A hospitalist-led short-stay rehabilitation unit reduced inpatient discharge delays.

Hospitalized patients requiring post-acute rehabilitation often remain in acute care while awaiting skilled nursing facility placement. In July 2023, the San Francisco VA Health Care System opened a hospitalist-led short-stay rehabilitation unit within its Community Living Center, called the Lotus. Mean referral-to-discharge time, excluding referral hold periods, decreased from 6.89 days in fiscal year (FY) 2023 to 4.58 days in FY2024 and 3.35 days in FY2025. The Lotus became the lowest-delay post-acute care destination and the facility's most frequently used individual rehabilitation destinat

Approach to Children with Aggressive Behavior for General Pediatricians and Hospitalists: Part 2-Evaluation, Tests, and Treatment.

After reviewing the various etiologies that can contribute to a presentation of aggressive behavior in a child (see part 1), a physician should conduct a thorough history and physical examination. The history should be obtained from the patient and caregivers, both together and separately in adolescents. A good physical examination starts with assessment and interpretation of vital signs, followed by a head-to-toe examination focusing on the skin, eyes, and thyroid, and then a neurologic examination. The testing and observation should be tailored to the individual patient, including laboratory

Impact of optimal medical therapy in heart failure certification for hospitalists on guideline-directed medical therapy utilization.

BACKGROUND: Significant gaps in guideline-directed medical therapy (GDMT) for heart failure (HF) stem from shortages of cardiologists and advanced HF providers, as well as a lack of optimal HF management knowledge among hospitalists. This study compared the impact of optimal medical therapy in HF (OMT-HF) certification on GDMT implementation and patient outcomes between an intervention group (IG) of hospitalists and a standard-of-care comparison group (SOC-CG). AIM: To evaluate if OMT-HF has a difference in GDMT and patients in outcomes between IG and SOC-CG. METHODS: This study was implemen

Derivation of a risk-adjusted model to predict antibiotic prescribing among hospitalists in an academic healthcare network.

BACKGROUND: Among inpatients, peer-comparison of prescribing metrics is challenging due to variation in patient-mix and prescribing by multiple providers daily. We established risk-adjusted provider-specific antibiotic prescribing metrics to allow peer-comparisons among hospitalists. METHODS: Using clinical and billing data from inpatient encounters discharged from the Hospital Medicine Service between January 2020 through June 2021 at four acute care hospitals, we calculated bimonthly (every two months) days of therapy (DOT) for antibiotics attributed to specific providers based on patient b

Global well-being, anxiety, social isolation, and emotional support among hospitalists during COVID-19 and Mpox outbreaks.

OBJECTIVE: The Coronavirus Disease-19 (COVID-19) pandemic caused a decline in hospitalist wellness. The COVID-19 pandemic has evolved, and new outbreaks (i.e. Mpox) have challenged healthcare systems. The objective of the study was to assess changes in hospitalist wellness and guide interventions. METHODS: We surveyed hospitalists (physicians and advanced practice providers [APPs]), in May 2021 and September 2022, at a healthcare system's 16 hospitals in four US states using PROMIS® measures for global well-being, anxiety, social isolation, and emotional support. Why the 2025 Nobel Prize in Economics Matters at the Bedside: Innovation, Institutions, and Hospitalist Leadership.

The 2025 Nobel Prize in Economics, awarded to Joel Mokyr, Philippe Aghion, and Peter Howitt for their work on innovation-driven growth, offers an unexpectedly practical framework for hospital medicine. Their scholarship emphasizes that progress depends not only on technological advances, but also on institutions capable of implementing, governing, and adapting innovation in real operational settings. This perspective resonates with hospital medicine, a field born from institutional redesign and now facing a new inflection point driven by digital technology and artificial intelligence. Hospital

Introducing the Hospitalist Morale Index: A new tool that may be relevant for improving provider retention.

OBJECTIVE: To develop a valid instrument to assess morale and explore the relationship between morale and intent to leave employment due to unhappiness. PATIENTS AND METHODS: An expert panel identified 46 drivers of hospitalist morale. In May 2009, responders of our single-site pilot survey rated each driver in terms of current contentment and importance to their morale. With exploratory factor analysis, a 28-item/7-factor instrument emerged. In May 2011, the refined scale was distributed to 108 hospitalists from 2 academic and 3 community hospitals.
