[Clinical results and perceived health among patients with hip prosthesis].

BACKGROUND: Assessment of total hip replacement has been performed without careful attention to the patients' perceived health. The objective of this study is to assess, in a standardized way, the benefit of hip replacement in terms of hip pain, mobility, and function and as an increase in general perceived health. And to quantify clinical complications of the procedure. PATIENTS AND METHODS: Observational study of a series of 131 consecutive patients of a tertiary hospital to whom two health questionnaires (one specific and the other generic) were administered before and one year after total

Cardiovascular collapse after femoral prosthesis surgery for acute hip fracture.

PURPOSE: Prosthetic revision of hip fractures may result in embolization of tissue products leading to death. In this report, from cases reported to the Anaesthesia Advisory Committee to the Chief Coroner of Ontario, emphasis is placed on the immediate resuscitative procedures which may offset a fatal outcome. CLINICAL FEATURES: Two elderly patients are reported in whom hip fractures necessitated primary prosthetic hip repair. The first patient, with a history of limited cardiac reserve and syncope, suffered a subcapital hip fracture. Under general anaesthesia, a Moore's prosthesis was insert

Primary cementless hip arthroplasty with a titanium plasma sprayed prosthesis.

One hundred two patients underwent 105 primary uncemented total hip arthroplasties and were reviewed at a minimum of 5 years after operation (mean, 6.1 years). The components were titanium alloy with a titanium plasma spray coating. The acetabular revision rate was 11.4%. Acetabular cavitary lytic lesions were identified in 25.5% at 5 years. All acetabular revisions were performed for a combination of wear and osteolysis. One femoral revision was performed to facilitate an acetabular revision, but the femoral revision rate for aseptic loosening was 0%. In addition, no femoral components had su

Thoracic disk herniation and recurrent dislocation of total hip prosthesis.

We have described a patient who had thoracic disk herniation, recurrent dislocation of a total hip prosthesis, and heterotopic ossification. The recurrent dislocation resulted from spasticity from a spinal lesion, but the treatment focused on technical solutions related to the hip. Evaluation for total hip arthroplasty should include careful neurologic examination. If recurrent dislocation occurs, spasticity from an occult lesion should be considered. Resection of calcified thoracic disks should be approached with caution.

Do hip prosthesis related infection codes in administrative discharge registers correctly classify periprosthetic hip joint infection?

PURPOSE: Administrative discharge registers could be a valuable and easily accessible single-sources for research data on periprosthetic hip joint infection. The aim of this study was to estimate the positive predictive value of the International Classification of Disease 10th revision (ICD-10) periprosthetic hip joint infection diagnosis code in the Danish National Patient Register. METHODS: Patients were identified with an ICD-10 discharge diagnosis code of T84.5 ("Infection and inflammatory reaction due to internal joint prosthesis") in association with hip-joint associated surgical proced

Application of uncemented Zweymüller hip prosthesis in adult patients with hip osteoarthritis secondary to developmental dysplasia.

BACKGROUND: Developmental dysplasia encompasses a wide spectrum of hip pathology ranging from a shallow acetabulum to a completely dislocated 'high-riding' hip. It is a common cause of secondary osteoarthritis in young adults and is the underlying diagnosis in up to 48% of patients requiring total hip arthroplasty (THA) for coxarthrosis. The aim of this study was to evaluate efficiency and safety of THA using Zweymüller hip implant in the treatment of severe osteoarthritis secondary to developmental dysplasia of the hip (DDH) in adults. METHODS: From January 2000 to February 2008, 35 patients

Cutaneous fistula communicating with the hip in a patient with a painful total hip prosthesis. Demonstration by radionuclide arthrography.

Radionuclide arthrography is becoming increasingly useful in the evaluation of femoral component loosening in patients with a painful total hip prosthesis. Additional potential advantages of radionuclide arthrography include detection of abnormal communications with the hip, such as bursae, abscess cavities, and fistulas. A case of cutaneous fistula communicating with the hip in a patient with loosening and infection of the femoral component of the total hip prosthesis that is clearly demonstrated by radionuclide arthrography is presented.

Results of the Girdlestone Procedure for the Treatment of Refractory Hip Prosthesis Infections.

INTRODUCTION: Refractory prosthetic joint infections (PJI) of the hip remain a major challenge in orthopedic surgery. The Girdlestone procedure, also known as resection arthroplasty, was originally described for native hips but has since been adopted as a salvage option for complex and recurrent prosthetic infections. The main objective of this study was to evaluate the effectiveness of this procedure in treating prosthetic hip infections in this context. MATERIALS AND METHODS: Eighty-eight patients (91 hips) who underwent femoral resection arthroplasty for refractory hip prosthesis infection

The Role of Proton Therapy for Prostate Cancer in the Setting of Hip Prosthesis.

PURPOSE: Given that the current standard of proton therapy (PT) for prostate cancer is through bilateral beams, this modality is typically avoided when it comes to treatment of patients with hip prosthesis. The purpose of this study was to evaluate whether novel PT methods, i.e., anterior proton beams and proton arc therapy (PArc), could be feasible options to treat this patient subpopulation. We evaluate PT methods in the context of dosimetry and robustness and compare with standard of practice volumetric modulated arc therapy (VMAT) to explore any potential benefits. METHODS: Two PT and one

Total hip arthroplasty with the porous-coated anatomic hip prosthesis: results at 11 to 18 years.

The authors had previously reported good results with apparent fixation of a series of porous-coated anatomic hips at 2 to 4 years. In a larger series of 133 hips with porous-coated anatomic uncemented components, 91 hips in 82 patients were available for a follow-up of 11 years or greater. Although 3.1% of acetabular cups had migrated or had been revised at 2 to 4 years; at 11 to 18 years, there was 37% lysis, 21.7% migration, and 32.3% revision. For the femur at 2 to 4 years, there was 3.1% migration and 1.5% revision. At 11 to 18 years, there was 27.2% lysis (proximal only), 6.6% migration,

[Total hip-joint prosthesis. Critical long-term review of the new McKee Arden CAD stem prosthesis as compared with the usual McKee Arden prosthesis].

The article makes a comparison between the results of total arthroprosthesis operation of the hip with traditional Mckee Arden prosthesis (50 cases operated in the two-year period 1976-77) and those with the new McKee Arden model with C.A.D. stem (52 cases operated in the two-year period 1977-78). All operations were performed at the "G. Pini" Orthopaedic Institute by the same team and with the same surgical technique. The results are based mainly on three parameters (according to McKee): pain, amplitude of movement, and need to use supports. THe Authors conclude by stressing the efficiency of

[Infection and detachment of total hip prosthesis due to listeria monocytogenes].

Joint prostheses are increasingly used in our current practice. Infection is one of the most dreaded complications; it is most often due to germs of the skin flora. Listeria monocytogenes rarely causes this infection. We here report the case of a patient with hip prosthesis infected by this germ. Favorable outcome was obtained by antibiotic therapy and single-stage prosthesis replacement.
