Hepatitis E virus as a cause of acute hepatitis acquired in Switzerland.

BACKGROUND: Autochthonous hepatitis E is increasingly recognized as zoonotic infection in western countries. Serological assays have varying sensitivity and specificity. METHODS: We implemented molecular testing to identify and characterize acute hepatitis E acquired in Switzerland. RESULTS: Ninety-three cases of mostly symptomatic acute hepatitis E acquired in Switzerland were documented by PCR between November 2011 and December 2016. Median HEV RNA was 7.5 x 104 IU/mL (range, 5.3 to 4.7 x 107 IU/mL). HEV genotyping was successful in 78 patients, revealing genotype 3 in 75 and genotype 4 in

Hepatitis A seroprevalence in patients with chronic viral hepatitis in Konya, Turkey.

AIM: Hepatitis A is among the diseases that can be prevented with vaccination in our time. Acute hepatitis A progresses more severely in individuals with a liver disease. Therefore, patients with a chronic liver disease (because of hepatitis B or hepatitis C) are advised vaccination with the hepatitis A vaccine. This study is aimed to determine the seroprevalence of hepatitis A virus (HAV) antibodies in patients infected with hepatitis C virus or hepatitis B virus in Konya province of Turkey. METHODS: A total of 537 patients who had chronic viral hepatitis between January 2011 and December 20

Felty's syndrome with chronic hepatitis and compatible autoimmune hepatitis: a case presentation.

A 68-year-old woman with Felty's syndrome was admitted to our hospital due to breathlessness. She was diagnosed as having rheumatoid arthritis at age 59 years. Abdominal computed tomography indicated ascites, splenomegaly and liver atrophy. She had no antigens or antibodies for hepatitis virus, or antibodies for mitochondria with the exception of antinuclear antibody. According to the International Autoimmune Hepatitis (AIH) scoring system, she was diagnosed as having chronic hepatitis, compatible with AIH. The association of Felty's syndrome with AIH is very rare and the most difficult proble

Hepatitis from 5-methoxypsoralen occurring in a patient with previous flucloxacillin hepatitis.

A 55-year-old woman with psoriasis vulgaris was treated with oral 5-methoxypsoralen and UVA photochemotherapy. After 40 treatments over 3 months she became unwell with hepatitis attributable to the psoralen. Six years earlier she developed cholestatic hepatitis to flucloxacillin. A previous history of drug-induced reactions should be sought before prescribing further drugs with similar adverse effects.

[Cholestatic hepatitis caused by glybenclamide in a patient with hepatitis C virus].

We report the case of a 68-year-old male HVC+ diagnosed as cholestatic hepatitis induced by glybenclamide, following a non-insulin dependent diabetes mellitus treatment. When the drug was discontinued normal analytic parameters were obtained. However, HVC may induce more cholestatic and cytolysic changes on the hepatitis due to glybenclamide. When reviewing the literature only one similar case has been previously reported, although several disturbances in liver function tests have been described in other reports. Routine biochemical tests including liver function are indicated in patients rece

Congenital syphylitic hepatitis: a case report with multiple imaging modalities (syphilitic hepatitis).

A case of syphilitic hepatitis is described with no evidence of mass effect on the ultrasonic and computerized tomographic study, but discrete areas of decreased uptake on liver scan suggestive of space-occupying lesion. This is the second instance in the literature of the incongruence of the liver scan and the other imaging modalities in syphilitic hepatitis.

Acute Viral Hepatitis Due to Co-infection With Hepatitis A and Hepatitis B in an Intravenous Drug User.

Injection drug users are at high risk of contracting human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV) due to parenteral exposure. Hepatitis A virus (HAV) is classically thought to be transmitted through the fecal-oral route, but injection drug use is increasingly recognized as a risk factor. It is well documented that there is a high prevalence of total antibodies to HAV in injection drug users, although there is limited data about the prevalence of acute HAV in injection drug users. Acute viral hepatitis is most often due to HAV, HBV, or hepatitis E vir

Recurrent hepatitis B, hepatitis C, and combined hepatitis B and C in liver allografts: a comparative pathological study.

Although recurrence of viral hepatitis in liver transplants is common, data comparing recurrent hepatitis B (HB), hepatitis C (HC), and co-existing dual hepatitis B and C (HB&C) are sparse. Posttransplantation liver biopsies, along with molecular, serological, immunohistochemical, and clinical data from 27 patients with pretransplantation diagnosis of chronic viral hepatitis, were reviewed. The patients were placed into 4 groups: Group I, with pretransplantation HB (n = 8); group II, with pretransplantation HC (n = 10); group III with pretransplantation HC and anti-HB surface or core antibody

Athlete's Hepatitis: A Case of a Young Healthy Male Developing Hypoxic Hepatitis After a Half-Marathon.

Athlete's hepatitis is a rare form of ischemic hepatitis caused by hypoxic liver injury during extreme physical exertion. We present the case of a 25-year-old healthy male who developed severe transaminitis after completing a half-marathon. He presented with syncope, hypotension, and hyperthermia, followed by markedly elevated liver enzymes. An extensive evaluation ruled out viral, toxic, and structural causes of liver injury. Conservative management led to full recovery, with liver enzymes normalizing within one week. This case highlights the liver's susceptibility to ischemic injury during i

Transfusion transmission of hepatitis A virus with fecal shedding in a previously hepatitis A vaccinated recipient.

We describe a rare case of hepatitis A virus (HAV) replication in feces despite presence of hepatitis A antibodies in an acute myeloid leukemia (AML) patient after transfusion with HAV contaminated platelets. The patient has been vaccinated against HAV years before the AML diagnosis. Transient infection and reshedding should thus be considered in antibody-positive hematological patients. Transfusion associated HAV transmission is rare, and little evidence exists on the clinical consequences and possible effect of treatment with immunoglobulin. Further reporting on fecal shedding despite antibo

A combined dual-chamber typhoid/hepatitis A vaccine as a booster dose in hepatitis A primed adults.

Vivaxim is a combined hepatitis A/typhoid fever vaccine (HA/Vi) licensed for vaccination of travellers, but long-term protection against hepatitis A requires two immunisations at least 6 months apart. A randomised, controlled study was performed in 116 healthy adults primed with hepatitis A vaccine (Avaxim) to compare immune responses to HA/Vi and Avaxim given as booster doses 6 months later. Both vaccines elicited marked booster responses achieving antibody geometric mean titres (GMTs) of 4576 and 3760 mIU/ml in the HA/Vi and Avaxim groups, respectively. Although twice as frequent in the HA/V

[Hepatitis A-E: the essentials].

Viral hepatitis is associated with high morbidity and mortality worldwide. Hepatitis A and E viruses are enterally transmitted and typically cause acute self-limited hepatitis. Hepatitis B, C, and D viruses are parenterally transmitted and can cause chronic hepatitis, with potential progression to cirrhosis and hepatocellular carcinoma. This article provides essential information on the virology, epidemiology, clinical manifestations, diagnosis, treatment, and prevention of hepatitis A-E.

Hepatitis E infection: A review.

Hepatitis E virus (HEV) is a small non-enveloped virus that is transmitted via the fecal-oral route. It is a highly common cause of acute hepatitis, particularly in low to middle income regions of Asia, Africa, and Central America. Most cases are self-limited, and symptomatic patients usually present with acute icteric hepatitis. A subset of patients including pregnant women, older men, those with pre-existing liver disease and immunocompromised patients however, may develop severe disease and hepatic failure. Immunocompromised patients are also at risk for chronic infection, and their immunos

Autoimmune hepatitis: a comprehensive review.

Autoimmune hepatitis (AIH) is an immune-mediated liver disorder characterised by female preponderance, elevated transaminase and immunoglobulin G levels, seropositivity for autoantibodies and interface hepatitis. Presentation is highly variable, therefore AIH should be considered during the diagnostic workup of any increase in liver enzyme levels. Overlap/variant forms of the disease, presenting with concomitant features of primary biliary cirrhosis or primary sclerosing cholangitis are increasingly recognised. AIH is exquisitely responsive to immunosuppressive treatment, which should be insti

Alcoholic hepatitis: a clinician's guide.

Alcoholic hepatitis is a frequent reason for admission and a common consultation request for hepatologists and gastroenterologists. Although it seems to occur acutely, it is usually subacute and often superimposed on underlying alcoholic cirrhosis. Typically patients have a background of drinking on a daily basis, but, in response to a life crisis, patients have started drinking massively.
