[Surgical voice rehabilitation in unilateral vocal fold paralysis].

Unilateral recurrent nerve paralysis leads to glottic insufficiency, significantly reducing vocal ability. Due to its unusually long course, the recurrent laryngeal nerve is prone to iatrogenic lesions involves many medical fields generally with little expertise in voice disorders. Whenever the etiology is uncertain, a complete diagnostic work-up is mandatory. Indirect laryngoscopy confirms the diagnosis. Laryngeal electromyography is of great value because it differentiates between paralysis and ankylosis of the cricoarytenoid joint. Moreover in many cases laryngeal electromyography yields a

Depression Among Teachers According to the Risk of Voice Disorders.

BACKGROUND: Teachers are professional voice users. Long-term and intensive voice use, noisy environments, personal factors and occupational stress experienced by these professional voice users may negatively affect vocal health, along with psycho-emotional factors such as depression. AIM: This cross-sectional and comparative study aimed to compare depressive symptoms between teachers at risk for voice disorders and those without such risk and to examine the relationship between depressive symptoms and voice-related disability levels among teachers at risk for voice disorders. Reoperative Thyroid Procedures: Do They Pose a Higher Risk of Complications Compared to First-Time Surgery?

INTRODUCTION: The outcomes of reoperative thyroid procedures are not well documented. We utilized the Collaborative Endocrine Surgery Quality Improvement Program to evaluate the outcomes and complications of reoperative thyroid surgery. METHODS: We identified patients undergoing thyroidectomy or thyroid-related procedure (central neck dissection, selective lateral neck dissection, and resection of recurrent disease) utilizing the Collaborative Endocrine Surgery Quality Improvement Program database (2013-2023). Outcomes were compared between patients undergoing first-time total thyroidectomy w

Temporal Coordination of Articulatory Gestures in Nonnative Onset Clusters: Evidence From American English Speakers Using Electromagnetic Articulography.

PURPOSE: Research in cross-language speech production indicates that, although the production of nonnative consonant clusters is often difficult, speakers of American English can produce some nonnative clusters (e.g., /fn/) with high accuracy. This ease of production for select nonnative clusters may occur due to similarity of phonetic structure with native clusters (e.g., nonnative /fn/ and native /sm/ are both fricative-nasal sequences). The current study tested this hypothesis by examining the extent of transfer of articulatory coordination from phonetically similar native onset clusters (i

Cochlear dead region and word recognition of Mandarin Chinese in Taiwan.

In a tonal language, the identity of a word depends largely on the tonal identification of the contour of vocal fundamental frequency energy of which usually centers in a low frequency of less than 600 Hz. However, cochlear dead region (DR) is present mostly in the frequency range of 2000 Hz to 4000 Hz, and the effect of DR on a tonal language is worth investigating. Thirty-two native Mandarin speakers with moderate-to-severe degree of sensorineural hearing loss were included in this study. The pure-tone audiometry, speech recognition threshold (SRT) and word recognition score (WRS) were used

Endovascular thoracic aortic transection repair has equivalent survival to open repair after blunt thoracic aortic injury.

BACKGROUND: Blunt thoracic aortic injury (BTAI) is highly lethal and its management has evolved with the advent of endovascular approaches. We hypothesized that endovascular repair (ER) would have equivalent/improved survival compared to open repair (OR). The aim of our study was to review our center's morbidity and mortality after BTAI. METHODS: Our Level I trauma center registry was queried for BTAI between 2002-2015. This cohort was stratified into three groups: ER, OR and those patients managed non-operatively (NOP). Primary endpoint was mortality and secondary endpoints included complica

Complications of Interventional Versus Surgical Closure of Patent Ductus Arteriosus in Very Preterm Infants-A Retrospective Analysis.

INTRODUCTION: Patent ductus arteriosus (PDA) is the most common cardiac anomaly in preterm newborns and may aggravate respiratory disease. Invasive closure options after failure of medical treatment include surgical ligation (SL) and transcatheter closure (TCC). Reports on side effects of intravenous contrast media are scarce. METHODS: In this retrospective single-center study, we compared 35 preterm infants below 1500 g birth weight undergoing SL with 35 matched infants undergoing TCC. Outcomes were procedural success, complications and postprocedural ventilation. RESULTS: Closure success w

High Tracheal Resection With Intralaryngeal Extension as an Alternative to Cricotracheal Resection for Treatment of Subglottic Stenosis.

OBJECTIVE: Some patients with subglottic stenosis (SGS) require open airway reconstruction, which traditionally involves resection of the anterior cricoid cartilage. As an alternative, we present a novel technique: cricoid-sparing high tracheal resection with excision of subglottic stenotic tissue from below. A novel set of posterior circumferential cricoid sutures is used to reline the exposed cricoid plate. STUDY DESIGN: Retrospective chart review. SETTING: Single tertiary care center. METHODS: The surgical technique is described and illustrated. A chart review was performed for all patie

Intraoperative neuromonitoring in traditional and miniinvasive thyroidectomy. A single center experience in 1652 nerve at risk.

BACKGROUND: The injury of laryngeal recurrent nerve (RLN) is one of the most severe adverse event in thyroid surgery. The rate in literature is reported as 2-11% for transient palsy and 0,6-1,6% for permanent palsy (after 6 months from surgical procedure). METHODS: The patients undergoing thyroid surgery associated to Intraoperative Neuromonitoring (IONM were enrolled consecutively from 21/08/2014 to 30/08/2018. WE collected data from I-IONM and not by Continuos IONM (C-IONM) because we have interrupted C-IONM after a case of cardiac arrest (15TH patient) during the APS placement. RESULTS: T

Investigation of Optimal Trafermin Dosage and Pre-Injection Factors in Vocal Fold Paralysis.

OBJECTIVE: Optimal trafermin dosages and outcome predictors for treating unilateral vocal fold paralysis (UVFP) remain unclear. We aimed to investigate the optimal trafermin dosage and pre-injection factors contributing to voice improvement after intracordal trafermin injection (ITI) for UVFP. METHODS: Medical records of 198 patients diagnosed with UVFP who received ITI at the Tokyo Voice Center between 2012 and 2024 were retrospectively evaluated. To investigate the effect of ITI on voice improvement prior to trafermin injection, we assessed age, sex, causative diseases, and pre- and post-in

Multicenter Development and Validation of the Vocal Cord Paralysis Experience (CoPE), a Patient-Reported Outcome Measure for Unilateral Vocal Fold Paralysis-Specific Disability.

IMPORTANCE: Unilateral vocal fold paralysis (UVFP) is a common and life-changing complication of cancer, trauma, and an estimated 500 000 head, neck, and chest surgeries performed annually in the US, among other causes (eg, idiopathic). Consequent disabilities are profound and often permanent and can include severe voice, swallowing, and breathing dysfunction and concomitant anxiety, isolation, and fear. Physiological measures often correlate poorly with patient-reported disability. The measure described herein was designed to be a comprehensive, psychometrically sound UVFP-specific patient-re

Vocal fold paresis: etiology, clinical diagnosis and clinical management.

PURPOSE OF REVIEW: Recent papers on incidence of unilateral paresis in general and for specific causes; techniques to assist clinical diagnosis; computer and animal modeling leading to new assessment tools; and studies on the effectiveness of clinical management. RECENT FINDINGS: There is much interest in being able to assess unilateral paresis in patients both pre and post-thyroidectomy. Because the gold standard for defining the presence of paresis is by laryngeal electromyography, which is not routinely available, much interest has been placed in finding other measures that correlate with

Melatonin binding in the house sparrow song control system: sexual dimorphism and the effect of photoperiod.

Avian song is a sexually dimorphic behavior which is regulated seasonally. This regulation involves the construction and growth of song control structures: the high vocal center (HVC), nucleus robustus archistrialis (RA), nucleus magnocellularis anterior (MAN), and Area X. Song behavior and its neural correlates are controlled by steroid-dependent and independent processes. The avian circadian system is known to be involved in both daily processes and seasonal reproduction. A major part of this system is the circadian secretion of melatonin by the pineal gland. To determine possible interactio

Vocal cord paralysis in children.

PURPOSE OF REVIEW: Vocal fold paralysis (VFP) is an increasingly commonly identified problem in the pediatric patient. Diagnostic and management techniques honed in adult laryngologic practice have been successfully applied to children. RECENT FINDINGS: Iatrogenic causes, including cardiothoracic procedures, remain a common cause of unilateral VFP. Neurologic disorders predominate in the cause of bilateral VFP. Diagnosis with electromyography is currently being evaluated in children. Treatment of VFP is centered around symptomology, which is commonly divided between voice and airway concerns.

Laryngeal fracture due to blunt trauma presenting with pneumothorax and pneumomediastinum.

AIM: Injuries due to traffic accidents are frequent in childhood, and they have high mortality and morbidity. Laryngeal injury due to a traffic accident is a rare pathology and might be missed if not suspected. Here we present a laryngeal fracture in a child after a blunt chest trauma during a traffic accident that presented with pneumomediastinum and pneumothorax. CASE: A 14-year-old girl was referred for pneumomediastinum. Her physical examination was normal except subcutaneous emphysema, edema and tenderness in the cervical area, hoarseness, facial and extremity abrasions and ecchymoses. C

Radiotherapy with 6-megavolt photons for early glottic carcinoma: potential impact of extension to the posterior vocal cord.

PURPOSE: Radiation therapy has a high success rate in the treatment of early glottic carcinoma. Excellent outcomes have been reported from centers using cobalt-60 or relatively low-energy (< or = 4 MV) radiation therapy to achieve these results. Whether similar outcomes can be achieved with a 6 MV linear accelerator has been less rigorously evaluated. This study assesses the efficacy of 6 MV radiation therapy for early stage glottic cancer and identifies prognostic factors for local control and overall survival in this common disease.
