which the systemic and pulmonary blood streams cross at the atrioventricular (AV) level, without mixing. A case of criss-cross heart is described in which the right atrium, in a solitus position, communicated with a left-superior positioned, morphologically right ventricle, and the left atrium communicated with a normally located, morphologically left ventricle. The interventricular septum occupied a horizontal plane. Associated defects were complete d-transposition of the great arteries with l-positioned aorta, pulmonary atresia, ventricula

Surgical technique to reduce the risks of heart block following closure of ventricular septal defect in atrioventricular discordance.

Traumatic heart block remains a major concern after ventricular septal defect (VSD) closure in patients with atrioventricular discordance. A technique of closing the VSD, placing the suture line on the morphologically right side of the septum without opening the systemic ventricle, is described. This was used in 13 consecutive patients. The VSD was closed through the right atrium in eight patients, the left ventricle in three patients, the right atrium and the left ventricle in one patient, and through the right ventricle in one patient with atrioventricular disordance and ventriculo-arterial

Unrepaired Congenital Heart Disease in Childhood: A Case of Ventricular Septal Defect and Double-Chambered Right Ventricle.

Congenital heart diseases are structural abnormalities of the heart with functional repercussions on the patient's hemodynamics. At the time of diagnosis, most patients are in childhood, and the clinical course exhibits a wide spectrum of severity, ranging from mild conditions that do not affect the quality of life to severe presentations that may lead to death. Under this concept, we present the case of a 45-year-old patient who was diagnosed in childhood with a ventricular septal defect for which he did not receive any treatment or intervention. Despite this, he remained asymptomatic until h

Cardiac Rehabilitation in a Patient With Severe Heart Failure and Ventricular Septal Defect Secondary to Acute Myocardial Infarction.

The treatment of choice for patients with advanced heart failure (HF) and with limiting symptoms with evidence of a poor prognosis despite optimal conventional treatment is a heart transplant. However, there is little literature dealing with the effects of cardiovascular prehabilitation with an important change in physical capacity, which can influence the admission on the waiting list for a heart transplant. We presented one young male, smoker, with no prior history of cardiovascular disease, severe ventricular dysfunction, interventricular defect, and HF. It was decided to implant a cardiove

Perventricular device closure of muscular ventricular septal defects on beating hearts: initial experience in eight children.

OBJECTIVE: The conventional surgical repair and transcatheter closure of muscular ventricular septal defects are known to have undesirable limitations. This communication describes the short-term results of perventricular device closure of muscular ventricular septal defects with the heart beating in 8 children with 15 muscular ventricular septal defects, with or without other congenital malformations. METHODS: A subxiphoid minimally invasive incision was used in 3 children with isolated muscular ventricular septal defects whereas standard full median sternotomies were used in the other 5 chi

Congestive heart failure during early infancy in patients with ventricular septal defect relative to early closure.

The hemodynamic features of 44 patients requiring surgical closure of a ventricular septal defect (VSD) during early infancy were evaluated. The VSD was closed within the first 6 months of life in 29 patients (group A) and during the second 6 months in 15 patients (group B). The left-to-right (L-R) shunts were significantly greater in group A than in group B, although in all patients they were more than 50%. In contrast, the left ventricular end-diastolic volume was significantly greater in group B than in group A, whereas the right ventricular end-diastolic volume was greater in group A; the
