Challenges in hospice and end-of-life care in the transfusion-dependent patient.

Despite promising advances leading to improved survival, many patients with hematologic malignancies end up dying from their underlying disease. Their end-of-life (EOL) care experience is often marked by worsening symptoms, late conversations about patient values, increased healthcare utilization, and infrequent involvement of palliative care and hospice services. There are several challenges to the delivery of high-quality EOL care that span across disease, patient, clinician, and system levels. These barriers include an unpredictable prognosis, the patient's prognostic misunderstandings and

Quality of Dying and Death of Patients With Cancer in Hospice Care in Uganda.

PURPOSE: Despite advances in palliative care in Uganda, there has been relatively little recent patient-centered research investigating end-of-life outcomes in this region. We assessed the quality of dying and death of patients with cancer in hospice care in Uganda. METHODS: Bereaved caregivers of patients who received hospice care in Uganda and died 2-12 months earlier (N = 201) completed the Quality of Dying and Death Questionnaire, which includes 31 items and single-item ratings of overall quality of dying and moment of death, and the FAMCARE measure of family satisfaction with cancer care

Music Therapy and Nursing Cotreatment in Integrative Hospice and Palliative Care.

Integrative hospice and palliative care is a philosophy of treatment framing patients as whole persons composed of interrelated systems. The interdisciplinary treatment team is subsequently challenged to consider ethical and effective provision of holistic services that concomitantly address these systems at the end of life through cotreatment. Nurses and music therapists, as direct care professionals with consistent face-to-face contact with patients and caregivers, are well positioned to collaborate in providing holistic care. This article introduces processes of referral, assessment, and tr

Pharmacovigilance in hospice/palliative care: net effect of haloperidol for delirium.

INTRODUCTION: Prescribing practice in hospice/palliative care is largely extrapolated from other areas of clinical practice, with few studies of net medication effects (benefits and harms) in hospice/palliative care to guide prescribing decisions. Hospice/palliative care patients differ in multiple ways from better studied participant groups, hence the applicability of studies in other participant groups is uncertain. Haloperidol, a butyrophenone derivative and dopamine antagonist, is commonly prescribed for nausea, vomiting, and delirium in hospice/palliative care. Its frequent use in deliriu

Hospice care for patients living alone: results of a demonstration program.

The Wissahickon Hospice initiated a demonstration program in 1988 to provide home hospice care to individuals living at home without primary care persons. This paper describes the planning process and admission requirements for the Live Alone Program and the evaluation results for 34 Live Alone patients compared to 105 regular hospice patients. Principal findings were that the hospice was able to maintain patients at home, to provide care safely to patients, and to enable home deaths for patients living alone. Patients living alone tended to require more supportive services than regular patien

Hospice family caregivers' preparedness to provide care during the death vigil.

Family caregivers of hospice patients face numerous demands during their family member's final days of life, including escalating symptom management needs, physical strain, and intense emotional challenges. Informed by preparedness theory and the National Cancer Institute framework for patient-centered communication (adapted for family caregivers), this qualitative descriptive study explored the knowledge, skills, and presence family caregivers required to fulfill their role as their family member was actively dying, as well as their perceptions of their communication with hospice teams during

Caregiver Activation and Home Hospice Nurse Communication in Advanced Cancer Care.

BackgroundActivated patients have the skills, knowledge, and confidence to manage their care, resulting in positive outcomes such as lower hospital readmission and fewer adverse consequences due to poor communication with providers. Despite extensive evidence on patient activation, little is known about activation in the home hospice setting, when family caregivers assume more responsibility in care management.ObjectiveWe examined caregiver and nurse communication behaviors associated with caregiver activation during home hospice visits of patients with advanced cancer using a prospective obse

Caregiving Relationships, Advance Care Planning, and Hospice Use in Later Life.

BACKGROUND: Little is known about whether primary caregiver relationship is associated with hospice use or advance care planning (ACP) in later life. This study examined variation in hospice use and ACP across primary caregiver relationships. METHODS: Gateway Harmonized Health and Retirement Study End-of-Life data were restricted to Wave 12 and later. The sample included 3952 decedents aged 60 years or older. Caregiving category identified the primary caregiver as spouse/partner, child/grandchild, extended family, or non-kin informal, with formal care only and no identified caregiver as addit

Medicare Plan Switching and Hospice Care Among Decedents With Advanced Cancer.

IMPORTANCE: Hospice is central to end-of-life (EOL) care for patients with advanced cancers and is an excluded benefit under Medicare Advantage (MA), with coverage instead provided by traditional Medicare (TM). With growing MA penetration, more beneficiaries also switch between MA and TM for financial protection and physician access considerations, although less is known about how different Medicare programs and plan switching behaviors affect EOL care for patients with advanced cancers. OBJECTIVE: To evaluate hospice utilization and places of hospice care by Medicare plan switching patterns.

Private Equity Ownership in Hospice Care: A Systematic Review (2012-2026).

Hospice care is associated with improved end-of-life outcomes. Recent shifts in hospice utilization highlight several key trends. Alzheimer's disease and related dementias (ADRD) (25%) have surpassed cancer (23%) as the leading primary diagnosis. Concurrently, industry ownership has transitioned from predominantly nonprofit to for-profit (70%) and private equity (PE) ownership has grown dramatically from 3% to 15%. To date, no study has synthesized evidence on PE ownership in hospice care. We conducted a systematic review of English-language, peer-reviewed studies published 2012-2026, followin

Transfusion Use Among Children With Hematologic Malignancies Receiving Concurrent Hospice Care.

BACKGROUND: Patients with hematologic malignancies enroll in hospice at lower rates than other diagnoses, potentially due to loss of transfusion support. OBJECTIVE: This study evaluated transfusion use among children with hematologic cancers enrolled in home-based hospice programs designed to maintain transfusion availability. METHODS: This three-year retrospective review included patients <21 years with hematologic malignancies enrolled in hospice between October 2022 and 2025. Structured query language reports and manual chart reviews captured demographic and clinical data. RESULTS: Thirt

Intensity of Healthcare Utilization Among Pediatric Hospice Patients Receiving Concurrent Care.

BACKGROUND: As children with life-threatening con- ditions near end-of-life, their disease burden and care requirements tend to increase, leading to high intensity of care in the hospital. Previous studies have examined patterns of healthcare usage among these children receiving hospice, but little is known about the impact of pediatric hospice involvement on healthcare utilization compared with usual care. We aimed to examine whether pediatric patients enrolled on concurrent hospice care utilize healthcare services at the same rate as prior to hospice enrollment. METHODS: A retrospective cha

Simplifying palliative symptom management: elastomeric infusions in hospice and home care.

BACKGROUND: Palliative care patients often require frequent, resource-intensive, intermittent subcutaneous injections or complex infusion pumps for end-of-life symptom management. PURPOSE: We aimed to explore the feasibility of using non-electronic elastomeric infusion devices for continuous subcutaneous medication delivery in hospice and home care. METHODS: A quality improvement initiative included an environmental scan to evaluate elastomeric devices and treatment protocols, followed by a stepwise implementation of elastomeric infusion devices for symptom management of palliative care pati
