Haemophagocytic lymphohistiocytosis (HLH) is a hyper-inflammatory syndrome characterised by widespread, uncontrolled T cell and histiocyte activation with accompanying cytokine storm. It can be inherited or acquired, with acquired forms triggered by infection, malignancy or autoimmune disease. There is high mortality, with patients commonly requiring critical care support. HLH is a relatively rare condition (though it is likely underdiagnosed), and there is an incomplete understanding of its pathogenesis, diagnosis and optimal management. In the course of one calendar year (1989-1990), 46 specimens of respiratory secretions (from 44 patients) cultured in the microbiology department of a large district general hospital in The Netherlands were found to yield Neisseria meningitidis. The production of sedoheptulose-7-phosphate in the red blood cells of 15 patients was found to be below the suggested normal value. This intermediate product of glucose metabolism is the result of transketolase activity for which thiamine pyrophosphate is one of the co-factors. Without thiamine, the transketolase reaction is inhibited. Some nonlocalizing symptoms, perhaps previously thought due to hypoglycemia, may be occasioned by faulty glucose metabolism at the cellular level from a relative lack of thiamine.

We analyzed the clinical and laboratory findings of 19 patients with tuberculous meningitis seen between 1966 and 1974 at the Henry Ford Hospital. Eighteen patients were adults at the time of diagnosis. In eight patients, the history suggested that the infection with the tubercle bacillus had occurred in the remote past. The information given by urodynamic investigations has been of practical value in clinical practice and we feel that the time and money spent has been worthwhile. The results are particularly useful in patients with neuropathic bladders, women and stress incontinence in whom detrusor instability is also suspected, young men with "obstructive" symptoms and in men with post-prostatectomy problems. Its value in te large group of women with frequency, urgency and nocturia is debatable given our present lack of satisfactory treatment for these patients.

We report 32 patients with erythema nodosum leprosum (ENL). Twenty-two patients developed ENL before beginning chemotherapy. The recognition that ENL in the absence of chemotherapy is not an uncommon event is of importance, not simply as a fact in and of itself, but because this observation raises provocative questions when placed in the context of current ideas concerning ENL.
