Mixed emotional and physical symptoms in general practice: what diagnoses do GPs use to describe them?

AIMS: To determine what diagnostic terms are utilized by general practitioners (GPs) when seeing patients with mixed emotional and physical symptoms. METHOD: Prototype cases of depression, anxiety, hypochondriasis, somatization and undifferentiated somatoform disorders were sourced from the psychiatric literature and the author's clinical practice. These were presented, in paper form, to a sample of GPs and GP registrars who were asked to provide a written diagnosis. RESULTS: Fifty-two questionnaires were returned (30% response rate). The depression and anxiety cases were identified correctl

Can DSM-V facilitate productive research into the somatoform disorders?

OBJECTIVE: To highlight key research issues relevant to the diagnostic classification of somatoform disorders and assess whether DSM-V can facilitate research. METHODS: A selective review of recent research findings in relation to the diagnostic categories of somatization disorder and hypochondriasis. RESULTS: The DSM-IV criteria have been ignored by researchers as they were either too restrictive or too broad to study the associated features of somatization and hypochondriasis. In view of the linear relationship between number of bodily symptoms and impairment, the appropriate cutoff point

HYPOCHONDRIA AS A DISTRACTION OF THE REASON WITHIN KANT'S TRANSCENDENTAL PHILOSOPHY.

The primary aim of this paper will be to understand hypochondria as a philosophical problem, rather than a medical-psychological one, since there is a significant difference between the modern (medical-psychological in the true sense of the word) understanding of hypochondria and all earlier anthropological, philosophical, and physiological understandings, including that of Kant. Kant's interpretation of hypochondria, as a mental illness, highlights an interesting dichotomy between actual bodily sensations and distorted perception. The hypochondriac experiences real physical sensations but int

Comorbidity and Suicidality in Patients Diagnosed with Panic Disorder/Agoraphobia and Major Depression.

BACKGROUND: Comorbidity of anxiety and depression (both current and lifetime) is associated with greater chronicity and an increased risk of suicidality. We wished to ascertain which symptom clusters had the strongest association with suicidality. Our aims were (1) to examine the presence of current comorbidity and suicidality in patients diagnosed with panic disorder/agoraphobia (PD/A) and major depression (MD), and their relationship with duration of psychiatric treatment and frequency of hospital admission; and (2) to examine which coexisting symptoms were most strongly predictive of suicid

Respiratory symptoms and mental disorders among youth: results from a prospective, longitudinal study.

OBJECTIVE: To determine the relationship between respiratory symptoms and mental disorders among youth in the community, and to investigate possible mechanisms of these linkages. METHODS: Data were drawn from the Oregon Adolescent Depression Project (n = 1,709), a longitudinal study of adolescents in the community. Multiple logistic regression analyses were used to examine the cross-sectional and longitudinal associations between respiratory symptoms and mental disorders at baseline, and linkages between respiratory symptoms at baseline and the onset of specific mental disorders at follow-up.

Impact of Cyberchondria on Health and Quality of Life: Scoping Review.

BACKGROUND: Cyberchondria is often associated with psychological distress, straining doctor-patient relationships, and financial burdens. Over the past few decades, increasing research has explored its associations with quality of life (QoL). However, existing reviews have not comprehensively synthesized or narratively analyzed these connections. OBJECTIVE: This study aims to consolidate current research, identify key trends, and examine how cyberchondria affects QoL, while providing insights for future research directions. METHODS: The literature search was conducted on 4 databases PsycINFO

Lumbar disc herniations: the predictive value of the Health Attribution Test (HAT) and the Minnesota Multiphasic Personality Inventory (MMPI).

Ninety-one patients who were treated for lumbar disc herniation with chymopapain chemonucleolysis were evaluated preoperatively by means of the Health Attribution Test (HAT) and the Minnesota Multiphasic Personality Inventory (MMPI). There were 54 good, 10 fair, and 27 poor results after chemo-nucleolysis. Nineteen patients subsequently underwent lumbar laminectomy and discectomy and the ultimate outcome for the entire series including these laminectomy patients was 66 good, 10 fair, and 15 poor results. The fair/poor chemonucleolysis outcome patients scored significantly lower than did the go

Correlates of the tourniquet ischemia pain ratio.

The Tourniquet Ischemia Pain Ratio was developed as a measure of pain. The present study examined the relationship between the ratio and other measures thought to be relevant to a patient's perception of, and reaction to, chronic pain. The ratio was correlated with the patient's estimate of the pain, the perceived impact of the pain on daily life, the degree of reactive depression, and a measure of hypochondriasis. The ratio relates to both pain and hypochondriacal tendencies.

Psychological characteristics and associations between living kidney transplantation recipients and biologically related or unrelated donors.

BACKGROUND: Although recipients and donors in living kidney transplantation experience psychological distress-including depression and anxiety-during the pre-operative period, very few studies have evaluated the related psychological reactions. This study aimed to determine the characteristics and correlations of the mood states and personality of recipients and donors (genetically related and unrelated) of living kidney transplantations. METHODS: A total of 66 pairs of living donors and recipients were enrolled from April 2008 to June 2019 in this study, of whom 53 eligible pairs of living d

Correlates of worry and functional somatic symptoms in generalized anxiety disorder.

BACKGROUND: Worry and functional somatic symptoms (FSS) are important clinical features of generalized anxiety disorder (GAD). Although there is literature on the prevalence of worry and FSS in GAD, there is limited data on psychological correlates of worry and somatic symptoms in patients with GAD. OBJECTIVE: The purpose of this research was to evaluate the relationship of worry and FSS with somatosensory amplification, health anxiety (hypochondriasis), and alexithymia in patients with GAD. METHODS: Forty patients with the diagnosis of GAD were assessed with Penn State Worry Questionnaire,

The relationship of obsessive-compulsive disorder to possible spectrum disorders: results from a family study.

BACKGROUND: The familial relationship between obsessive-compulsive disorder (OCD) and "obsessive-compulsive spectrum" disorders is unclear. This study investigates the relationship of OCD to somatoform disorders (body dysmorphic disorder [BDD] and hypochondriasis), eating disorders (e.g., anorexia nervosa and bulimia nervosa), pathologic "grooming" conditions (e.g., nail biting, skin picking, trichotillomania), and other impulse control disorders (e.g., kleptomania, pathologic gambling, pyromania) using blinded family study methodology. METHODS: Eighty case and 73 control probands, as well as

Measurement and interrelations of psychiatric symptomatology in inpatients.

For 152 psychiatric inpatients scores on the Beck Depression Scale, State form of the State-Trait Anxiety Inventory, the Self-report Inventory, Hopelessness Scale and 3 MMPI scales, Hypochondriasis, Schizophrenia, and Hypomania, were factor analyzed. The two factors appeared to confirm Gotlib's 1984 suggestion that such questionnaires measure general distress, as responding endorses negative affect.

A critique of cognitive therapy for anxiety disorders.

Progress in the cognitive-behavioral treatment of anxiety disorders is reviewed. Significant advances have been made in treating panic disorders and there are promising signs of an expansion of cognitive theory and therapy to other disorders, notably hypochondriasis, obsessional disorders and circumscribed phobias. Nevertheless, some difficult obstacles have emerged to present serious problems for the prevailing cognitive theory.

Changes in the Mean of and Variance in Psychological Disease Incidences before and during the COVID-19 Pandemic in the Korean Adult Population.

The coronavirus disease 2019 (COVID-19) pandemic has been suggested to increase the risk of depression and anxiety disorders. This study expanded upon previous findings by estimating the changes in medical visits for various psychological disorders during the COVID-19 pandemic compared to before COVID-19. The entire Korean population ≥ 20 years old (~42.3 million) was included. The first COVID-19 case in Korea was reported on 20 January 2020. Thus, the period from January 2018 through to February 2020 was classified as “before COVID-19”, and the period from March 2020 through to May 2021 was c

Personality traits in chronic daily headache patients with and without psychiatric comorbidity: an observational study in a tertiary care headache center.

BACKGROUND: Previous studies suggest that patients with Chronic Daily Headache (CDH) have higher levels of anxiety and depressive disorders than patients with episodic migraine or tension-type headache. However, no study has considered the presence of psychiatric comorbidity in the analysis of personality traits. The aim of this study is to investigate the prevalence of psychiatric comorbidity and specific personality traits in CDH patients, exploring if specific personality traits are associated to headache itself or to the psychiatric comorbidity associated with headache. METHODS: An observ
