Nutritional care in a nursing home in Italy.

INTRODUCTION: Malnutrition is a clinical condition due to the imbalance among needs, intake and use of nutrients, leading to the increase of morbidity and mortality, and to the impairment of quality of life. Even in industrialized countries undernutrition is becoming an alarming phenomenon, especially involving elderly institutionalized subjects. A multicentric study called PIMAI (Project Iatrogenic MAlnutrition in Italy), was carried out in Italy over 2005. The aims of this study were to determine the prevalence of malnutrition in hospitals and in nursing care homes (NH), to assess the level

Causes of death in elderly nursing home residents.

OBJECTIVES: According to previous studies, 20% to 24% of deaths in the United States occur in nursing homes. This number is increasing. Overall vital statistics document that the leading causes of death in the United States remain cardiovascular/cerebrovascular and cancer, with Alzheimer's disease seventh as of 2004. However there are few statistics on causes of death specifically in the nursing home population. The objective of this study was to determine the most common causes of death in one nursing home in our practice, and specifically to see how common it was to die of dementia in the nu

Care needs of higher-functioning nursing home residents.

OBJECTIVES: To quantify and characterize the chronic conditions of older Americans who live in nursing homes (NHs) but have minimal disability and might be able to live in less restrictive and less expensive settings. DESIGN: Secondary analysis of the 1999 National Nursing Home Survey. PARTICIPANTS: NH staff members familiar with the care of residents who were 65 years or older and had resided in the NH for more than 100 days. MEASUREMENTS: We defined "higher-functioning" residents as those who received help from NH staff in 0-2 activities of daily living. We then classified these higher-fu

[Trichophyton violaceum infection occurring in a nursing home].

Six cases of Trichophyton (T.) violaceum infection seen in a nursing home are reported. A 66-year-old female (case 1) was found with tinea corporis on her face, chest and shoulder, associated with black dot ringworm. A KOH examination of hair showed endothrix parasitism. Reddish purple colonies were isolated from the patient on Sabouraud's dextrose agar, and intercalary and terminal chlamydospores were observed on slide culture. PCR-RFLP analysis of the microorganism showed a pattern of T. violaceum type. Therefore, the isolated fungus was identified as T. violaceum, a typical anthropophilic d

Predictors of nursing home placement in Huntington disease.

OBJECTIVE: To determine whether motor, behavioral, or psychiatric symptoms in Huntington disease (HD) predict skilled nursing facility (SNF) placement. METHODS: Subjects were participants in the Huntington Study Group's Unified Huntington Disease Rating Scale Database (Rochester, NY) between January 1994 and September 1999. Specific motor, psychiatric, and behavioral variables in subjects residing at home and in SNF were analyzed using chi2 and Student's t-tests. For a subset of subjects for whom longitudinal data existed, a Cox proportional hazards model controlling for age, sex, and disease

Predictors of aspiration pneumonia in nursing home residents.

Aspiration pneumonia is a serious problem for the elderly institutionalized person, often requiring transfer to a hospital and a lengthy stay there. It is associated with a high mortality rate and is very costly to the health care system. The current study sought to determine the key predictors of aspiration pneumonia in a nursing home population with the hope that health care providers could identify those residents at highest risk and focus more efforts on prevention of this serious disease. A cross-sectional, retrospective analysis was done, using the Minimum Data Set (MDS) nursing home ass

Nursing interventions for urinary incontinence in home health.

Urinary incontinence (UI) is a significant health care issue among older patients in the acute care, extended care, and home care settings. With the shift toward health care delivery in the home setting, it is becoming increasingly necessary for home health care professionals to become knowledgeable about the causes, assessment, and treatment. This article will review the epidemiology, causes, assessment, and management of UI in the home care setting. Emphasis is placed on the role of the WOC nurse as coordinator of a multidisciplinary team providing care of the home bound patient with UI.

Individualizing nighttime incontinence care in nursing home residents.

BACKGROUND: Urinary incontinence is a common problem among nursing home (NH) residents and can be successfully treated with prompted voiding during daytime hours. A previous study of incontinent NH residents demonstrated that noise from the staff and other residents and light and noise from staff-initiated incontinence care routines were associated with waking episodes. Nighttime incontinence care should be individualized to minimize sleep disruption while considering moisture exposure that could affect skin health. Although descriptive studies have been published, there are no published inter

Prognosis of falls among elderly nursing home residents.

The survival rate of 207 nursing home residents who fell during a two-month study period was followed for 12 months and compared with that of 94 residents of the same nursing home who did not fall during the same period. One year after the fall, 74 fallers and 13 controls had died. In the group of fallers, the main factors associated with mortality were male gender, dementia, Parkinsonism, the use of antidepressants, diuretics and vasodilators. Among the controls, the main factors associated with mortality were Parkinsonism, diabetes and the use of diuretics. Thirty-eight fallers (33 women and

Mental disorders in the nursing home: another perspective.

OBJECTIVE: Many studies of the prevalence of mental disorders among residents of long-term care facilities have had substantial methodological shortcomings. This study was conducted to replicate a previous well-designed study and to examine additional characteristics of nursing home residents. METHOD: Randomly selected residents (N = 80) in a public long-term care facility were evaluated by a psychiatric team using DSM-III-R criteria and quantitative assessments of behavior and cognitive status. RESULTS: Of the 80 subjects, 91% had at least one psychiatric diagnosis and at least one behavior

[Norton's decubitus risk score in a nursing home].

Decubitus must be considered an important problem in public health care. In the Netherlands (total population 15 million) the costs of prevention and treatment of decubitus in the hospitals and nursing homes are approximately Dfl. 700 million per year. In order to identify patients at risk for the development of decubitus at an early stage. Norton and colleagues developed a scoring system that includes an assessment of general physical condition, mental status, activity level, mobility and incontinence. In a prospective study in 224 somatic nursing home patients we investigated the relationshi

UTI: managing the most common nursing home infection.

Urinary tract infections (UTIs) are the most common type of infection in nursing homes. Asymptomatic bacteriuria, the most common form, requires no treatment. Symptomatic UTI requires treatment, but symptoms (acute deterioration in functional ability or mental status) may be nonspecific. Complicated UTI is more common in nursing home residents than in other populations. Immobility and the use of bedpans instead of commodes contribute to decreased bladder emptying and an increased potential for UTI. Poor perineal hygiene in many females in nursing homes also increases the potential for UTI. For

Determinants of transitory and permanent nursing home admissions.

This article describes an analysis of data from the 1982-84 National Long-Term Care Demonstration Project to estimate the risks of any nursing home admission, a temporary or transitory admission, and a permanent admission. Using a multinomial logit model, the relative predictive power of several individual characteristics on nursing home use and admission type were evaluated. It was found that the cognitively impaired subgroup was at the greatest risk of entering a nursing home, especially on a permanent basis. The results also demonstrated that the combination of cognitive impairment and func

Clinical features of depression in the nursing home.

The prevalence of depression among nursing home residents was estimated by screening a group of residents selected from a random sample on the basis of cognitive status; it ranged from 18-20% for major depression to 27-44% for other dysphoric states. Though individual patients were observed to change over a 6-month period, depression as assessed with the Geriatric Depression Scale was, in general, persistent. Major depression was clinically confirmed in 8 of 10 patients identified at screening. It was characterized by medical disorders that complicated diagnosis and treatment, and by increased
