We present a rare case of open lateral condyle Hoffa's fracture with concurrent patellar intra-articular dislocation, quadriceps rupture and posterior cruciate ligament (PCL) avulsion. The early adolescent male sustained these injuries in a road traffic accident. Diagnostic evaluation and a multidisciplinary approach guided treatment decisions. The patient underwent single-stage open reduction and internal fixation for the lateral condyle Hoffa's fracture and quadriceps tendon repair, while conservative management was chosen for the PCL avulsion. At the 1-year follow-up, the patient exhibited

CASE: A 25-year-old man sustained a right femur Hoffa fracture with an ipsilateral patella dislocation from a twisting injury to his right knee. He underwent open reduction and internal fixation of the right femur with medial patellofemoral ligament (MPFL) reconstruction with allograft. One year postoperatively, he had returned to full activity and painless range of motion.
CONCLUSION: Hoffa fracture associated with ipsilateral patella dislocation is a rare injury in young men and warrants judicious clinician awareness. The literature regarding management of this injury combination is minimal

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

BACKGROUND Distal femoral and proximal tibial fractures often lead to nonunion and post-traumatic osteoarthritis, and total knee arthroplasty (TKA) has emerged as an effective alternative for older patients. This report includes 3 cases of successful TKA treatment. The cases involve a 42-year-old man and a 62-year-old man with Hoffa coronal fractures of the distal femur, and a 50-year-old man with a proximal tibial fracture. These patients underwent multiple osteosynthesis procedures before receiving TKA. CASE REPORT Case 1: A 42-year-old man with a displaced Hoffa's fracture had persistent kn

BACKGROUND: The stability of the Hoffa fracture fixed with a cannulated screw is limited. In the present work, we adopted two cannulated screws from anterior to posterior and posteriorly anti-gliding plate to treat 12 cases with Hoffa fracture of the lateral femoral condyle.
METHODS: Open reduction and internal fixation were performed in 12 patients with Hoffa fractures of the lateral femoral condyle. The Hoffa fracture end was fixed with an anti-gliding steel plate based on cannulated screw fixation in the rear of the distal femur. All patients were followed for 12-24 months, with a median o

OBJECTIVE: To simulate and compare the biomechanical characteristics of different internal fixation constructs for treating Letenneur type I Hoffa fractures via finite element analysis to establish a theoretical foundation for selecting a fixation method in clinical practice. METHODS: On the basis of computed tomography (CT) data acquired from a healthy adult male volunteer, a finite element model of Letenneur type I Hoffa fracture—characterized by a fracture line oriented parallel to the posterior femoral cortex—was developed. Four different internal fixation constructs were constructed: ante

INTRODUCTION: Current literature lacks recommendations regarding proper fixation of comminuted coronal plane fractures of the posterior femoral condyles (Hoffa fractures). Therefore, the aim of this study was to compare the biomechanical characteristics of different plate-augmented constructs to isolated posteroanterior screw fixation in comminuted lateral Hoffa fractures.
MATERIALS AND METHODS: Comminuted Letenneur type IIb lateral Hoffa fractures were simulated in 24 human cadaveric distal femora. The fractures were fixed with either isolated crossed posteroanterior screws or additionally w

The incidence of coronal fractures of the femoral condyle, Hoffa fractures, ranges from 8.7% to 13% of all fractures of the distal femur and are often observed in polytraumas. Hoffa fractures may be misdiagnosed and consequently not properly treated. Reduction and synthesis of this type of fracture should be achieved to avoid complications such as nonunion, pain, functional impairment. The authors present a case of a 5 year old nonunion of a Hoffa fracture of the medial condyle with chronic patellar tendon rupture. Methods: Revision surgery consisted of reduction and fixation of the Hoffa fr

Isolated coronal fractures of femoral condyle are rare in adults and nonunion of Hoffa fracture is reported only a few times in the literature. We analyzed six cases of nonunion of Hoffa fractures over a period of three years. Three patients were treated conservatively and three patients had fixation failures. Delay of presentation was 2 months to one year. Treatment protocol consisted of open reduction, excision of pseudoarthrosis, bone grafting and internal fixation along with knee arthrolysis. Union was achieved in all patients at mean 16 weeks. The treatment of nonunion of Hoffa fractures

INTRODUCTION: Hoffa fractures are rare intra-articular fractures of the distal femur in the coronal plane, typically affecting a single femoral condyle. A bicondylar involvement, essentially double Hoffa fractures, is exceedingly uncommon and rarely documented, especially in the absence of metaphyseal comminution.
CASE REPORT: We present the case of a 25-year-old male who sustained an isolated bicondylar Hoffa fracture following a motorcycle accident. Radiographs and computed tomography (CT) imaging confirmed isolated coronal plane fractures of both femoral condyles (AO 33-B3), without metaph

INTRODUCTION: Medial Hoffa fractures are rare intra-articular injuries of the femoral condyle, and their association with posterior cruciate ligament (PCL) avulsion fractures presents a highly uncommon and complex clinical scenario. This case report describes the successful surgical management of such a combination using a posterior open approach.
CASE REPORT: A 48-year-old male sustained a medial Hoffa fracture with a PCL avulsion following a road traffic accident. Surgical intervention involved open reduction and internal fixation (ORIF) through a posterior approach, allowing direct access

Coronal fractures of the femoral condyle (Hoffa fractures) are intraarticular fractures that are commonly treated surgically by open reduction and internal fixation. Surgical fixation is demanding because anatomic reduction is mandatory and adequate exposure is often difficult. Herein we describe a new technique that permits excellent visualization and fixation of lateral Hoffa fractures based on osteotomy of Gerdy's tubercle and reflection of the attached iliotibial band.

INTRODUCTION: Medial Hoffa fractures are exceptionally rare, especially when associated with intra-articular knee injuries (anterior cruciate ligament, medial collateral ligament, and meniscus tears). The discussed case highlights a single-stage fully arthroscopic fixation of a medial Hoffa fracture with associated ligament and meniscal injuries - representing a novel and effective, though challenging, treatment option. This is the first report of this kind in the literature.
CASE REPORT: A 42-year-old male patient presented 5 days after an injury to the left knee after a slip and fall in the

A coronal fracture of the distal femoral condyle is a rare entity and a nonunion even more so. The coronal fractures, also known as Hoffa fracture, more often involve the lateral than the medial condyle. Open reduction and internal fixation are the primary treatment of choice. We describe two cases of nonunion of a Hoffa fracture (one medial and one lateral). Although the nonunion fragment was not attached to any soft tissues in both cases the fragment was noted to be vital both clinically and histologically. After operative treatment, consisting of debridement, compression with use of headles

Coronal fractures of the femoral condyle (Hoffa fractures) are uncommon injuries that have a better outcome when treated surgically. We report a series of five Hoffa fractures (including one nonunion) treated at a Level 1 trauma center by one surgeon employing a protocol of open reduction and internal fixation with lag screws through a formal parapatellar approach. Postoperatively, all patients began immediate unrestricted range of motion. Initial weight bearing was limited, but all patients were permitted full weight bearing by 10 weeks. All fractures healed within 12 weeks without complicati

BACKGROUND: Open knee fracture-dislocation is a rare orthopedic injury. However, the importance of its correct management could not be overstated. To the best of our knowledge, this is the fifth study reporting a case with simultaneous Hoffa fracture and knee dislocation and the 1st study describing a patient with open plateau fracture-dislocation accompanied with Hoffa fracture, patella fracture, and patellar tendon tear. In addition, this report is noticeable as our case had no gross ligament injury unlike frequent association of knee dislocation with knee collateral ligament damage.

AIMS & OBJECTIVES: Surgical treatment of Hoffa's fracture is often challenging for orthopedic surgeons. Current surgical techniques recommend stabilization of most of these fractures using a plate and screws. The objective this study was to design, simulate using Finite Element Analysis (FEA), and to describe the mechanical behavior of a specific orthopedic implant model to treat a Letenneur type III Hoffa fracture.
MATERIALS & METHODS: From CT scan images in DICOM format, a 3D bone model was created and modeled using Invesalius and Meshmixer software. The orthopedic implants were modeled by

A case of isolated tangential fracture of the dorsal part of both condyles of the femur (bicondylar Hoffa fracture) is described. After open reduction, four lag screws were introduced through the fractured condyles to fix the fragments. A plaster cast was applied for a period of four weeks. Full weight-bearing was authorized after six weeks. Full range of motion of the knee was observed as early as after three months, as well as after twenty-one months. The mechanism and therapeutic implications are discussed and the literature is reviewed.

Dual-plating of the distal femur is required in some cases to achieve stable fixation. The indications of a medial plate in addition to the lateral plate are medial supracondylar bone loss, low trans-condylar bicondylar fractures, medial Hoffa fracture, peri-prosthetic distal femur fractures, non-union after failed fixation with single lateral plate, poor bone quality and comminuted distal femur fractures (AO type C3). We recommend orthogonal plate configuration with locked plates by a single incision or dual incision approach as per surgeon choice.
