Hospital volume stratification using stratum-specific likelihood ratios for pancreatectomy.

BACKGROUND: Recent literature suggests that volume-outcome associations for pancreatectomy have attenuated over time, leading some researchers to question the continued relevance of volume thresholds. However, this perceived attenuation may reflect the methodological limitations of single, binary cutoffs rather than a true weakening of the underlying relationship. Stratum-specific likelihood ratios (SSLRs) generate multiple empirically derived volume strata that may detect persistent gradients that are obscured by binary stratification. The objectives of this study were to (i) define volume st

The Impact of Hospital Volume on Postoperative Outcomes for Esophagectomy and Gastrectomy: A Systematic Review and Meta-analysis.

BACKGROUND: High-volume hospitals tend to have better outcomes in complex surgeries, but outcome variations and volume thresholds complicate conclusions. This systematic review/meta-analysis assessed the impact of hospital volume on postoperative outcomes after esophagogastric surgery and identified volume thresholds beyond which benefits plateau. METHODS: PubMed/MEDLINE was searched for cohort studies (2013-2023) on volume and outcomes after esophagectomy/gastrectomy for cancer. Primary outcome was 30-day mortality. Secondary outcomes included 90-day mortality, complications, length of stay

Association Between Hospital-Level Volume of Extracorporeal Membrane Oxygenation and Complications: A Clustered, Mixed-Effect Analysis of the Extracorporeal Life Support Organization Registry.

OBJECTIVE: Extracorporeal membrane oxygenation (ECMO) has been used to provide pulmonary and circulatory support in patients with ongoing critical illness. Studies investigating whether an association exists between ECMO center-volume and complications are lacking. Our main aim was to investigate whether an association between ECMO center-volume and the rate of ECMO-related complications for adult patients exists. DESIGN: Retrospective observational study. SETTING: Patients from the Extracorporeal Life Support Organization Registry were collected from 2018 to 2021. SUBJECTS: Adult patients

Utilization of low-volume hospitals for total hip replacement.

OBJECTIVE: To identify factors associated with utilization of low-volume hospitals for total hip replacement (THR) and to estimate differences in the distances that Medicare beneficiaries had to travel to reach low- or high-volume hospitals. METHODS: We studied a population-based sample of 1,146 Medicare beneficiaries who underwent elective THR in 1995. Using multiple data sources including medical record review, Medicare claims data, 1990 Census data, and a patient survey, we examined factors independently associated with utilization of low-volume hospitals for elective THR. We estimated the

The Relationship Between Hospital Immunotherapy Volume and Treatment and Survival of Advanced Cutaneous Melanoma.

INTRODUCTION: Immunotherapy (IMT) has improved survival for patients with advanced cutaneous melanoma, yet disparities in IMT receipt persist. Additionally, hospital case volume is associated with outcomes in melanoma; however, the relationship between hospital IMT volume and treatment patterns and survival across racial groups remains unknown. We examined associations between hospital-level IMT volume and receipt of IMT/survival among patients with advanced cutaneous melanoma by race. METHODS: Adults with stage III/IV cutaneous melanoma diagnosed between 2012 and 2020 were identified in the

[Should the surgical treatment of primary malignant bone tumors only take place in high-volume centers?].

BACKGROUND: Primary malignant bone tumors are rare, biologically heterogeneous malignancies that require highly specialized, multidisciplinary treatment. Surgical excision of the primary tumor is considered a prerequisite for long-term survival in most affected patients. An association between hospital volume and outcome has been demonstrated for numerous oncological procedures but has been investigated less extensively in the context of bone sarcomas. MAIN FINDINGS: The surgical expertise concentrated at high-volume centers (HVC) appears to translate directly into superior oncologic outcomes

Type A aortic dissection: optimal annual case volume for surgery.

BACKGROUND AND AIMS: The current study proposes a novel volume-outcome (V-O) meta-analytical approach to determine the optimal annual hospital case volume threshold for cardiovascular interventions in need of centralization. This novel method is applied to surgery for acute type A aortic dissection (ATAAD) as an illustrative example. METHODS: A systematic search was applied to three electronic databases (1 January 2012 to 29 March 2023). The primary outcome was early mortality in relation to annual hospital case volume. Data were presented by volume quartiles (Qs). Restricted cubic splines we

Effects of Extracorporeal Life Support Volume on Neonatal Mortality in Congenital Diaphragmatic Hernia: A Retrospective Cohort Study.

We conducted a retrospective cohort study of 29 499 neonatal cases requiring extracorporeal life support (ECLS) at 237 centers from 2000 to 2023; there were 6747 infants with congenital diaphragmatic hernia (CDH) and 22 752 without CDH. Infants with CDH treated at ECLS centers with low total neonatal volumes (<28.5 total cases) had increased odds of inpatient mortality compared with those at high-volume centers (> 140 total cases, adjusted odds ratio [aOR]: 1.9; 95% confidence interval [CI]: 1.2-2.9). Similarly, centers with low non-CDH ECLS volumes were associated with higher odds of mortalit

Center Volume and Failure to Rescue in Pediatric Cardiac Surgery: Is There a Relationship?

BACKGROUND: Failure to rescue (FTR) is a validated quality metric for evaluating outcomes in pediatric cardiac surgery. Using a national administrative database, we sought to assess the relationship between annual hospital case volume and FTR. METHODS: Cardiopulmonary bypass (CPB) cases were extracted from the Kids' Inpatient Database for 2016 and 2019. FTR was defined as inpatient mortality of patients with at least 1 postoperative complication. Hospitals were divided into high-, mid-, and low-volume hospitals on the basis of annual CPB case volume terciles. A mixed effects multivariable log

Practice Patterns and Outcomes After Hospital Admission With Acute Para-esophageal Hernia in England.

OBJECTIVE: (i) To establish at a national level clinical outcomes from patients presenting with acute para-esophageal hernia (PEH); and (ii) to determine if a hospital volume-outcome relationship exists for the management of acute PEH. BACKGROUND: Currently, no clear guidelines exist regarding the management of acute PEH, and practice patterns are based upon relatively small case series. METHODS: Patients admitted as an emergency for the treatment of acute PEH between 1997 and 2012 were included from the Hospital Episode Statistics database. The influence of hospital volume upon clinical out

Understanding the relationship between hospital volume and patient outcomes for infants with gastroschisis.

BACKGROUND: For many surgical operations, there is a well-established relationship between surgical volume and outcome. We investigated whether this relationship exists for infants with gastroschisis. METHODS: Using the Kids' Inpatient Database for years 2003, 2006, 2009, and 2012, we identified all patients undergoing gastroschisis repair. Controlling for patient characteristics and complexity of disease (comorbid intestinal atresia/perforation, necrotizing enterocolitis, and respiratory distress syndrome), we compared surgical outcomes (mortality, length of stay, and incidence of TPN choles
