Posterior Cloacal Variant with Clitorolabial Transposition and a Rectoperineal Fistula.

We present a case and discuss the management of a posterior cloacal variant not as yet described in the literature. A 5-week-old infant presented to our institution with a posterior cloacal variant and transposition of the clitoris and labia. After initial radiological investigations, staged operative intervention was performed over a 1-year period. This included an initial laparotomy (with drainage of hydrocolpos and formation of a colostomy), a left ureteric reimplantation and a posterior sagittal anorectoplasty due to a rectoperineal fistula. The child is under continued long-term follow-up

Urethral Duplication Mimicking an Interlabial Cyst in a Girl: A Diagnostic Challenge.

Urethral duplication is a rare urogenital anomaly, especially in females. We present the case of a preterm female with various congenital anomalies, hydrocolpos, and a persistent interlabial cyst, later found to be secondary to urethral duplication. At age 2, cystoscopy and VCUG confirmed a duplicated urethral tract. Surgical resection of the accessory urethra and genitoplasty were performed successfully. Postoperative recovery was uneventful, with excellent cosmetic and functional outcomes at follow-up. This case highlights urethral duplication as a rare but possible differential diagnosis fo

Imperforate hymen and vaginal atresia and their associated anomalies.

The presenting features and associated abnormalities of imperforate hymen and vaginal atresia were studied in 24 girls under the age of 16 years. Hydrocolpos or hydrometrocolpos occurred in 8 infants, 13 older girls developed haematocolpos, but 3 of the girls had no distension of the genital tract. Seven of the older girls were diagnosed as having appendicitis. Anorectal anomalies were present in 9 of the children. Intravenous pyelography was performed on 16 girls and was normal in only one. Urinary tract investigations are indicated in all girls with vaginal outlet obstruction, and the vagina

Ultrasonographic diagnosis of congenital hydrometrocolpos in prenatal and newborn period: a case report.

Our purpose is to present the ultrasonographic findings of a rare case of prenatally and postnatally congenital hydrometrocolpos secondary to imperforate hymen. By ultrasonography (US) at 38 weeks of gestation, a retrovesical, 60 x 43 mm pelvic cystic mass, was demonstrated in a female fetus. After delivery, US of the newborn revealed a 77 x 60 mm retrovesical, pear-shaped cystic structure with internal echoes, interpreted as congenital hydrometrocolpos. Mild pelvicaliectasis in the left kidney was associated. At physical examination imperforate hymen was detected and a hymenotomy was done. Af

Unusual presentation of McKusick-Kaufman syndrome in a female Bedouin Arab baby.

McKusick-Kaufman syndrome is a rare, autosomal, recessive disorder characterized by hydrometrocolpos, post-axial polydactyly, and congenital heart disease. Less than one hundred cases have been reported in the English literature to date, mainly in the Amish population; sporadic cases have also been described. We present a case of an Arab Bedouin girl who presented with features resembling this syndrome.

McKusik-Kaufman syndrome: prenatal diagnosis, genetics and follow up.

McKusick-Kaufman syndrome (MKKS) is a rare autosomal recessive genetic disease with classical hexadactyly and hydrocolpos in females and sometimes cardiac abnormality. We report such a case diagnosed just before birth with a favourable outcome. From this case we describe and discuss all the prenatal sonographic signs which are not always present. On the genetic side, the gene has recently been localized together with the mutation responsible for MKKS. The phenotypic relationship between MKKS which has a good prognosis and Bardet-Biedl syndrome (BBS) with a worse prognosis requires great cautio

Rupture of the female urethra in childhood.

Rupture of the female urethra is a rare disorder, but perhaps more liable to occur in childhood than in adult life. As in the male, it can follow a fractured pelvis, but the level of the rupture is not simply in the membranous region, and is frequently much higher with destruction of the urethra over a considerable length. The vagina is always involved, so that almost inevitably there is a urethra-vaginal fistula. In the low lesions the bladder and urethral function may be adequate, but stenosis of the vaginal introitus can result in a considerable hydrocolpos, leading to a spurious form of in

Complete transverse vaginal septum complicated by pyocolpos, primary vesicoureteral reflux and ectopic ureter with renal loss: a case report.

A transverse vaginal septum is a rare Müllerian anomaly caused by incomplete fusion between the Müllerian ducts and the urogenital sinus. It can obstruct menstrual flow, leading to hematocolpos, hydrocolpos, or pyocolpos. We report a 12-year-old girl with recurrent hematocolpos, abdominal pain, urinary symptoms, and infections. Imaging showed a complete transverse vaginal septum and grade II vesicoureteral reflux. She underwent surgical drainage, septum resection, and ureteral reimplantation. Recovery was favorable, with symptom resolution and preserved renal function. This case emphasizes the

Uterus didelphys and vaginal duplication with unilateral obstruction presenting as a newborn abdominal mass.

Vaginal duplication with uterus didelphys is a rare anomaly in which unilateral obstruction of either vaginal orifice may result in hydrocolpos but with normal-appearing external genitalia in the female newborn. We recently treated 2 female infants with this anomaly who presented with a lower abdominal mass that persisted after bladder drainage. Both infants had evidence of urological abnormalities on the side ipsilateral to the obstructed vagina. One infant had renal agenesis and 1 had severe renal dysplasia. Owing to the high incidence of associated renal abnormalities excretory urography, v

[Spontaneous rupture of the imperforate hymen in two newborns].

Imperforate hymen is the most frequent congenital malformation of the female genital tract and usually remains asymptomatic until puberty. The diagnosis of this malformation is rarely made at birth and surgical treatment is often by hymenotomy. However, spontaneous rupture of the imperforate hymen is possible but is very rare. We report two cases of imperforate hymen revealed by mucocolpos at birth. Ultrasound examination does not objectify hydrocolpos or hydrometrocolpos. Spontaneous rupture of the imperforate hymen was observed respectively in the 4th and 7th day of life with a satisfactory

The incomplete cloaca and Bardet-Biedl syndrome: A remarkable association with broader implications on patient's care.

BACKGROUND: Incomplete cloaca is a rare cloacal variant that refers to a persistent urogenital sinus in girls with the bowel opening anteriorly in the perineum or the vestibule (summary figure). Most of these girls present primarily to pediatric surgeons/urologists as neonatal hydrocolpos. This study outlines the diagnosis and management of this condition while highlighting its association with Bardet-Biedl syndrome (BBS). The latter is a genetically heterogenous group of autosomal recessive disorders characterised by multisystem affection: pigmentary retinopathy, obesity, mental defect, polyd

Radiologic diagnosis of a newborn with cloaca.

When a female is born and has only a single perineal orifice on the newborn clinical examination, a diagnosis of cloaca type of anorectal malformation is made. Along with associated malformations which may initiate the ordering of radiologic imaging, there are a finite number of radiologic tests that are performed to help in the immediate management of the patient with cloaca. The following discussion will outline the most important radiologic tests and demonstrate examples of images from newborn females with cloaca.

Isolated hydrometrocolpos and cloacal malformation: can we prenatally distinguish them?-A case report and literature review.

BACKGROUND: Hydrometrocolpos (HMC) is a rare prenatal ultrasound abnormality, presenting in two distinct types: urinary-type and secretory-type. The urinary variation is intricately linked to cloacal malformation (CM), thereby posing a heightened risk of perinatal adverse events. Additionally, children affected by this type often face long-term challenges that impact their quality of life. Therefore, predicting the presence of CM in HMC fetuses is of great clinical significance. CASE PRESENTATION: We present a case involving hydrometrocolpos accompanied by oligohydramnios. Prenatally, the con

Imperforate hymen and urinary retention in a newborn girl.

Urinary retention is relatively rare in infants, especially in girls. Imperforate hymen is the most frequent congenital malformation of the female genital tract and is usually asymptomatic until puberty. Mucocolpos with an abdominal mass in neonatal age is extremely rare. We report a case of a 20-day-old newborn girl with acute urinary retention due to isolated imperforate hymen and mucocolpos.

Risk for recombinants in pericentric inversions of the (p11 leads to q21) region of chromosome 18.

A child with female hypospadia complicated by bilateral hydronephrosis, hydroureter, and hydrocolpos was heterozygous for a pericentric inversion of chromosome 18, 46,XX,inv(18)(p11q21). The normal mother and her father had the same inversion. The abnormal phenotype of the girl could be due to undetectable recombination or to a position effect. She had a low level of the enzyme peptidase-A whose locus is on 18q, while her mother and grandfather had normal levels. The two other cases of familial inversions for chromosomes 18 in the literature both involve the same (p11 leads to q21) region. The

Antenatal diagnosis of hydrometrocolpos with Mullerian duplication on ultrasound and fetal MRI: case report and literature review.

Fetal abdomino-pelvic cystic lesions are uncommon and can have varied etio-pathogenesis. Most commonly they originate from the gastrointestinal or genitourinary tract. These include choledochal cyst, hydronephrosis, renal cyst, mesenteric/omental cyst, ovarian cyst, meconium pseudocyst, and hydrocolpos/hydrometrocolpos among others. Fetal hydrometrocolpos is rare with a reported incidence of 0.006% and its diagnosis requires a high index of suspicion. Antenatal ultrasound and magnetic resonance imaging (MRI) is invaluable in diagnostic evaluation. This case report describes the imaging feature

Imperforate hymen: a new benign reason for highly elevated serum CA 19.9 and CA 125 levels.

Imperforate hymen is a urogenital tract anomaly that represents the most frequent congenital malformation of the female genital tract. CA 19-9 and CA 125 are widely used as tumor markers, however several benign conditions are also known to increase serum CA 19-9 and CA 125 levels. According to classical textbook knowledge, imperforate hymen is not listed under the benign conditions that increase serum CA 19-9 and CA 125 levels. In this case report we describe a relation between imperforate hymen and elevated serum CA 19-9 and CA 125 levels.
