Medication incidents reported to an online incident reporting system.

AIMS: Approximately 20% of deaths from adverse events are related to medication incidents, costing the NHS an additional £500 million annually. Less than 5% of adverse events are reported. This study aims to assess the reporting rate of medication incidents in NHS facilities in the north east of Scotland, and to describe the types and outcomes of reported incidents among different services. Furthermore, we wished to quantify the proportion of reported incidents according to the reporters' profession. METHODS: A retrospective description was made of medication incidents reported to an online r

[Identity vigilance and implementation of transfusion software in hospitals].

In hospitals, patients' identity mismatch is the cause of serious incidents during transfusion or other healthcare processes. With a wider use of healthcare software, new dysfunctions and risks may occur in case of ID mismatch between different software and for transfusion between blood bank and hospital. Four critical stages are to be considered when using software management of hospital care: initial patients' identification, modifications of identification data during hospital stay and then synchronization of all different software, proper use of patient's identification means by hospital s

Breaking down barriers: Call-taker strategies to address caller perception of inappropriateness of cardiopulmonary resuscitation during the emergency ambulance call.

BACKGROUND: Ambulance call-takers perform the critical role of prompting callers to initiate and continue cardiopulmonary resuscitation (CPR) for patients with suspected out-of-hospital cardiac arrest (OHCA). This study aimed to identify call-taker strategies to address callers' perceptions of CPR 'inappropriateness' (perceiving the patient as dead and beyond help, or as showing signs of life). METHODS: Using a linguistic approach, we analysed 31 calls previously identified as having an inappropriateness barrier to CPR initiation or continuation. In Phase 1, we listened to call recordings and

Adopting real-time surveillance dashboards as a component of an enterprisewide medication safety strategy.

BACKGROUND: High-alert medications are frequently responsible for adverse drug events and present significant hazards to inpatients, despite technical improvements in the way they are ordered, dispensed, and administered. METHODS: A real-time surveillance application was designed and implemented to enable pharmacy review of high-alert medication orders to complement existing computerized provider order entry and integrated clinical decision support systems in a tertiary care hospital. The surveillance tool integrated real-time data from multiple clinical systems and applied logical criteria t

Unlocking the Mystery: Resident-Led POCUS Intervention in Community Hospital Revealing Pericardial Tamponade in a Complex Case.

Pericardial effusions, though relatively rare, can lead to life-threatening complications such as cardiac tamponade. While viral etiologies are common culprits, rapid and accurate diagnosis remains challenging. We present the case of a 74-year-old male with a history of upper respiratory infection who developed sudden onset dyspnea and chest discomfort. Bedside point-of-care ultrasound (POCUS) revealed a large pericardial effusion, prompting urgent intervention. Despite initially stable vital signs, the patient rapidly deteriorated, necessitating emergent pericardiocentesis. Laboratory finding

A review of verbal order policies in acute care hospitals.

BACKGROUND: Although verbal and telephone orders (VOs) are commonly used in the patient care process, there has been little examination of the strategies and tactics used to ensure their appropriate use or how to ensure that they are accurately communicated, correctly understood, initially documented, and subsequently transcribed into the medical record and ultimately carried out as intended. A systematic review was conducted of hospital verbal and telephone order policies in acute care settings. METHODS: A stratified random sample of hospital verbal and telephone order policy documents were

Partnering With Doulas in Clinical Settings.

Fostering positive and strong partnerships between clinicians and doulas is essential and can benefit patients, clinicians, and the workplace. The evidence suggests that, when doulas are part of health care teams, they improve patients' experiences and outcomes. Doula-clinician partnerships have demonstrated improvements in communication and patient-centered care, accountability in the health care team, and continuity of care for individuals who have given birth. Doula support aligns with evidence-based outcomes and reduces costs to the overall health system. Barriers to doula-clinician partne

A Parent's Perspective on Systemic Gaps in Supporting Complex Pediatric Care at Home.

Children with complex medical needs often require substantial medical care, including at home, where family members perform many procedures typically reserved for licensed nurses. This article presents a caregivers' perspective of navigating the healthcare system while managing her daughter's care following a diagnosis of Ataxia-Telangiectasia, a rare neurodegenerative disease. It reflects on the systemic gaps in how we prepare and support family caregivers, particularly regarding end-of-life discussions. This article highlights how caregivers are uniquely positioned to identify inefficiencies

The experiences of people with type 2 diabetes in communicating with general practitioners in China - a primary care focus group study.

BACKGROUND: With the implementation of health care reforms in China, primary care is on a journey to provide care for most patients with type 2 diabetes. While Chinese general practitioners (GPs) have described challenges in communication with diabetes patients in their daily practice, little is known about patients' experiences in communicating with their GPs. METHODS: Five focus groups (of 4-5 participants each) were used to explore views from patients with type 2 diabetes. Purposive sampling was used to recruit a spread of participants from general practices in Guangzhou city, China. Focus

Patient Safety During Transitions: Identifying and Mitigating Risks.

Transitions of care, such as intrahospital handoffs and hospital discharge, are high-risk periods for adverse events like medication errors and diagnostic delays, often stemming from communication lapses and care fragmentation. Evidence-based strategies, including multidisciplinary team collaboration, standardized handoff protocols, and electronic health record tools, can mitigate these risks and improve patient safety. Emerging technologies, such as artificial intelligence and remote monitoring systems, offer opportunities to personalize care and provide real-time support. Engaging patients a

Is health information technology associated with patient safety in the United States?

(1) Estimates using national data provide tentative evidence of a positive relationship between health information technology (HIT) and clinical quality. Electronic medical records (EMRs) are associated with a statistically significant aversion of two post-operative infections per year at the average U.S. acute care hospital. (2) The study found no significant relationships for two other types of HIT - nurse charting and picture archiving communication systems (PACS) - or for two other measures of patient safety - post-operative hemorrhages/hematomas and post-operative pulmonary embolism/deep

A structured communication model for advance care planning in Chinese oncology nursing: a mixed-methods participatory action research study.

BACKGROUND: Despite the global recognition of advance care planning as a critical component of patient-centred end-of-life care, its implementation remains challenged by skill-based deficiencies (e.g., inadequate training), cultural and communication barriers, and system-level structural impediments within healthcare settings. This study aimed to develop and implement a structured advance care planning communication model to improve nurses' communication practices and facilitate patient engagement in end-of-life care discussions. METHODS: A participatory action research design with embedded m

Outcomes of 72-Hour Hospital Treatment Responsibility Post Discharge to Municipal Nursing Care.

BACKGROUND: The increasing prevalence of individuals living with chronic conditions and multi-morbidity presents challenges to healthcare systems and coordination of coherent patient pathways. In Denmark, transitions between healthcare sectors are susceptible to disruption due to inadequate cross-sectoral collaboration. To improve continuity of care and cross-sectoral collaboration, an agreement of 72-hour hospital treatment responsibility post-discharge was implemented. METHODS: We used data from a questionnaire survey for healthcare professionals, data from electronic patient records, and d

Factors affecting paramedics situational awareness in out of hospital emergencies: A mixed methods systematic review.

INTRODUCTION: Out-of-hospital emergency care is complex, time-critical, and demands rapid decisions that directly affect patient safety. Paramedics deliver life-saving treatment in unpredictable environments, requiring swift judgment and adaptability under pressure. Despite expertise, errors occur-especially under stress. Situational awareness (SA), the ability to perceive, interpret, and anticipate key elements, is vital for safe, effective care and coordination. Yet research on how paramedics develop and maintain SA in these challenging settings remains limited, leaving critical knowledge ga

Outbreak of listeriosis associated with consumption of deli meats in a hospital, Germany, February to March 2023.

Listeria monocytogenes can cause severe illness in individuals with weakened immune systems. In March 2023, L. monocytogenes was isolated from blood (n = 2) or pleural fluid (n = 1) of three febrile patients receiving synthetic glucocorticoids in a tertiary hospital in Germany. Food supply records suggested sliced parboiled sausage as the likely source, and L. monocytogenes was isolated from four samples of sealed packaged sliced sausages and ham from one manufacturer. The patient and food isolates clustered within 0-4 allelic differences. Counts of L. monocytogenes in all four food samples we

Critical care readiness in conflict and disaster-affected Somalia: mass-casualty response, emergency referral, and continuity of life-saving care special collection: critical care in war and disaster areas.

Critical care readiness during war and disasters is often judged by intensive care unit capacity, yet preventable mortality in fragile health systems may occur before, during, or after ICU-level care. In Somalia, recurrent mass-casualty events, emergency referral constraints, limited oxygen and blood readiness, constrained operative access, and weak post-resuscitation monitoring expose a critical gap between first contact and sustained life-saving care. This Comment addresses that gap by reframing critical care readiness in conflict and disaster-affected Somalia as a pathway problem rather tha
