Psoas release at the pelvic brim in ambulatory patients with cerebral palsy: operative technique and functional outcome.

Seventeen patients with cerebral palsy (29 hips) underwent psoas recession at the pelvic brim. The operative technique was a direct anterior approach, lateral to the femoral sheath. There were no infections or nerve or arterial injuries. After surgery, clinical examination revealed that fixed hip-flexion contractures decreased significantly in all patients. All of the subjects retained the ability to flex the hip against gravity and against manual resistance. All of the subjects underwent pre- and postoperative gait analysis. Stance-phase dynamic minimum hip flexion decreased significantly. Dy

Biomechanical characterization and clinical implications of artificially induced toe-walking: differences between pure soleus, pure gastrocnemius and combination of soleus and gastrocnemius contractures.

The purpose of this study was to characterize biomechanically three different toe-walking gait patterns, artificially induced in six neurologically intact subjects and to compare them to selected cases of pathological toe-walking. The subjects, equipped with lightweight mechanical exoskeleton with elastic ropes attached to the left leg's heel on one end and on shank and thigh on the other end in a similar anatomical locations where soleus and gastrocnemius muscles attach to skeleton, walked at speed of approximately 1m/s along the walkway under four experimental conditions: normal walking (NW)

[Reconstructions of the teratogenic hip dislocation in childrens with AMC].

UNLABELLED: Hip diclocation is the cause of the mobility impairment in children with arthrogryposis. Hip joint reconstruction is a commonly accepted treatment method in unilateral dislocations, but opinions are divided in bilateral. AIM OF THE STUDY: Hip joint function assessment after reconstruction due to teratogenic dislocation in arthrogryposis. MATERIAL AND METHODS: In years 1988 to 2005 we performed 9 reconstructions of the teratogenic hip dislocation in 8 children with AMC (Arthrogryposis multiplex congenita). Mean age of children at the time of procedure was 3.5 years. Mean follow-up

Hip Adduction Asymmetry in Girls with Adolescent Idiopathic Scoliosis.

Background: The aim of the study was to assess the difference in hip adduction in extended joint position in girls with adolescent idiopathic scoliosis (AIS). Methods: The study group consisted of 69 girls aged 9 to 14 years. The observational cross-sectional study involved interview, the clinical examination (body weight measurement, measuring the body height in an upright position, assessment of the alignment of the spinous processes of the thoracic and lumbar vertebrae, assessment of the location of selected anatomical landmarks of the torso), physical examination in which the shape of the

[Ehlers-Danlos syndrome with concomitant joint contractures].

We present 6 children with Ehlers-Danlos syndrome (EDS) with concomitant joint contractures. In 3 of them bilateral clubfeet together with hip, and (or) knee, or wrist flexion contractures were observed. In all of them, after initial soft tissue surgery for clubfeet, reccurences were noted requiring another surgery. Clubfeet were the only manifestation of joint contractures in another 2 children. In one of them soft tissue surgery was succesful, in the second a hypercorrection was noted. One child with EDS had a recurvatum knee deformity which was succesfully treated conservatively. We specula

Flexion contractures in secondary adrenal insufficiency.

We report the case of a 55-year-old male with flexion contractures of the hips and the knees due to an isolated adrenocorticotropin (ACTH) deficiency, a rare cause of secondary adrenal insufficiency. The presenting symptoms and signs, the laboratory investigations and the treatment are described. The case description is followed by a brief overview of ACTH deficiency and its symptoms and causes. Our case is then compared with other case reports available in the literature. The aetiology of the flexion contractures in adrenal insufficiency is still unclear. The authors want to draw attention to

Postburn perineal obliteration: elimination of perineal, inguinal, and perianal contractures with the groin flap.

Deep burns of the perineum result in perineal obliteration, hip adduction contracture, and limitation of hip range of motion. Bodily hygiene becomes difficult to maintain. Perineal obliteration is often combined with inguinal and perianal contractures and genitalia injury. These factors present a challenge for the surgeon. The extreme scar surface deficit and the fold absence in the perineal region do not allow contracture elimination with local tissues. As skin grafts shrink, success can be achieved only by using pedicled or free flaps. The groin flap is an excellent tissue for simultaneous p

Sacral agenesis.

Twenty-three patients with sacral agenesis evaluated at Newington Children's Hospital between 1959 and 1977 were classified according to patterns of morphologic deficiency in the bones and articulations. Motor deficit in these patients corresponded, within one level, to vertebral loss; sensory deficit did not correspond. The major orthopaedic problems often associated with sacral agenesis are spinopelvic instability, scoliosis, myelomeningocele, hip dislocation and contracture, knee contracture, and foot deformity. These can be controlled or corrected by proper orthopaedic management.

Operative treatment of bilateral hip dislocation in children with arthrogryposis multiplex congenita.

PURPOSE: Arthrogryposis multiplex congenita (AMC) is a rare syndrome with multiple joint contractures. It is commonly believed that bilaterally dislocated hips associated with joint contractures should not be reduced, because movement is satisfactory, while open reduction leads to poor results. This report presents our experience with surgical management of bilateral dislocation of hips in children with AMC. METHODS: During the period 1990 to 2000, we performed open reduction on 8 hips of 4 children with AMC. The mean age at surgery was 23 months (range, 5-48 months). Open reduction and capsu

Arthrogryposis multiplex congenita occurring with maternal multiple sclerosis.

All children of a mother with multiple sclerosis (MS) had increasing grades of congenital joint contractures without demonstrable neuromuscular disease. Two had talipes equinovarus, one had congenital hip subluxation, and the youngest had arthrogryposis multiplex congenita. Maternal MS may be causally related to the development of congenital joint contractures.

Congenital contractures and distinctive phenotypic features consistent with Stuve-Wiedmann syndrome in a male infant.

INTRODUCTION: Expressionless face associated with multiple contractures has been encountered in an infant. There is a wide range of misconception regarding the categorization of children with multiple contractures among different pediatric disciplines. The fundamental element in categorizing children with multiple contractures is "the etiological understanding". In the absence of concomitant neuromuscular disease, however, the search for other reasons is mandatory. Our present paper signifies the necessity of proper interpretations of unusual clinical and radiographic features. CASE PRESENTAT

[Neurophysiologic aspects of the etiology and pathogenesis of congenital hip dislocation].

Till today the meaning of neuromuscular factors in etiology and treatment of congenital dislocation of the hip is not quite clear. Our observation in 62 children with 78 dislocated hips show that neurophysiological mechanisms play of very important role in the development and upkeep of CDH. The neurological examination in 17 children pointed to the typical asymmetries of the children which correlated with the side of the dislocated hip. The knowledge of neurophysiological disturbances on the dislocated hip is of great value for the diagnosis of CDH and even more for the exercise treatment in h

Neuro-orthopedic management of the dysfunctional extremity in upper motor neuron syndromes.

Orthopedic surgery can help restore extremity function to many patients with upper motor neuron syndromes (UMN). Impairments are divided into those that cause problems with the active function of the extremity versus those that impede passive function. Limb deformities commonly result from both dynamic (spastic) and static (contractural) components. Clinical examination supplemented with dynamic electromyographic studies provides the optimal information for planning the most effective surgical procedures. In the upper extremity, selective lengthening of the shoulder adductors and extensors com

Functional adaptation after femoral intertrochanteric valgus osteotomy in Legg-Calvé-Perthes disease.

Legg-Calvé-Perthes disease (LCPD) requires individualized treatment in order to regain a functional hip joint. In severe cases, in which a congruent joint cannot be achieved, other options are necessary in order to improve functionality and prevent early osteoarthritis. Therefore, we analysed the clinical and radiologic outcome of 28 patients after valgus osteotomy of the proximal femur (VOF). We examined the range of hip motion, functionality and health-related quality of life (HRQoL) via modified Harris Hip Score (mHHS) and Kidscreen-10. Radiographic analysis contained quantitative and quali

Complex Interplay: A Gigantic Recurrent Popliteal Cyst Following Total Knee Arthroplasty in a Patient with Refractory Rheumatoid Arthritis Case Report.

INTRODUCTION: Juvenile idiopathic arthritis ( JIA) is a persistent autoimmune-inflammatory disease that affects children younger than 16. Aggressive synovitis of the hip may cause joint destruction, hip protrusion, erosion, pseudosubluxation, dysplasia, and osteoarthritis. Subluxation of the hip had been reported previously. However, dislocation of the hip in JIA is an extremely rare situation, and only two cases have been reported up to date. Reduction of the dislocated hip has to be performed in any way. However, there is no algorithm to be followed for the treatment of hip dislocations caus

Surgical Treatment of Hip Instability in Patients With Lower Lumbar Level Myelomeningocele: Is Muscle Transfer Required?

BACKGROUND: Treatment of hip instability in patients with lower lumbar level myelomeningocele is clinically challenging. Muscle transfer procedures, release of contractures, and intertrochanteric varus-rotation osteotomies have been described to restore weak or absent abductor strength as well as relocation of the hip. However, controlled trials evaluating hip instability in lower lumbar myelomeningocele are limited in the current literature. QUESTIONS/PURPOSES: The purposes of this study were to compare the (1) radiographic evidence for joint stability; (2) clinical outcomes (including abduc

Arthrofibrosis and large joint scarring.

Large joint arthrofibrosis and scarring, involving the shoulder, elbow, hip, and knee, can result in the loss of function and immobility. The pathway of joint contracture formation is still being elucidated and is due to aberrations in collagen synthesis and misorientation of collagen fibrils. Novel antibodies are being developed to prevent arthrofibrosis, and current treatment methods for arthrofibrosis include medical, physical, and surgical treatments. This article describes the biology of joint contracture formation, along with current and future pharmacologic, biologic, and medical interv

Neurophysiology of lower-limb function in hemiplegic children.

Equinus in hemiplegic children is multifactorial. In some cases it is due to a short muscle, in others to simple foot-drop, tonic spasticity, rigidity, compensation for a short limb, fixed flexion contracture at the hip, dominantly inherited forefoot deformity, forefoot equinus secondary to chronic toe-walking, or abnormalities of the visco-elastic properties of the muscle, with true intramuscular contracture. This neurophysiological study confirms that hemiplegia in children is not a homogeneous condition. Some have tonic spasticity; some, although stiff, show electrical silence on stretching

Hip dislocation in spina bifida: when is surgery required  and what type of surgery should be performed?

Children with spina bifida develop a wide variety of congenital and acquired hip joint deformities. Among these are contractures, subluxation or dislocation. This paper will review both the overall orthopedic care of a spina bifida patient with hip problems and provide a focused review of surgery management of hip deformities. Special emphasis is placed on the indications and contraindications to surgery based on a literature review and the author's personal experience.

Subtrochanteric shortening osteotomy combined with cemented total hip arthroplasty for Crowe group IV hips.

BACKGROUND: Total hip arthroplasty (THA) is a challenging surgical procedure that can be used to treat severely dislocated hips. There are few reports regarding cemented THAs involving subtrochanteric shortening osteotomy (SSO), even though cemented THAs provide great advantages because the femur is generally hypoplastic with a narrow, deformed canal. PURPOSES: We evaluated the utility of cemented THA with SSO for Crowe group IV hips, and assessed the relationship between leg lengthening and nerve injury. Our goal was to describe surgical techniques for optimizing surgical outcomes while mini
