[Surgical therapy in cholecysto-/choledocholithiasis. Results of a Germany-wide questionnaire sent to 859 clinics with 123,090 cases of cholecystectomy].

BACKGROUND: With the advent of laparoscopic cholecystectomy, the choice of procedure (laparoscopic vs open) and surgical management in gallstone disease are of interest. The purpose of this study was to examine the current approach and various strategies for cholecystectomy in Germany. METHODS: A retrospective survey at 859 (of 1,200, 67.6%) hospitals in Germany was conducted. Data from 123,090 patients that had undergone cholecystectomy were analyzed. RESULTS: 71.9% of the procedures were finished laparoscopically (LC: 88.537), whereas 22.5% (27.727) were done in the open technique (KC). Co

Neural regulation of tone in the oesophageal body: in vivo barostat assessment of volume-pressure relationships in the feline oesophagus.

BACKGROUND: Deimplementation, the systematic elimination of low-value practices, has emerged as an important focus within implementation science. Bronchiolitis is the leading cause of infant hospitalization. Among stable inpatients with bronchiolitis who do not require supplemental oxygen, continuous pulse oximetry monitoring is recognized as an overused, low-value practice in pediatric hospital medicine. There is strong scientific evidence and practice guideline support for limiting pulse oximetry monitoring of stable children with bronchiolitis who do not require supplemental oxygen, yet the

Bleeding After Musculoskeletal Surgery in Hospitals That Switched From Hydroxyethyl Starch to Albumin Following a Food and Drug Administration Warning.

BACKGROUND: While US Food and Drug Administration (FDA) black box warnings are common, their impact on perioperative outcomes is unclear. Hydroxyethyl starch (HES) is associated with increased bleeding and kidney injury in patients with sepsis, leading to an FDA black box warning in 2013. Among patients undergoing musculoskeletal surgery in a subset of hospitals where colloid use changed from HES to albumin following the FDA warning, we examined the rate of major perioperative bleeding post- versus pre-FDA warning. METHODS: The authors of this article used a retrospective, quasi-experimental,

Sex and Race/Ethnicity Differences in Implantable Cardioverter-Defibrillator Counseling and Use Among Patients Hospitalized With Heart Failure: Findings from the Get With The Guidelines-Heart Failure Program.

BACKGROUND: Previous studies have found that women and black patients eligible for a primary prevention implantable cardioverter-defibrillator (ICD) are less likely than men or white patients to receive one. METHODS: We performed an observational analysis of the Get With The Guidelines-Heart Failure Program from January 1, 2011, to March 21, 2014. Patients admitted with heart failure and an ejection fraction ≤35% without an ICD were included. Rates of ICD counseling among eligible patients and ICD receipt among counseled patients were examined by sex and race/ethnicity. RESULTS: Among 21 059

An emergency department intervention to protect an overlooked group of children at risk of significant harm.

BACKGROUND: Parental psychiatric disorder, especially depression, personality disorder and deliberate self-harm, is known to put children at greater risk of mental illness, neglect or physical, emotional and sexual abuse. Without a reliable procedure to identify children of parents presenting with these mental health problems, children at high risk of significant harm can be easily overlooked. Although deliberate self-harm constitutes a significant proportion of emergency presentations, there are no guidelines which address the emergency physician's role in identifying and assessing risk to ch

Utility of 90-Day Mortality vs 30-Day Mortality as a Quality Metric for Transcatheter and Surgical Aortic Valve Replacement Outcomes.

IMPORTANCE: Questions have recently arisen as to whether 30-day mortality is a reasonable metric for understanding institutional practice differences after transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR). OBJECTIVE: To examine the utility of 30-day vs 90-day mortality after TAVR and SAVR as a mortality quality metric. DESIGN, SETTING, AND PARTICIPANTS: This nationally representative, multicenter, cohort study analyzed data from Medicare beneficiaries undergoing TAVR and SAVR procedures from January 1, 2012, to December 31, 2015. Substantial international variation in the cost of blood group and save and crossmatch: A systematic review.

Blood transfusion is one of the most common interventions in hospitals. Blood tests that support this widespread practice are considered routine, such as 'group and save' (also known as 'type and screen'), yet there has been very limited evaluation of the needs for these tests and the associated costs. The aim of this systematic review is to synthesise international evidence on the costs of group and save and crossmatch tests and their component processes. Seventy-two studies published between 2012 and 2023 were identified and data were extracted. Most cost estimates were derived from local in

Exploring Pain Management for People Living With Dementia in Acute Care Settings Using the COM-B Framework to Guide Baseline Data Gathering.

AIM: To establish a comprehensive pre-implementation baseline of pain management for people living with dementia (PLWD) in acute hospitals by exploring behavioural, contextual, organisational and clinical practice factors to inform the implementation of the DOTS approach. BACKGROUND: Pain is common yet frequently under-recognised and inconsistently managed in PLWD in acute care. Behaviour change frameworks, such as COM-B, may help identify determinants of practice and inform implementation strategies. DESIGN: Multiple-methods study, underpinned by the COM-B framework and guided by the Medica

Hypospadias: The lay of the land.

INTRODUCTION: Hypospadias surgery is uniquely split across the specialities of paediatric surgery urology and plastic surgery. The division may restrict best practice as there is a paucity of cross-speciality forum for discussion and dissemination. Separately achieving training competence is a challenge. The authors present a travelling fellowship as a template future programme to address some of these issues. METHODS: A fellowship was established visiting seven UK hospitals. Surveys were sent to mentors and plastics trainees via the BAPRAS Specialist Interest Group and Plastic Surgeon Traine

Comparing prevalence and types of potentially inappropriate medications among patient groups in a post-acute and secondary care hospital.

Reducing potentially inappropriate medications (PIMs) is a challenge in post-acute care hospitals. Some PIMs may be associated with patient characteristics and it may be useful to focus on frequent PIMs. This study aimed to identify characteristic features of PIMs by grouping patients as in everyday clinical practice. A retrospective review of medical records was conducted for 541 patients aged 75 years or older in a Japanese post-acute and secondary care hospital. PIMs on admission were identified using the Screening Tool for Older Person's Appropriate Prescriptions for Japanese. The patients

A cluster randomised trial to assess the impact of clinical pathways on AMI management in rural Australian emergency departments.

BACKGROUND: People living in rural Australia are more likely to die in hospital following an acute myocardial infarction than those living in major cities. While several factors, including time taken to access hospital care, contribute to this risk, it is also partially attributable to the lower uptake of evidence-based guidelines for the administration of thrombolytic drugs in rural emergency departments where up to one-third of eligible patients do not receive this life-saving intervention. Clinical pathways have the potential to link evidence to practice by integrating guidelines into local

Low-Interventional Approaches to Intrapartum Care: Hospital Variation in Practice and Associated Factors.

INTRODUCTION: Despite evidence supporting the safety of low-interventional approaches to intrapartum care, defined by the American College of Obstetricians and Gynecologists as "practices that facilitate a physiologic labor process and minimize intervention," little is known about how frequently such practices are utilized. We examined hospital use of low-interventional practices, as well as variation in utilization across hospitals. METHODS: Data came from 185 California hospitals completing a survey of intrapartum care, including 9 questions indicating use of low- versus high-interventional
