Radiography of trigeminal neuralgia and hemifacial spasm.

Radiographic findings in 68 patients with trigeminal neuralgia and 24 patients with hemifacial spasm are reviewed. The relative value of various radiographic diagnostic procedures is discussed. Trigeminal neuralgia and hemifacial spasm are usually caused by vascular compression of the trigeminal root entry zone and facial nerve exit zone respectively. Computed tomography (CT) of the posterior fossa is the only radiographic screening procedure required. Angiography should be reserved for patients in whom CT findings suggest an aneurysm or tumor.

Botulinum toxin-A for the treatment of hemifacial spasm.

Management of hemifacial spasm can actually be done medically, surgically and with Botulinum-A Toxin. The Botulinum-A Toxin treatment locally injected into the involved facial muscles offers a useful alternative to medical and surgical therapy. The objective of this study was to evaluate the efficacy of Botulinum-A Toxin for the treatment of hemifacial spasm in those subjects for whom the presently available medical therapy is inadequate. A total of 28 individuals were enrolled in the clinical study. Patients were evaluated using the Fahn's blepharospasm rating and disability scales. Efficacy

Delayed relief of hemifacial spasm after microvascular decompression.

Microvascular decompression (MVD) has been accepted worldwide as a reasonable treatment for hemifacial spasm (HFS); however, resolution of the HFS is often gradual. To conclude the delayed relief rate of the MVD for the treatment of HFS, we conducted a systematic review. Using the keywords delayed relief, hemifacial spasm, or microvascular decompression, articles published in English-language journals and indexed in PubMed between June 1, 1994 and June 1, 2014 on the treatment of HFS with emphasis on delayed relief were considered for this study. Twelve articles with 2727 patients with HFS wer

Management of Bilateral Hemifacial Spasm with Microvascular Decompression.

BACKGROUND: Bilateral hemifacial spasm (HFS) is very rare. The literature contains only 32 clinical reports. Although microvascular decompression (MVD) is widely accepted as effective therapy for HFS, the etiology and surgical treatment of bilateral HFS are seldom addressed. We report our experience with MVD for patients with bilateral HFS. METHODS: This retrospective report included 10 patients with bilateral HFS. All patients underwent MVD 1 or 2 times and were followed for 5-92 months. The clinical data were retrospectively analyzed. The etiology and treatment strategies were discussed. R

Hearing outcomes following microvascular decompression for hemifacial spasm.

PURPOSE: Facial nerve microvascular decompression (MVD) for hemifacial spasm (HFS) provides relief to most patients. Due to the proximity of the cochlear and facial nerves, hearing loss is a potential MVD complication, however, there is a wide range in the reported incidence of hearing loss (HL) in the literature. In order to better understand the HL incidence in our MVD population, we utilized the combination of speech discrimination scores (SDS) and air and bone pure tone threshold averages (PTA) to identify patients with no hearing change, sensorineural hearing loss, or conductive hearing l

Hemifacial spasm, vertebrobasilar dolichoectasia and neurofibromatosis type 1.

Hemifacial spasm (HFS) is usually produced by compression of the facial nerve by tortuous blood vessels at the root exit zone, including vertebrobasilar dolichoectasia (VBD). Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder with a variety of symptoms, affecting mainly the skin and nervous system. Cerebrovascular abnormalities are becoming a recognized complication of the disease and the most constantly described lesions are stenosis and occlusions affecting the internal carotid artery. VBD has rarely been associated with NF1. We report a 38-year-old female patient with HFS prod

Hemifacial spasm caused by cross type vascular compression.

OBJECTIVES: The meatal segment of anterior inferior cerebellar artery usually crosses over the gap between cranial nerves VII and VIII, and may compress the cisternal portion (CP) of the facial nerve. This is defined as cross type compression, which is easy to be neglected and thus leads to poor outcome. Here our experience in treating patients of cross type hemifacial spasm (HFS) is reported. METHODS: Twenty-one patients of HFS due to cross type compression were treated with microvascular decompression (MVD) surgery with the aid of abnormal muscle response monitoring. RESULTS: In addition t

Coexistent blepharospasm and hemifacial spasm: overlapping pathophysiologic mechanism?

BACKGROUND/AIM: Blepharospasm (BEB) and hemifacial spasm (HFS) appear to be distinct disorders. Clinical characteristics of coexistent BEB and HFS have not been examined. The aim of this study was to determine the prevalence, clinical, and imaging features of coexistent BEB among a cohort of HFS patients and controls. RESULTS: Among 665 study subjects, nine (5.5%) of the 164 consecutive HFS patients had coexistent BEB, significantly higher than age and gender matched controls (0/501, 0%) without neurological diseases (p<0.0001). The mean age of the nine patients was 61.4 (SD 9.9) (range 51-72

Pediatric hemifacial spasm: the efficacy of microvascular decompression.

Hemifacial spasm (HFS), generally a disease of the elderly, is caused by vascular compression of the seventh nerve. Vascular compression is thought to result from atherosclerotic changes within the vessels of the posterior fossa, and therefore rarely presents in childhood. Here we describe our experience with 12 patients with onset of HFS during childhood (age 18 or less) and who had surgical exploration of the cerebellopontine angle. These patients represent less than 1.2% of the patient population with HFS operated upon at this institution during the study period. Nine patients had follow-up

Magnetic resonance imaging and angiography in hemifacial spasm.

We used magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) to investigate 14 patients with unilateral hemifacial spasm (HS) and 20 controls. The relationship of the seventh and eighth cranial nerves to adjacent vessels was best visualized on the contiguous flow sensitive 3D-FISP images. Reconstruction of projectional MRA was helpful to assess the complex architecture of the vertebrobasilar system. Neurovascular contact in the facial nerve root exit zone was present in 4 of 20 controls and in 12 of 14 patients, in whom it predicted the affected side. These results support

Treatment of idiopathic hemifacial spasm with botulinum toxin.

Twelve patients with idiopathic hemifacial spasm received treatment with botulinum toxin A over a period of 18 months. Of 76 treatments given, most (94.7%) led to successful relief of eyelid spasms and all treatments were successful for perioral and lower facial muscle spasms. An average dose of 9.3 units of toxin per session was given to produce a mean interval of relief of 10.8 weeks. Blepharoptosis was the only ocular side effect; it was mild, reversible and occurred in 2 patients. However, lower facial palsy was frequent (9 patients); it was mild to moderate in severity but only partially

Transtympanic surgery for hemifacial spasm--an electrophysiological study.

Producing deliberate damage to the facial nerve in the middle ear is an established method of treating idiopathic hemifacial spasm. However, no previous electrophysiological studies have investigated the effect of this operation on facial nerve function. Eleven patients undergoing transtympanic facial nerve needling were investigated with electromyography. Following operation reduction in the abnormal discharges and synkinesis typical of hemifacial spasm was seen in the majority of patients. The success of this type of operation, although temporary in some patients, together with other neuroph

Hemifacial spasm: treatment by microsurgical facial nerve decompression.

The literature on hemifacial spasm and its surgical therapy is reviewed, and the authors' experiences with 20 patients are described. Vascular cross compression of the facial nerve adjacent to the brain stem is seen in 90% of the surgical patients. Mobilizing the offending vessel will cure or significantly improve approximately 80% of the patients. Complications occur in 25% of the patients and usually involve decreased hearing or facial weakness. Mortality is virtually zero, and this operation is vastly superior to any other medical or surgical therapy for hemifacial spasm. The pathophysiolog

The many faces of hemifacial spasm: differential diagnosis of unilateral facial spasms.

Hemifacial spasm is defined as unilateral, involuntary, irregular clonic or tonic movement of muscles innervated by the seventh cranial nerve. Most frequently attributed to vascular loop compression at the root exit zone of the facial nerve, there are many other etiologies of unilateral facial movements that must be considered in the differential diagnosis of hemifacial spasm. The primary purpose of this review is to draw attention to the marked heterogeneity of unilateral facial spasms and to focus on clinical characteristics of mimickers of hemifacial spasm and on atypical presentations of n

Staged Bilateral Microvascular Decompression for Bilateral Hemifacial Spasm.

Bilateral hemifacial spasm (BHFS) is rare and typically arises from neurovascular compression of the facial nerve. Optimal treatment strategies, including the timing and laterality of microvascular decompression (MVD), remain uncertain. We report a case of a 43-year-old man with a 10-year history of left hemifacial spasm, and right-sided symptoms beginning 1 year prior to initial evaluation. Magnetic resonance imaging demonstrated bilateral neurovascular compression at the root exit zone. The patient underwent staged bilateral MVD-left MVD first, followed by right MVD after 6 months-resulting

[Surgical treatment of hemifacial spasm: a systematic review].

UNLABELLED: The annual incidence of hemifacial spasm is 8-15 cases per 100.000. This disease is not life-threatening, but it significantly impacts the quality of life. Primary treatment is vascular decompression of the facial nerve. Thus, improvement of appropriate technique is crucial for higher efficacy and safety. OBJECTIVE: To determine the optimal algorithm for vascular decompression according to available international experience in surgical treatment of hemifacial spasm. MATERIAL AND METHODS: A systematic review was conducted in accordance with the PRISMA guidelines. To identify relev

Large posterior fossa meningioma presenting with hemifacial spasm.

BACKGROUND: Hemifacial spasms are involuntary paroxysmal muscle contractions commonly presenting as unilateral involvement of the orbicularis oculi and oris muscles. CASE DESCRIPTION: A 62-year-old woman presented with spasms of progressively increasing frequency for 2 months in the right orbicularis oculi muscle, with subsequent involvement of the orbicularis oris muscle. Cerebral magnetic resonance imaging revealed an intensely enhanced dural-based tumor in the right posterior fossa. On constructive interference in steady-state (CISS) imaging, the right cerebellopontine angle cistern showed
