American Indian youth substance abuse: community-driven interventions.

Substance abuse among American Indians has a long history that dates back to the colonial era. American Indian youth today continue to have one of the highest substance abuse rates when compared with other groups. Researchers have implemented American Indian youth substance abuse interventions that previously have worked in the general population, but studies have found that they are generally unprepared and poorly designed for American Indian populations. The lack of inclusion of American Indian populations in the interventional studies, poor understanding of American Indian diversity and cul

Beyond resilience: A scoping review of Indigenous survivance in the health literature.

Health inequity scholars, particularly those engaged with questions of structural and systemic racism, are increasingly vocal about the limitations of "resilience." This is true for Indigenous health scholars, who have pushed back against resilience as a descriptor of modern Indigeneity and who are increasingly using the term survivance. Given the growing frequency of survivance in relation to health, we performed a scoping review to understand how survivance is being applied in health scholarship, with a particular interest in its relationship to resilience. Results from 32 papers indicate th

Shattered Chains: Shedding Light on Child Sexual Abuse in the Black Community.

Child Sexual Abuse (CSA) remains deeply silenced in many Black communities, where cultural stigma, historical trauma, and systemic mistrust often discourage disclosure and limit access to supportive resources. These barriers contribute to delayed intervention, untreated trauma, and long-term psychological, emotional, and relational harms. This theoretical manuscript synthesizes interdisciplinary scholarship to examine CSA disclosure among Black children through three interconnected frameworks: Critical Race Theory, Attachment Theory, and Trauma-Informed Care, which illuminate how structural ra

The Holocaust after 70 years: Holocaust survivors in the United States(.).

Over 70 years, there have been different narratives of the Holocaust survivors coming to the United States. Survivors' stories begin with an event of major historical significance. Difficulties in conceptualizing historical trauma, along with common distortions and myths about Holocaust survivors and their children are examined. This article proposes that it is impossible to discuss the consequences of extreme suffering without consideration of historical meaning and social context with which they are entwined. The evolution of the social representation of the Holocaust and the contradictions

Traumatic air embolism; a case report and literature review of this rare complication of trauma.

Trauma management forms a significant component of any health system. It can affect any body system, and as such knowledge of the wide possible presentations and sequalae are critical. Systemic air embolism (SAE) is a rare presentation in trauma, though it can be associated with significant morbidity and mortality. We present the case of a 23-year-old gentleman with isolated penetrating trauma to the chest who developed widespread neurological insult as a result of SAE, and review historical and proposed contemporary management of SAE.

How Is Cultural Responsiveness Integrated Into Trauma-Informed Care in Adult Healthcare Settings? A Scoping Review.

ObjectiveTrauma does not occur in a cultural vacuum. Individuals' experiences of trauma and their interactions with the healthcare systems are shaped by identity, socioeconomic conditions, and historical context. Substance Abuse and Mental Health Services Administration recognises these influences through its Trauma-Informed Care (TIC) principle, 'Cultural, Historical, and Gender Issues', which is commonly operationalised through cultural responsiveness. This scoping review mapped how cultural responsiveness has been defined, implemented, and evaluated within trauma-informed adult health care

Pediatric injury trends and relationships with social vulnerability during the COVID-19 pandemic: A multi-institutional analysis.

BACKGROUND: The impact of the COVID-19 pandemic on pediatric injury, particularly relative to a community's vulnerability, is unknown. The objective of this study was to describe the change in pediatric injury during the first 6 months of the COVID-19 pandemic compared with prior years, focusing on intentional injury relative to the social vulnerability index (SVI). METHODS: All patients younger than 18 years meeting inclusion criteria for the National Trauma Data Bank between January 1, 2016, and September 30, 2020, at nine Level I pediatric trauma centers were included. The incidences of positive Kleihauer-Betke test in low-risk pregnancies and maternal trauma patients.

OBJECTIVE: This study was undertaken to determine the incidence of positive Kleihauer-Betke (KB) tests in low-risk, third trimester patients and compare them with historical "control" patients who received KB tests for maternal trauma evaluations. STUDY DESIGN: Institution Review Board approval was obtained. KB tests were performed on 100 low-risk gravid women at the time of their routine glucose challenge tests after informed consent. All specimens were analyzed for fetal hemoglobin using the Clayton modification. Patients with identified risk factors for placental abruption were excluded. A

Trauma, Power, and Intimate Relationships Among Women in Prison.

The present study, which included four focus groups of women (n = 21) in four New England prisons, aimed to understand how power impacted women's relationships, exposure to violence, and health. Women described power in three ways: (a) power as control over their sexuality and their sexual partners, (b) power emerging from emotional strength, and (c) power referring to a process of empowerment. Women's perceptions and experiences of power were informed by their trauma histories and influenced their sexual behavior and health. Our findings provide a framework for considering incarcerated women'

The Other Side of the ACEs Pyramid: A Healing Framework for Indigenous Communities.

For over two decades, extensive research has demonstrated significant associations between adverse childhood events (ACEs) and a wide range of negative health, mental health, and social outcomes. For Indigenous communities globally, colonization and historical trauma are commonly associated with ACEs, and these effects reverberate through generations. While the ACEs conceptual framework expanded pyramid is a useful model and a visual aid for understanding the historical and present-day dimensions of ACEs in Indigenous communities, a healing conceptual framework is needed to outline a path towa

Osteochondritis dissecans of the talus: case report on two family members.

Historically the investigators studying the etiology of osteochondritis dissecans can be grouped into two schools: (a) those considering trauma to previously normal bone as the chief cause; and (b) those noting causes other than trauma as being the primary reason for the lesion's development. In the case studies presented here, three medial lesions in two siblings are discussed. In neither case could trauma be substantiated.

Trauma and Chinese heroin users.

This qualitative study explores the traumas of Chinese heroin users. The results showed that the Chinese experience traumas during (childhood, later in life but prior to heroin-use onset, and throughout their heroin-use career. Themes related to the traumas include the historical economic transition, the value of family orientation, an emphasis on scholarly pursuits, the shame orientation, and a scarcity of resources conducive to recovery. This article provides a framework to understand the traumas experienced by Chinese people and offers insights on how macrofactors may impact the trauma and

That's just life: older adult constructs of trauma.

OBJECTIVES: Researcher, clinical professional and lay understandings of the meaning of trauma may differ. An awareness of older people's perspectives on trauma may be important, given the potential for historical and cultural features to shape how the term is conceptualised among this group, thereby influencing related help-seeking behaviour. METHOD: Older people living in Essex, Cambridgeshire and Suffolk were interviewed about their perceptions of the term, as was a group of mental health professionals who work with the elderly. RESULTS: Many older people regarded significant adversity as

Missing and murdered Indigenous women, girls, and Two Spirit people: a paediatric health crisis.

Indigenous children and adolescents across the USA and Canada experience increased emotional, physical, and sexual violence resulting from the longstanding effects of colonialism and historical trauma. There is a substantial lack of research exploring these issues and scarce efforts outside of Indigenous communities to support victims. However, the association between exposure to violence and abuse and adverse health outcomes among Indigenous children and adolescents is clear. In this Viewpoint, we explore this association, discuss historical context, highlight important work by governments an

Intracavernous carotid pseudoaneurysm after blunt trauma: case report and discussion.

Epistaxis following blunt facial trauma is usually self-limited and arises from the nasal mucosa, sphenopalatine, or anterior ethmoid arteries. Pseudoaneurysm of the intracavernous carotid artery (ICCA) occurs rarely with penetrating facial trauma, but is even more rare following blunt trauma. Rupture carries a greater than 50% mortality. Because rupture usually occurs after several episodes of epistaxis, recognition of subtle historic and physical findings is paramount to optimal management. Once suspicion is raised, definitive diagnosis and treatment is best accomplished with cerebral arteri

From FOB to PICU: military trauma systems that changed pediatrics.

The U.S. Department of Defense Joint Trauma System (JTS) has transformed battlefield trauma care through data-driven performance improvement, standardized guidelines, and continuous feedback. Lessons learned from pediatric wartime casualties in Iraq and Afghanistan, spanning more than two decades, have extended beyond combat, influencing how civilian pediatric trauma and disaster systems prepare for and manage mass-casualty incidents (MCIs). This manuscript examines the historical development of military trauma systems, the evaluation of Critical Care Air Transport Teams (CCATT), and the trans

Auricular Trauma Protocol for Trauma Spectrum Disorders: Development, Rationale, and Clinical Applications.

The Auricular Trauma Protocol (ATP), developed by Helms, Elkiss, and Pittman, is a six-point auricular acupuncture protocol designed for rapid, field-applicable treatment of trauma spectrum disorders. Initially developed for U.S. military use, ATP has since been applied in civilian contexts for post-traumatic stress disorder, anxiety, insomnia, and pain. This article reviews the protocol's historical development, theoretical underpinnings in neurobiology, point rationale, and clinical applications, including evidence from case studies and field implementation. The ATP exemplifies a neurobiolog

Management of cervical spine trauma: can a prognostic classification of injury determine clinical outcomes?

Although the management of cervical spine trauma is relatively complex, multiple classification systems have attempted to simplify it through the use of descriptive terms. Most historical classification systems failed to yield sufficient prognostic information to guide clinical treatment until the Subaxial Injury Classification system was developed. This classification system takes into account the injury morphology, discoligamentous complex, and the most important prognostic factor, neurologic status. The early results of this classification system have been encouraging and it is expected to
