Intradural-extramedullary inflammatory pseudotumour of the cervical spinal cord of a dog.

An 8-year-old mixed-breed dog was presented with cervical hyperesthesia, tetraparesis, and mild proprioceptive ataxia in all four limbs. 3 Tesla MRI showed a dorsal compressive intradural-extramedullary mass at the level of C1-C2, isointense to the gray matter with a hypointense ventral core on T2 weighted images (WI), isointense on T1WI, with a strong and homogeneous contrast enhancement. A C1-C2 partial dorsal laminectomy was performed, and the lesion was removed en bloc. The histopathological and immunohistochemical analysis defined the diagnosis of inflammatory pseudotumor.

[Paranoid personality disorder].

PARANOID PERSONALITY DISORDER. The paranoid personality disorder fascinates and worries health professionals, who are sometimes victims of aggressive claims from their patients. Overestimation of oneself, psychorigidity, distrust and relational hyperesthesia characterize the paranoid type of personality disorder. This disorder is often associated with co-morbidities which mask it and promote decompensation towards a delusional disorder with the risk of heteroaggressive acts or towards a severe depressive disorder with suicidal risk. On the basis of regular monitoring and exhaustive assesment, …

Syringomyelia in three small breed dogs secondary to Chiari-like malformation: clinical and diagnostic findings.

Three small breed dogs were referred for the evaluation of neurologic deficits. Upon physical and neurologic examination, all dogs displayed hyperesthesia, pain, and neck stiffness. Magnetic resonance imaging was performed on the brain and spinal cord, and all three dogs presented Chiari- like malformations and syringomyelia. These dogs were treated with prednisolone and furosemide, and showed rapid improvement of clinical signs. Chiari malformations and syringomyelia were not improved because of congenital disorders. This case report demonstrates the clinical and diagnostic features of Chiari…

Eosinophilic meningitis due to Angiostrongylus cantonensis in Switzerland.

Eosinophilic meningitis is a well-known disease in areas where the rat lung worm Angiostrongylus cantonensis is endemic, i.e. in Southeast Asia and in the Caribbean. We report a case of eosinophilic meningitis due to infection with Angiostrongylus cantonensis in a traveler returning from Cuba. The diagnosis was based upon symptoms of chronic meningitis associated with hyperesthesia and presence of eosinophilia in the cerebrospinal fluid. With increasing mobility rare infections like eosinophilic meningitis due to A. cantonensis may be encountered in the western hemisphere. A Giemsa stain of th…

Idiopathic sterile inflammation of the epidural fat and epaxial muscles causing paraplegia in a mixed-breed dog.

CASE DESCRIPTION: A 4-year-old sexually intact male mixed-breed dog was evaluated because of clinical signs of acute-onset pelvic limb ataxia, rapidly progressing to paraplegia with severe spinal hyperesthesia. CLINICAL FINDINGS: General physical examination revealed pyrexia, tachycardia, and tachypnea. Neurologic examination demonstrated severe spinal hyperesthesia and paraplegia with decreased nociception. Magnetic resonance imaging revealed extradural spinal cord compression at T13-L1 and hyperintense lesions on T1- and T2-weighted images in the epaxial musculature and epidural space. TRE…

A lumbosacral ventral root avulsion injury and repair model for studies of neuropathic pain in rats.

Neuropathic pain may develop after a variety of injuries to peripheral nerves and roots. Most injury models have included a direct injury to primary afferent fibers or neurons. Recently, it has been demonstrated that injury to motor fibers in ventral roots may also result in neuropathic pain. A lumbosacral ventral root avulsion injury results in acute and persistent mechanical allodynia, but not thermal hyperesthesia. Interestingly, an acute replantation of the avulsed ventral roots into the spinal cord results in amelioration of the neuropathic pain. A detailed description of this injury and …

Topographical features of cutaneous tactile hypoesthetic and hyperesthetic abnormalities in chronic pain.

Tactile sensory abnormalities, such as tactile hypoesthesia and mechanical allodynia, are frequently present in patients with chronic pain. A growing body of evidence indicates that hyperesthetic phenomena, like mechanical allodynia, are at least in part due to altered processing by neurons in the CNS. We propose that the hyperesthesia is associated with a functional tactile hypoesthesia that is similarly mediated by altered processing by CNS neurons, and that this association is characterized by a particular topography that may be related to the receptive field organization of somatosensory C…

Correlation between severity of clinical signs and transcranial magnetic motor evoked potentials in dogs with intervertebral disc herniation.

Transcranial magnetic motor evoked potentials (TMMEPs) can assess the functional integrity of the spinal cord descending motor pathways. In … unction. TMMEPs were recorded in 50 dogs with thoracolumbar IVDH. Clinical signs ranged from spinal hyperesthesia to non-ambulatory paraparesis in 19 dogs and paraplegia with/without deep pain sensation in 31 dogs. In these 31 paraplegic dogs, transcranial magnetic stimulation (TMS) was repeated during follow-up examinations. Ten healthy Beagle dogs served as c …

IMAGING DIAGNOSIS-IMAGING AND HISTOPATHOLOGIC CHARACTERISTICS OF A VERTEBRAL HAMARTOMA IN A CAT.

A 9-month-old domestic shorthair cat had progressive ambulatory paraparesis, proprioceptive ataxia, and thoracolumbar hyperesthesia. An extradural mass affecting the left pedicle and lamina of the second lumbar vertebra (L2) causing marked spinal cord impingement was identified in magnetic resonance (MR) images. The mass was predominantly calcified in computed tomographic (CT) images. A hemilaminectomy was performed to resect the mass. Clinical signs were greatly improved at 12-month follow-up. The histopathologic diagnosis was vascular hamartoma. To our knowledge, this is the first report des…

Sensory disturbances after two-portal endoscopic carpal tunnel release: a preliminary report.

Very few complications following endoscopic carpal tunnel release have been reported. We studied the incidence and pattern of sensory disturbances in 53 consecutive cases in which the Chow two-portal technique was used. Persistent sensory disturbances for 4 months or more were noted in six hands. Sensory tests were normal or near normal in all except one case. The disturbance was described by the patients as an uncomfortable tingling sensation or hyperesthesia, and in all cases the ring finger, especially the radial half, was involved. We wish to direct attention to this potential complication…

Acquired hydrocephalus and hydromyelia in a cat with feline infectious peritonitis: A case report and brief review.

A one-year-old domestic long-haired cat was referred to the New York State College of Veterinary Medicine because of acute onset of paraparesis and hyperesthesia associated with trauma. Myelography and cerebrospinal fluid analysis revealed severe hydromyelia and myelitis, respectively. The definitive diagnosis of feline infectious peritonitis was made by histological examination at necropsy. Lesions were confined exclusively to the brain and spinal cord. Partial occlusion of the third and fourth ventricles with pyogranulomatous debris caused hydrocephalus and subsequent hydromyelia. The hydrom…
