Impact of Large Ventral Hernia Repair on Postoperative Renal Function.

BACKGROUND: Large hernia repairs, particularly those involving abdominal wall reconstruction (AWR), can elevate intra-abdominal pressure due to the reintegration of herniated contents into the abdominal cavity, potentially contributing to renal injury. Despite the theoretical risk, the long-term impact of AWR-induced intra-abdominal pressure elevation on renal function remains insufficiently studied. This study aims to evaluate the incidence of acute kidney injury (AKI) and chronic kidney disease (CKD) after AWR and to identify key factors associated with postoperative renal impairment. STUDY

Venous thromboembolism after ventral hernia repair with transversus abdominis release.

INTRODUCTION: Venous thromboembolism (VTE) events, such as pulmonary embolism (PE) and deep venous thrombosis (DVT), are a significant source of morbidity and mortality after major abdominal wall reconstruction. We aim to describe the incidence of VTE events in patients undergoing ventral hernia repair (VHR) with transversus abdominis release (TAR) at our institution. METHODS: The Abdominal Core Health Quality Collaborative registry was queried for patients, 18 years and older, who underwent VHR with TAR at our institution between August 2014 and December 2023. Patient characteristics and ope

Rare presentation of incarcerated ventral hernia containing transverse colon diverticulitis.

Ventral hernias are common abdominal wall defects; however, the presence of complicated diverticulitis within a hernia sac is exceedingly rare. We report the case of a 76-year-old female with multiple comorbidities who presented with acute left-sided abdominal pain and a tender, irreducible mass. Computed tomography imaging revealed a left lateral ventral hernia containing an inflamed diverticulum of the transverse colon. The patient underwent emergency hybrid laparoscopic surgery involving hernia sac resection, diverticulectomy, and primary abdominal wall repair. Her postoperative course was

Botulinum Toxin A Facilitated Laparoscopic Repair of Complex Ventral Hernia.

Background: Complex ventral hernia repair can be challenging despite the recent advances in surgical techniques. Here, we aimed to examine the effectiveness of preoperative combined use of botulinum toxin A (BTA) and preoperative progressive pneumoperitoneum (PPP) for surgical preparation of patients with complex ventral hernia. Methods: In this prospective, observational study, we included 22 patients with complex ventral hernia between January 2018 and May 2021. All patients were treated with BTA injections into the lateral abdominal muscles and PPP before hernia repair. The lengths of abdom

Risks of subsequent abdominal operations after laparoscopic ventral hernia repair.

INTRODUCTION: Laparoscopic ventral hernia repair (LVHR) with intraperitoneal mesh placement is well established; however, the fate of patients requiring future abdominal operations is not well understood. This study identifies the characteristics of LVHR patients undergoing reoperation and the sequelae of reoperation. METHODS: A retrospective review of a prospectively maintained database at a hernia referral center identified patients who underwent LVHR between 2005 and 2014 and then underwent a subsequent abdominal operation. The outcomes of those reoperations were collected. Data are presen

Computed tomographic measurements predict component separation in ventral hernia repair.

BACKGROUND: Preoperative imaging with computed tomography (CT) scans can be useful in preoperative planning. We hypothesized that CT measurements of ventral hernia defect size and abdominal wall thickness (AWT) would correlate with postoperative complications and need for complex abdominal wall reconstruction (AWR). MATERIALS AND METHODS: Patients who underwent open ventral hernia repair and had preoperative abdominal CT imagining were identified from an institutional hernia-specific surgery outcomes database at our tertiary referral hernia center. Grade III and IV hernias and biologic mesh c

Spontaneous mesh evacuation per rectum after incisional ventral hernia repair.

We present a case of a 58-year-old man who was admitted to our hospital because of abdominal pain. He underwent incisional ventral hernia repair with intraabdominal mesh (ePTFE). On the day of admission, physical examination included the discovery of a foreign body in the rectum. There were no signs of acute abdomen. We induced stool, and the mesh came out with it. His further course was uneventful. Gastrografin series showed persisting fistula between the small intestine and colon, but without extralumination into the peritoneal cavity. The patient was discharged in good health and without si

Experimental evaluation of four biologic prostheses for ventral hernia repair.

PURPOSE: To evaluate two bioprostheses derived from bovine pericardium, one cross-linked (Peri-Guard) and the other non-cross-linked (Veritas), and to compare them with Alloderm and Permacol for abdominal wall repair. METHODS: The four prostheses were tested in acute and chronic hernia models. Prostheses were either sutured to the edge of the abdominal wall defect (inlay) or secured as an underlay with surgical tacks. Evaluation at 3 and 6 months included adhesion formation, defect area size and thickness, tensile strength, and histology. RESULTS: Mean adhesion coverage area ranged from 25 t

Definitive surgical treatment of infected or exposed ventral hernia mesh.

OBJECTIVE: To discuss the difficulties in dealing with infected or exposed ventral hernia mesh, and to illustrate one solution using an autogenous abdominal wall reconstruction technique. SUMMARY BACKGROUND DATA: The definitive treatment for any infected prosthetic material in the body is removal and substitution. When ventral hernia mesh becomes exposed or infected, its removal requires a solution to prevent a subsequent hernia or evisceration. METHODS: Eleven patients with ventral hernia mesh that was exposed, nonincorporated, with chronic drainage, or associated with a spontaneous enteroc

Successful salvage of infected PTFE mesh after ventral hernia repair.

Rates of hernia recurrence following repair of abdominal wall hernia defects have been shown to be lower when prosthetic biomaterials are used, but their presence may be associated with a higher rate of infectious complications. Traditional surgical teaching has advocated removal of contaminated or exposed prosthetics, although the morbidity of these revisions is high. The case presented involves a ventral hernia repair complicated by methicillin-resistant Staphylococcus aureus infection and exposed polytetrafluoroethylene mesh. The open abdominal wound was successfully managed with a combinat

Patient risk factors influencing access to timely ventral hernia repairs.

BACKGROUND: Optimization strategies, including weight loss, smoking cessation, and controlling blood glucose may delay ventral hernia surgery resulting in nonelective (ie, urgent or emergent) repair. Evidence on the risk factors influencing presentation that provide insight into who may be less amenable to optimization strategies are limited. This study evaluated the sociodemographic factors, insurance types, and medically optimizable factors associated with undergoing nonelective ventral hernia surgery. METHODS: This retrospective cohort study included ventral hernia repairs from the Nationa

Year-Over-Year Ventral Hernia Recurrence Rates and Risk Factors.

IMPORTANCE: Recurrence is one of the most challenging adverse events after ventral hernia repair as it impacts quality of life, utilization of resources, and subsequent need for re-repair. Rates of recurrence range from 30% to 80% after ventral hernia repair. OBJECTIVE: To determine the contemporary ventral hernia recurrence rate over time in patients with previous hernia repair and to determine risk factors associated with recurrence. DESIGN, SETTING, AND PARTICIPANTS: This retrospective, population-based study used the Abdominal Core Health Quality Collaborative registry to evaluate year-o

Prescription opioid use increases resource utilization following ventral hernia repair.

BACKGROUND: Previous studies showed that preoperative opioid use is associated with increased postoperative opioid use and surgical site infection (SSI) in patients undergoing ventral hernia repair (VHR). Orthopedic surgery literature cites increased resource utilization with opioid use. This study aimed to determine the effect of preoperative opioid use on resource utilization after open VHR. METHODS: A retrospective institutional review board-approved study of VHRs from a single tertiary care practice between 2013 and 2020 was performed. Preoperative computed tomography for acutely incarcerated ventral or inguinal hernia.

BACKGROUND: The utility of preoperative computed tomography for urgent abdominal wall hernia repair is unclear. This study tests the hypothesis that there is no difference in patient outcomes for acutely incarcerated ventral or inguinal hernias diagnosed by preoperative computed tomography versus clinical assessment alone. METHODS: This retrospective cohort analysis included 270 adult patients undergoing urgent repair of ventral or inguinal hernia. Demographics, risk factors for complications, operative management strategies, and 1-year outcomes were compared between patients with (n = 179) v

Outcomes specific to patient sex after open ventral hernia repair.

BACKGROUND: Male and female hernia patients often have different surgical history, fat distribution, and medical comorbidities. Female surgical patients seemingly experience worse outcomes after open ventral hernia repair. This study evaluates the impact of sex and the distribution of abdominal adiposity on outcomes after open ventral hernia repair. METHODS: A prospective hernia database was queried for patients from 2007 to 2018 with a computed tomography within 1 year of open ventral hernia repair. Three-dimensional volumetric analysis was performed. Demographics, abdominal fat distribution

Oral, intestinal, and skin bacteria in ventral hernia mesh implants.

BACKGROUND: In ventral hernia surgery, mesh implants are used to reduce recurrence. Infection after mesh implantation can be a problem and rates around 6-10% have been reported. Bacterial colonization of mesh implants in patients without clinical signs of infection has not been thoroughly investigated. Molecular techniques have proven effective in demonstrating bacterial diversity in various environments and are able to identify bacteria on a gene-specific level. OBJECTIVE: The purpose of this study was to detect bacterial biofilm in mesh implants, analyze its bacterial diversity, and look fo

Heterotopic intra-abdominal ossification in a complex ventral hernia defect.

Heterotopic ossification, an entity common in orthopaedic practice frequently involving the hip, knee or other joints, is rarely encountered in abdominal wounds and mesentery. This unusual condition, referred to as mesenteric ossification, is typically associated with intra-abdominal catastrophes. Surgical repair following such catastrophes has always been a challenge as the abdominal wall architecture is frequently distorted by the multiple laparotomies previously performed. In addition, the presence of several enterocutaneous fistulae further compounds the reconstruction approach, especially

Polypropelene mesh eroding transverse colon following laparoscopic ventral hernia repair.

Polypropylene mesh when used in laparoscopic ventral hernia repair can produce the worst complication such as enterocutaneous fistula. We report an interesting case of incisional hernia operated with laparoscopic polypropylene mesh hernioplasty who subsequently developed an enterocutaneous fistula 1 month after surgery. A fistulogram showed dye entering into the transverse colon. On exploration, the culprit polypropylene mesh was found to have eroded into the mid-transverse colon causing the fistula. Resection and end-to-end anastomosis of the colon were done with the removal of the mesh. On l

Deepithelialized flap closure: an adjunct to complex ventral hernia repairs.

Ventral hernias are a major surgical challenge with complications such as wound separations, infections, and recurrences contributing to patient morbidity. We describe a new adjunctive technique that may be helpful in repairing difficult ventral hernias: it involves using an appropriately chosen, redundant abdominal skin edge that is deepithelialized and used to reinforce the hernia repair. A series of 7 patients aged 23 to 84 years in whom the technique was used is presented. All patients had complete repair of their incisional ventral hernia defects without complications of infection, wound
