["Hyperferritinemia-cataract syndrome." Description of a new hereditary disease, from anamnesis to molecular diagnosis].

A new autosomal-dominant genetic disorder, which has been recently identified by our group is described. The disease is clinically characterized by the combination of a substantial increase of serum ferritin and early-onset bilateral cataract. Moreover, it is clearly distinguishable from genetic hemochromatosis because of: 1) normal to low serum iron and transferrin saturation, without evidence of parenchymal iron overload; 2) the dominant transmission; 3) the lack of any relation with HLA; 4) the rapid development of iron-deficient anemia when unnecessary phlebotomies are performed. The molec

Adult-onset Still's Disease Presenting with Prolonged Fever, Polyarthralgia, and Hyperferritinemia: A Case Report.

Adult-onset still's disease (AOSD) is a rare systemic inflammatory disorder characterized by quotidian fever, inflammatory arthritis, rash, and markedly elevated serum ferritin levels. Due to its nonspecific presentation and the need to exclude infectious, autoimmune, and malignant etiologies, diagnosis remains challenging. We report a case of a middle-aged patient presenting with prolonged high-grade fever, diffuse abdominal pain, polyarthralgia, and generalized pruritus. Extensive investigations ruled out infectious and autoimmune causes. Markedly elevated inflammatory markers and hyperferri

Lactoferrin efficacy in treating hyperferritinemia in patients suffering from pathologies unrelated to hereditary hemochromatosis.

Ferritin (Ftn), a globular protein, sequesters 4500 atoms of iron per molecule. Elevated serum Ftn levels (hyperferritinemia) is an indicator of iron homeostasis disorders. We present the results of an observational study involving 17 patients with hyperferritinemia unrelated to hereditary hemochromatosis (HH). All participants received treatment with 200 mg of bovine lactoferrin (bLf) once (n = 14) or twice (n = 3) a day before meals. The patients, treated with 200 mg/day of bLf, exhibited a significant increase in red blood cells (+10%, p < 0.001), hemoglobin (+4%, p < 0.001), and hematocrit

Factors associated with pre-treatment hyperferritinemia in patients with chronic hepatitis C virus infection.

Pre-treatment host and viral factors may affect serum ferritin levels in patients with hepatitis C virus (HCV) infection. We delineated pre-treatment factors associated with hyperferritinemia in these patients. 1682 eligible patients underwent pre-treatment assessment for serum ferritin and various host/viral factors. Univariate and multivariate logistic regression analyses were conducted to evaluate factors associated with hyperferritinemia. Multivariate logistic regression analyses revealed that age > 50 years (adjusted odds ratio [OR]: 1.38 (95% confidence interval [CI] 1.09-1.74), p = 0.00

Beyond Hyperferritinemia: Evaluating Ferritin as a Predictor of Advanced Therapy in Adult-Onset Still's Disease.

Background: Ferritin is a hallmark biomarker of systemic inflammation in adult-onset Still's disease (AOSD), but its potential role in guiding escalation to advanced therapy has not been established. This study aimed to evaluate whether ferritin, alone or in combination with simple clinical variables, could predict the need for advanced therapy in AOSD. Methods: A retrospective review was conducted of 113 patients with AOSD fulfilling the Yamaguchi criteria at Chonnam National University Hospital. Baseline demographic, clinical, and laboratory data were collected at the index episode. The prim

Recurrent mutations in the iron regulatory element of L-ferritin in hereditary hyperferritinemia-cataract syndrome.

BACKGROUND AND OBJECTIVE: Hereditary hyperferritinemia-cataract syndrome (HHCS) is an autosomal dominant disorder characterized by bilateral cataracts and increased serum and tissue L-ferritin, in the absence of iron overload. The deregulation of ferritin production is caused by heterogeneous mutations in the iron regulatory element (IRE) of L-ferritin that interfere with the binding of iron regulatory proteins. DESIGN AND METHODS: We have identified several patients from three unrelated Italian families with HHCS. Iron parameters were assessed by standard methods. The IRE element of L-ferrit

Hepatic tuberculosis presenting with extreme hyperferritinemia masquerading as adult-onset Still's disease: a case report.

INTRODUCTION: Isolated hepatic tuberculosis is an uncommon manifestation of one of the most common infections worldwide, caused by Mycobacterium tuberculosis. Extremely high serum ferritin, which is regarded as a marker of adult onset Still's disease, has not been observed in patients with tuberculosis of the liver. We report a case of hepatic tuberculosis who presented with clinical criteria of adult-onset Still's disease and extreme hyperferritinemia, which posed a diagnostic confusion. CASE PRESENTATION: Our patient was a 48-year-old Sri Lankan man who presented with fever, polyarthralgia

Prevalence and Characteristics of Hyperferritinemia in Metabolic Dysfunction-Associated Steatotic Liver Disease: A Meta-Analysis.

BACKGROUND & AIMS: Hyperferritinemia (HF), defined as elevated serum ferritin (SF) levels, can reflect iron stores and chronic inflammation. However, the prevalence and clinical features of HF in metabolic dysfunction-associated steatotic liver disease (MASLD) have not been systematically evaluated. This systematic review and meta-analysis aimed to examine this topic. METHODS: PubMed and Embase were searched from inception to December 2, 2024 for observational studies reporting the prevalence and/or impact of HF (under any definition) in MASLD diagnosed by liver biopsy, imaging, or biomarkers

Early hyperferritinemia in polytrauma: a biomarker of systemic inflammation and mortality risk in a multicenter cohort study.

BACKGROUND: Severe polytrauma is one of the leading causes of death and disability worldwide. Hyperferritinemia is increasingly recognized as a biomarker of critical illness, yet its prognostic significance in polytrauma remains underexplored. This multicenter study investigates the temporal dynamics of serum ferritin in polytrauma patients and its association with systemic inflammation, organ dysfunction, and mortality. METHODS: A prospective observational cohort of 1475 polytrauma patients from four trauma centers was analyzed. Serum ferritin levels were measured on admission day 1 and day

Conservative Management of Hyperferritinemia in Hemolytic Disease of the Fetus and Newborn: A Case Report and Review of the Literature.

We report a newborn with hemolytic disease of the fetus and newborn (HDFN) with rapid resolution of extreme hyperferritinemia without chelation. An infant born at 35+3 weeks with HDFN and a history of 3 intrauterine transfusions developed severe hyperferritinemia (maximum, 8258 mcg/L) without evidence of toxic iron deposition on liver biopsy. Her hyperferritinemia was managed with observation alone, and ferritin levels normalized rapidly. This case supports observation as being the preferred alternative to chelation therapy for significant hyperferritinemia in newborns with HDFN in the absence

Hyperferritinemia in neonatal and infantile human parechovirus-3 infection in comparison with other infectious diseases.

Human parechovirus-3 (HPeV-3) has been associated with severe clinical manifestations in neonates and infants in the form of sepsis or hemophagocytic lymphohistiocytosis (HLH)-like illness. To clarify the clinical features of HPeV-3 infection, we compared clinical signs and laboratory findings among enteroviruses (EVs), HPeV-3, and other infections. Participants were 26 febrile infants in whom EVs (n = 20) or HPeV-3 (n = 6) were isolated from throat swab or fecal specimens. Clinical and laboratory data were compared among EVs, HPeV-3, respiratory syncytial virus (RSV) infection (n = 15), and b

Analysis of ferritins in lymphoblastoid cell lines and in the lens of subjects with hereditary hyperferritinemia-cataract syndrome.

Hereditary hyperferritinemia-cataract syndrome (HHCS) is an autosomal and dominant disease caused by heterogeneous mutations in the iron responsive element (IRE) of the 5' untranslated flanking region of ferritin L-chain mRNA, which reduce the binding to the trans iron regulatory proteins and make L-chain synthesis constitutively upregulated. In the several families identified so far, the serum and tissue L-ferritin levels are fivefold to 20-fold higher than in nonaffected control subjects, iron metabolism is apparently normal, and the only relevant clinical symptom is early onset, bilateral c

Clinical and Molecular Clues to Diagnosing Hereditary Hyperferritinemia-Cataract Syndrome: Case Report and Literature Review.

Background: Hereditary hyperferritinemia-cataract syndrome (HHCS) is a rare autosomal dominant disorder characterized by persistently elevated serum ferritin and early-onset bilateral cataracts in the absence of systemic iron overload. It is caused by pathogenic variants in the iron-responsive element (IRE) of the FTL gene, leading to dysregulated L-ferritin synthesis. Methods: We evaluated a 12-year-old Czech girl with markedly elevated serum ferritin identified incidentally during workup for abdominal pain. Clinical assessment included biochemical, radiological, ophthalmological, and genetic

Hyperferritinemia and Macrophage Activation Syndrome in Septic Shock: Recent Advances with a Pediatric Focus (2020-2025).

Macrophage activation syndrome (MAS), a hyperinflammatory condition driven by uncontrolled immune activation, is widely recognized as a critical complication in pediatric septic shock. This syndrome shares pathophysiological features with hemophagocytic lymphohistiocytosis (HLH) and other cytokine storm syndromes, and it contributes to significant morbidity and mortality in pediatric and adult patients. Hyperferritinemia-a hallmark of MAS-is not only a diagnostic clue but also a prognostic marker for poor outcomes in sepsis. High ferritin levels are strongly suggestive of MAS, yet even moderat

Adult-Onset Still's Disease With Severe Hyperferritinemia and the Asian Salmon-Pink Rash: A Case Report.

Adult-onset Still's disease (AOSD) is a systemic inflammatory disease characterized by rash, arthritis, and persistent spiking fever. The diagnosis and treatment of AOSD are challenging due to the lack of specific diagnostic criteria and little evidence of effective treatments. Here, we reported a case of an 18-year-old woman with a fever of unknown origin (FUO), evanescent rash (without the typical "salmon-pink" color), and systemic lymphadenopathy. Laboratory tests at hospital admission revealed marked hyperferritinemia of 12,100 ng/mL. AOSD was subsequently suspected. Additional anti-nuclea

[Value of hyperferritinemia and glycosylated ferritin in the diagnosis of adult-onset Still's disease. 3 case reports].

INTRODUCTION: The diagnosis of adult-onset Still's disease currently requires application of Yamaguchi's criteria and then exclusion of infectious, hematologic, autoimmune and neoplastic disorders. A recent article suggests that very high levels of serum ferritin together with an unusually low percentage of glycosylated ferritin may serve as diagnostic criteria or even as markers of disease progression. This new score has greater specificity than the other scores, including that of Yamaguchi. CASES: We describe 3 new cases with very high ferritin levels (exceeding 9000 microg/L) and glycosyla
