Are hypertonic hyperoncotic solutions safe for prehospital small-volume resuscitation? Results of a prospective observational study.

RATIONALE: This prospective observational study was designed to shed light on the routine use of a hypertonic hyperoncotic solution for prehospital small-volume resuscitation in trauma patients. Effects on homeostasis and haemodynamics as well as safety were evaluated. METHODS: One hundred patients with haemorrhagic shock were given hypertonic hyperoncotic solution (6% hydroxyethyl starch plus 7.5% sodium chloride) by bolus infusion (approximately 4 ml/kg bodyweight). Blood was taken before small-volume resuscitation and after the patients' arrival in the emergency room and serum electrolytes

Cardiovascular effects of hypertonic sodium chloride solutions when injected into the liquor space of anaesthetized cats.

1 In cats anaesthetized with chloralose, hyper- and hypotonic solutions were injected into the cisterna magna (in 0.5 ml) or into a lateral cerebral ventricle (in 0.2 to 0.3 ml), with aqueduct cannulated to prevent the injected solution from entering the subarachnoid space, and the effects on blood pressure and heart rate were examined. 2 Cisternal injections of hyper- and hypotonic solutions of NaCl (0.51 M and 0.05 M), glucose (1.03 M and 0.10 M), or sucrose (1.02 M and 0.10 M), as well as distilled water produced a rise in arterial blood pressure with tachycardias. Isotonic solutions of NaC

Cardiovascular and pulmonary effects of hetastarch plus hypertonic saline solutions during experimental endotoxemia in anesthetized horses.

BACKGROUND: Small volume resuscitation has been advocated as a beneficial therapy for endotoxemia in horses but this therapy has not been investigated in a prospective manner. The objective of this study was to determine the cardiopulmonary effects of small-volume resuscitation using hypertonic saline solution (HSS) plus Hetastarch (HES) during experimental endotoxemia in anesthetized horses. HYPOTHESIS: Treatment of horses with induced endotoxemia using HES-HSS does not alter the response of various cardiopulmonary indices when compared to treatment with either small- or large-volume isotoni

The dynamics of vascular volume and fluid shifts of lactated Ringer's solution and hypertonic-saline-dextran solutions infused in normovolemic sheep.

UNLABELLED: Infusions of hyperosmotic-hyperoncotic solutions such as hypertonic saline dextran (HSD) are used in Europe for resuscitation of traumatic shock and perioperative volume support as an adjunct to conventional isotonic crystalloids. Whereas plasma volume expansion of HSD has been measured at single time points after the intravascular volume expansion, the detailed time course of fluid shifts during and after infusions have not been reported. We compared the time course of volume expansion during and after 30-min infusions of 4 mL/kg HSD and 25 mL/kg lactated Ringer's solution (LR) in

[Haemodynamic effects following preoperative hypervolemic haemodilution with hypertonic hyperoncotic colloid solutions in coronary artery bypass graft surgery].

OBJECTIVE: Using hyperoncotic colloids as volume replacement to provide haemodynamic stability appears to be a suitable approach to diminish fluid overload and subsequent interstitial edema during cardiac surgery. The aim of the present study was to investigate for the first time the haemodynamic effects following preoperative haemodilution with different hypertonic hyperoncotic colloid solutions in patients undergoing coronary artery bypass grafting. METHODS: A total of 43 patients with normal left ventricular ejection fraction, undergoing elective coronary artery bypass grafting received pr

An Audit and Comparison of pH, Measured Concentration, and Particulate Matter in Mannitol and Hypertonic Saline Solutions.

Background: The preferred hyperosmolar therapy remains controversial. Differences in physical properties such as pH and osmolality may be important considerations in hyperosmolar agent selection. We aimed to characterize important physical properties of commercially available hyperosmolar solutions. Methods: We measured pH and concentration in 37 commonly-used hyperosmolar solutions, including 20 and 25% mannitol and 3, 5, 14.6, and 23.4% hypertonic saline. pH was determined digitally and with litmus paper. Concentration was determined by freezing point and vapor pressure osmometry. Salinity/s

Effects of a hypertonic saline solution and dextran 70 combination in the treatment of diarrhoeic dehydrated calves.

Thirty dehydrated diarrheic calves aged 2-45 days were used to study the effects of small volume intravenous 7.2% hypertonic saline and 6% dextran 70 solution in combination with alkalinising oral fluids and to compare this therapy with the large volume intravenous isotonic saline with alkalinising oral fluid treatment. This study included a total of 30 diarrhoeic dehydrated calves aged 2-45 days. After routine clinical and haematological examinations, the calves were allotted to two treatment groups. Isotonic saline solution (32 ml/kg, i.v.) with an oral electrolyte solution (22 ml/kg, p.o.)

Small volume resuscitation with 7.5% hypertonic saline solution--treatment of haemorrhagic shock in the tropics.

Rapid intravenous injection of 4 mL/kg body weight of a 7.5% hypertonic sodium chloride solution immediately increases intravascular osmotic pressure and intravascular volume after haemorrhage. This 'small volume resuscitation' rapidly improves blood pressure and microcirculatory perfusion in patients with hypovolaemic shock after large blood losses. Pathophysiological findings as well as practical application approaches are described. Small volume resuscitation is an effective and economic method in the first-line treatment of acute haemorrhagic shock.

Body fluid changes during hypertonic lactated saline solution therapy for burn shock.

The body fluid changes of 12 burn patients treated with Hypertonic Lactate Saline solution (HLS group) were compared with 26 burn patients receiving isotonic therapy (iso Na group). 1) Total infusion volume during the first 48 hours postburn in the HLS group was only one-half to two-thirds of that in the iso Na group, although the sodium loads were nearly equal. 2) HLS therapy maintained sufficient functional extracellular fluid volume (f-ECFV) in the shock period and protected the excess increase of f-ECFV in the postresuscitative period, in contradistinction to the iso Na group. Addition of hyaluronic acid improves tolerance to 7% hypertonic saline solution in bronchiectasis patients.

BACKGROUND: The excessive retention of sputum in the airways, leading to pulmonary infections, is a common consequence of bronchiectasis. Although inhalation of 7% hypertonic saline (HS) has proven an effective method to help remove the mucus, many patients are intolerant of this treatment. The addition of 0.1% hyaluronic acid to HS (HS+HA) could increase tolerance to HS in these patients. The main objective of this study was to evaluate the tolerability of HS+HA in bronchiectasis patients who are intolerant to HS. METHODS: This prospective, observational, open-label study analysed the outcom

Prognostic factors to predict outcome following the administration of hypertonic/hyperoncotic solution in hypovolemic patients.

Hypertonic solutions effectively improve hemodynamic parameters in patients admitted to the emergency room. However, no significant differences in outcome were observed compared with standard isotonic treatment in most previously published studies. This study evaluates pretreatment prognostic factors that predict a beneficial effect of hypertonic solution in patients admitted to the emergency room with hemorrhagic hypovolemia in a prospective double-blind fashion. The patients (n = 212) were randomized upon admission to receive 250 mL intravenous (i.v.) bolus of hypertonic 7.5% NaCl + 6% dextr
