Hoarding Disorder and a Systematic Review of Treatment with Cognitive Behavioral Therapy.

Until recently, compulsive hoarding has been treated as a type of obsessive-compulsive disorder with mixed results. Little research exists on the efficacy of behavioral interventions specifically designed to treat hoarding disorder, and most existing research is limited with regard to the numbers of participants, their ethnic and cultural diversity, and study replication; therefore, the generalizability of findings is limited. This article reviews the prevalence of hoarding disorder, cognitive behavioral therapy treatment approaches, and measurement of symptoms. A systematic review compares th

Can hoarding be a symptom of social anxiety disorder? A case study.

BACKGROUND: Hoarding is defined as the excessive collection and failure to discard possessions of apparently little value, leading to clutter, distress, and disability. Although patients with hoarding typically may feel ludicrous for not discarding useless, and sometimes bizarre, possessions, we are not aware of any previous description of patients displaying hoarding as a result of social anxiety. METHOD: Single case report. RESULTS: In this article, we describe a patient with severe social anxiety disorder who displayed hoarding as a direct consequence of social anxiety-related beliefs and

Determinants of object choice and object attachment: Compensatory consumption in compulsive buying-shopping disorder and hoarding disorder.

BACKGROUND AND AIMS: Individuals who meet criteria for compulsive buying-shopping disorder (i.e., acquiring problems only) or hoarding disorder (i.e., acquiring and discarding problems) may acquire possessions to compensate for unmet belonging needs, but may do so in different ways. Those with compulsive buying-shopping disorder may acquire objects that they believe will relieve the distress associated with unmet belonging needs (e.g., objects that distract or comfort), whereas those with hoarding disorder may acquire objects that they believe achieve belonging needs (e.g., objects that have i

Stimulus-related oscillatory brain activity discriminates hoarding disorder from OCD and healthy controls.

Hoarding disorder (HD) and obsessive-compulsive disorder (OCD) are highly comorbid and genetically related, but their similarities and differences at the neural level are not well characterized. The present study examined the time-frequency information contained in stimulus-related EEG data as participants worked on a visual flanker task. Three groups were included: participants diagnosed with HD (N = 33), OCD (N = 26), and healthy controls (N = 35). Permutation-controlled mass-univariate analyses found no differences between groups in terms of the magnitude of the oscillatory responses. Diffe

Home-based motivational interviewing for late-life rural hoarding disorder: a pilot study.

OBJECTIVES: The primary purpose of this pilot study was to determine the feasibility and acceptability of a novel brief (six-session) motivational interviewing home-based intervention for hoarding disorder designed for rural older adults. The secondary aims were to examine the engagement of proposed mechanisms of change and the preliminary efficacy of symptom reduction. METHOD: Fifteen rural-dwelling older adults were enrolled in the pilot trial for Project RECLAIM (Reduce Clutter and Increase Meaning). Engagement of proposed mechanisms of change was assessed weekly for reported engagement in

Clinical features of children and adolescents with obsessive-compulsive disorder and hoarding symptoms.

OBJECTIVE: This study was conducted to examine whether pediatric patients with obsessive-compulsive disorder (OCD) and hoarding symptoms differed in terms of clinical characteristics from pediatric OCD patients without hoarding symptoms. METHOD: Eighty children and adolescents with OCD (range, 7-17 years) completed clinician-administered and parent- and child-report measures of OCD symptom severity, impairment, and emotional and behavioral symptoms. RESULTS: Twenty-one youth endorsed significant hoarding symptoms. Relative to nonhoarders, youth with hoarding symptoms had worse insight, more

Anosognosia in hoarding disorder is predicted by alterations in cognitive and inhibitory control.

Insight impairment contributes significantly to morbidity in psychiatric disorders. The neurologic concept of anosognosia, reflecting deficits in metacognitive awareness of illness, is increasingly understood as relevant to psychopathology, but has been little explored in psychiatric disorders other than schizophrenia. We explored anosognosia as an aspect of insight impairment in n = 71 individuals with DSM-5 hoarding disorder. We used a standardized clutter severity measure to assess whether individuals with hoarding disorder underreport home clutter levels relative to independent examiners.

Cognitive rehabilitation and exposure/sorting therapy for hoarding disorder: A randomized controlled trial.

OBJECTIVE: Hoarding disorder (HD) is one of the most common and impairing neuropsychiatric conditions in late life. Cognitive Rehabilitation and Exposure/Sorting Therapy (CREST) for HD is effective in reducing HD symptoms and improving functioning when compared with case management. In the current trial (NCT02402647), we compared CREST to a robust control condition, exposure therapy (ET), to determine if CREST provides additional benefit to HD symptom reduction, functioning, and quality of life over ET. METHOD: One hundred fifteen Veterans with HD were randomized to receive 26 once weekly, 1-

Investigating the neuropsychological features of hoarding disorder using a novel virtual reality paradigm.

INTRODUCTION: Hoarding disorder represents a considerable health concern that warrants further investigation of its associated neuropsychological components. The present study examined a key aspect of the cognitive-behavioural model of hoarding, information processing (memory, attention, decision making, categorisation). Mixed findings in the literature on the presence of cognitive deficits may be attributable to the use of assessment tools with low ecological validity. Thus, novel virtual reality (VR) environments were developed to examine the information-processing components with improved e

Beliefs about possessions associated with Hoarding Disorder: A systematic review and meta-analysis.

There is substantial evidence highlighting the importance of beliefs about possessions in understanding and treating Hoarding Disorder. Despite this, there has not yet been a comprehensive review of this topic. The aim of the current pre-registered systematic review and meta-analysis was to synthesise the evidence base concerning the association between hoarding beliefs and hoarding symptom severity. Meta-analytic techniques were used to pool effect sizes concerning the association between hoarding beliefs and symptom severity (r, d) and narrative synthesis was applied to data which could not

Decluttering Minds: Psychological interventions for hoarding disorder - A systematic review and meta-analysis.

BACKGROUND: Cognitive Behavioural Therapy (CBT) is conventionally considered the primary intervention for Hoarding Disorder (HD), yet various psychological interventions have recently emerged. This study, pre-registered at Prospero (CRD42023427534), aims to comprehensively assess a range of psychological interventions, including CBT, for reducing HD symptomatology. METHODS: A systematic literature search using PubMed and SCOPUS identified 41 eligible studies comprising 47 samples (N = 1343). Risk of bias for RCTs was assessed using the Cochrane RoB2 tool, and methodological quality for all st

A systematic review and psychometric evaluation of self-report measures for hoarding disorder.

BACKGROUND: Hoarding disorder (HD) affects approximately 2.5% of the general population, leads to significant distress and impairment, and is notoriously difficult to treat. The crux of developing effective treatments for HD is our ability to reliably and validly measure relevant constructs in HD to better understand its presentation and, subsequently, formulate appropriate interventions. METHODS: We identified measures specific to HD and evaluated their psychometric properties using rating criteria formulated by the Consensus-based Standards for the selection of health Measurement INstrument

Deficits in physiological and self-conscious emotional response to errors in hoarding disorder.

Hoarding disorder (HD) has been hypothesized to arise from deficits in error monitoring and abnormalities in emotional processing, but the relationship between error monitoring and emotional processing has not been examined. We examined measures of self-report, as well as behavioral, physiological, and facial responses to errors during a Stop-Change Task. 25 participants with HD and 32 healthy controls (HC) were recruited. Participants reported on number of errors committed and pre/post emotional response to errors. Skin conductance response (SCR) during correct and error commission trials was

Cognitive behavior therapy for hoarding disorder: follow-up findings and predictors of outcome.

BACKGROUND: A cognitive-behavioral model of hoarding posits deficits in information processing, maladaptive beliefs about and attachments to possessions that provoke distress and avoidance, and positive emotional responses to saving and acquiring that reinforce these behaviors. A 26-session individual cognitive-behavioral therapy (CBT) based on this model showed significant reductions in hoarding symptoms and large effect sizes (Steketee et al.([1])). METHODS: The present study presents findings at follow-up (up to 12 months), as well as predictors of outcome at posttreatment (n = 37) and fol

Hoarding's place in the DSM-5: another symptom, or a newly listed disorder?

Hoarding behavior, long considered a symptom of obsessive-compulsive disorder (OCD) in the Diagnostic and Statistical Manual (DSM-IV-TR; American Psychiatric Association, 2000), has recently garnered significant attention and has only begun to be more carefully studied and understood. Recent research reveals that hoarding is frequently comorbid with anxiety, depression, and other diagnoses, including OCD. The DSM-5 Working Group on Anxiety, Obsessive-Compulsive Spectrum, Posttraumatic, and Dissociative Disorders is currently working to determine appropriate placement of hoarding in the DSM-5 (

Augmenting Buried in Treasures with in-home uncluttering practice: Pilot study in hoarding disorder.

Hoarding disorder is characterized by difficulty parting with possessions and by clutter that impairs the functionality of living spaces. Cognitive behavioral therapy conducted by a therapist (individual or in a group) for hoarding symptoms has shown promise. For those who cannot afford or access the services of a therapist, one alternative is an evidence-based, highly structured, short-term, skills-based group using CBT principles but led by non-professional facilitators (the Buried in Treasures [BIT] Workshop). BIT has achieved improvement rates similar to those of psychologist-led CBT. Rega

Lifetime prevalence of hoarding disorder symptoms in adults with an eating disorder: A systematic review and meta-analysis.

Hoarding disorder has been found to co-occur with eating disorders. However, no prior systematic reviews or meta-analyses have examined the pooled lifetime prevalence of hoarding disorder symptoms in adults with an eating disorder. Based on PRISMA guidelines, 4 electronic databases (MEDLINE, PubMed, PsycINFO, and Cochrane) were searched on September 17, 2025. A total of 539 articles were identified for screening using a combination of search terms. The effect size is the pooled lifetime prevalence of hoarding disorder symptoms among adults with an eating disorder. A total of 5 studies were inc

Repetitive transcranial magnetic stimulation (rTMS) for comorbid major depressive disorder and hoarding disorder: An open label pilot study.

BACKGROUND: Hoarding Disorder (HD) is characterized by persistent difficulty discarding possessions due to a strong urge to save them, resulting in excessive clutter that compromises functioning. Despite the common prevalence and profound consequences of HD, there are no FDA approved treatments. Individuals with HD often have comorbid major depressive disorder (MDD). This study examined the feasibility and initial efficacy of repetitive transcranial magnetic stimulation (rTMS) for HD patients with comorbid MDD. Pharmacotherapy for Hoarding Disorder: How did the Picture Change since its Excision from OCD?

This brief review deals with the various issues that contributed to the creation of the new Diagnostic and Statistical Manual condition of hoarding disorder (HD) and attempts at reviewing its pharmacotherapy. It appears that after the newly founded diagnosis appeared in the literature as an autonomous entity, distinct from obsessive-compulsive disorder, drug trials are not being conducted and the disorder is left in the hands of psychotherapists, who on their part, report fair results in some core dimensions of HD. The few trials on HD specifically regard the serotonin-noradrenaline reuptake i
