Temporal trends in depression-related hospitalizations and suicide among patients with inflammatory bowel disease.

BACKGROUND: Individuals with inflammatory bowel disease (IBD) are at higher risk of depression. However, data on depression-related hospitalizations and suicide death remain limited. METHODS: This population-based study used administrative data to identify annual prevalent cohorts of individuals with IBD and matched controls between fiscal years 2005/2006 to 2021/2022. The primary outcomes were hospitalization for depression and suicide death. An IBD incident cohort was used to estimate the 1-, 3-, and 5-year cumulative incidence with 95% confidence intervals (CIs). Temporal trends were evalu

Improving hospital-based care for patients with injection-drug related infections: provider perspectives.

BACKGROUND: Injection-related severe bacterial infections (SBIs), including skin and soft tissue infections, endocarditis, and osteomyelitis, are rising in prevalence in the United States and disproportionately affect people who inject drugs (PWID). Hospitalization for SBI presents a critical opportunity to engage patients in addiction and infectious disease care, yet healthcare systems often fail to capitalize on this moment. METHODS: This qualitative study explores the perspectives of 22 clinical stakeholders, including physicians, surgeons, nurses, and social workers, on barriers and strate

Thick Liquids and Clinical Outcomes in Hospitalized Patients With Alzheimer Disease and Related Dementias and Dysphagia.

IMPORTANCE: Oropharyngeal dysphagia is common in hospitalized patients with Alzheimer disease and related dementias (ADRD). Although the use of thick liquids in patients with dysphagia has been shown to reduce aspiration on direct visualization, there is no clear evidence that this practice translates into improved clinical outcomes. OBJECTIVES: To determine whether a diet of thick liquids compared with thin liquids is associated with improved outcomes in hospitalized patients with ADRD and dysphagia. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included adults aged 65 years and olde

Factors Related to the Development of Infective Endocarditis in Hemodialysis Patients in a Third-Level Hospital in Panama.

INTRODUCTION: Venous access for hemodialysis (HD) makes patients more susceptible to transient bacteremia, predisposing them to the development of infective endocarditis (IE). Among the risk factors observed in this population are temporary access to HD, hypoalbuminemia, diabetes mellitus, female gender, anemia, and colonization by methicillin-resistant Staphylococcus aureus (MRSA). METHODOLOGY: A retrospective case-control study with a one-to-two ratio was carried out on patients with chronic kidney disease (CKD) undergoing renal replacement therapy with at least one vascular access for HD a

MiR-150 levels are related to in-hospital mortality in non-HIV Pneumocystis pneumonia patients.

Pneumocystis pneumonia (PCP) is a common opportunistic infection that occurs in immunocompromised patients. Compared with HIV patients, PCP in non-HIV patients tends to follow up a more urgent course and poorer prognosis. Therefore, markers that could predict survival of PCP patients in non-HIV population are of great value. MiRNA-150 has been widely studied in many diseases since it has been identified as a vital regulator of immune cell differentiation and activation. We thus conduct this study aiming to evaluate the prognostic value of miR-150 level in non-HIV PCP. First, the expression lev

Annual Cardiovascular-Related Hospitalization Days Avoided with Tafamidis in Patients with Transthyretin Amyloid Cardiomyopathy.

BACKGROUND: Patients with transthyretin amyloid cardiomyopathy (ATTR-CM) experience infiltrative cardiomyopathy and heart failure symptoms requiring costly hospitalizations. The Transthyretin Amyloidosis Cardiomyopathy Clinical Trial (ATTR-ACT) demonstrated the efficacy of tafamidis on the frequency of cardiovascular (CV)-related hospitalizations in patients with ATTR-CM. PURPOSE: As length of stay can affect the total hospitalization burden, our study aimed to better understand the impact of tafamidis on the number of CV-related hospital days avoided in the management of ATTR-CM patients. Reducing the risk of oxygen-related fires and explosions in hospitals treating Covid-19 patients.

On 24 April 2021, a disastrous fire in an Iraqi hospital took the lives of 82 people. Since the outbreak of the pandemic in March 2020, incidents of oxygen-related hospital fires in various countries around the world have caused over 200 deaths, the majority of whom were patients extremely ill with the novel Coronavirus. Fires involving medical oxygen are not a new phenomenon but are more common in the operating theatre where oxygen is routinely administered. In these settings, strict safety protocols are normally enforced and surgical staff are well trained in dealing with oxygen hazards. It

Hospitalization-related economic impact of patients with cardiogenic shock in a high-complexity reference centre.

AIMS: Cardiogenic shock (CS) is associated with high mortality. Current guidelines strongly recommend centralizing the care of these patients in high-complexity centres. We described the hospitalization-related economic cost and its main determinants in patients with CS in a high-complexity reference centre. METHODS AND RESULTS: This is a single-centre, retrospective study. All patients with CS (2015-17) were included. Hospitalization-related cost per patient was calculated by analytical accountability method, including hospital stay-related expenditures, interventions, and consumption of dev

Risk factor-related lifestyle habits of hospital-admitted stroke patients - an exploratory study.

AIMS AND OBJECTIVES: To describe and explain the lifestyle habits of hospital-admitted stroke patients by identifying relationships between the different lifestyle habits and by examining associations between them and clinical measurements. BACKGROUND: Stroke is associated with several risk factors that are closely intertwined. However, lifestyle factors have profound effects on its incidence. It has been shown that healthy lifestyle habits are associated with a reduced stroke risk both in combination and independently. DESIGN: A cross-sectional, exploratory study. METHODS: Data were collec

A machine learning model for in-hospital mortality prediction in patients with sepsis-related myocardial injury.

Sepsis-related myocardial injury (SRMI) is associated with high mortality, but the transportability of prediction models across heterogeneous cohorts remains uncertain. We developed and externally validated a machine learning model for in-hospital mortality prediction in SRMI and evaluated its discrimination, calibration, interpretability, and distributional shift across cohorts. Using MIMIC-IV for development and MIMIC-III and a local cohort for external validation (n = 1,226, 962, and 280, respectively), we compared eight classification algorithms. A two-stage feature-selection strategy, rec

Traumatic brain injuries in paediatric patients: individual vs. team sports-related hospitalizations.

Paediatric sport participation continues to increase in the United States, with a corresponding increase in sports-related concussions or traumatic brain injuries (TBIs). It is important to recognize which sports are at elevated risk and identify risk factors for hospital admission and length of stay (LOS). Paediatric patients (ages 5-18) from 2008 to 2014 were identified from the Healthcare Cost and Utilization Project (HCUP) National Inpatient Sample (NIS). Eight hundred and ninety-four patients included those who were hospitalized with a TBI resulting from participation in an individual (45

Retrospective identification of patients with SRRM2-related neurodevelopmental disorder in a single tertiary children's hospital.

SRRM2-related neurodevelopmental disorder is a recently described genetic diagnosis caused by loss-of-function variants in SRRM2. In order to understand the clinical spectrum of SRRM2-related neurodevelopmental disorder, we performed a retrospective exome data and clinical chart review at a single tertiary children's hospital, Children's Hospital of Philadelphia (CHOP). Among approximately 3100 clinical exome sequencing cases performed at CHOP, we identified three patients with SRRM2 loss-of-function pathogenic variants, in addition to one patient previously described in the literature. Common

Opioid-related adverse drug reactions in patients visiting the emergency division of a tertiary hospital.

Opioid use and associated morbidity and mortality have increased in several countries during the past 20 years. We performed a study whose objective was to assess the frequency and causes of opioid-related emergency division (ED) visits in an adult tertiary Swiss University Hospital over 9 weeks in 2018. We primarily assessed opioid-related adverse drug reactions (ADR), secondary overdose, misuse, abuse, and insufficient pain relief. Current opioid use was identified in 1037 (8.3%) of the 12 470 included ED visits. In 64 opioid users, an ADR was identified as a contributing cause of the ED vis
