[VRS bronchiolitis with severe hyponatremia leading to acute seizures: a report of three cases].

Acute bronchiolitis due to respiratory syncytial virus (RSV) can present with extrapulmonary manifestations, notably severe hyponatremia. Hyponatremia is caused by excess secretion of antidiuretic hormone and can be exacerbated by intravenous infusion of hypotonic solutions. We report three cases of infants admitted for acute bronchiolitis and hyponatremia leading to acute seizures. We describe how hyponatremia was corrected and analyze the management aspects that might have worsened the magnitude of hyponatremia. We underline the basic principles of water and electrolyte management of bronchi

Topical Use of NaCl Solution with Different Concentration Affects Lens Transparency in Anesthetized Mice.

PURPOSE: To study the influence of NaCl solution with different concentration on lens transparency in anesthetized mice. METHODS: Four kinds of NaCl solution with different concentration were prepared as eye drops to imply graded osmolarity (100, 300, 500 and 1000 mOsmol/kg). Five groups of anesthetized mice were set-up to induce lens opacity, in which four groups were treated with NaCl solution and another group naturally exposed to air. The lens opacity was graded as no opacity, mild, medium and severe opacity at 0, 10, 20, 30, 45 and 60 min after the start of the experiment. A numerical va

Pharmacotherapy of sodium disorders in neurocritical care.

PURPOSE OF REVIEW: To describe the pathophysiology and pharmacotherapy of dysnatremia in neurocritical care patients. RECENT FINDINGS: Sodium disorders may affect approximately half of the neurocritical care patients and are associated with worse neurological outcome and increased risk of death. Pharmacotherapy of sodium disorders in neurocritical care patients may be challenging and is guided by a careful investigation of water and sodium balance. SUMMARY: In case of hyponatremia, because of excessive loss of sodium, fluid challenge with isotonic solution, associated with salt intake is the

The GEN.S: a fortuitous finding of a routine screening test for hereditary spherocytosis.

As part of the evaluation of the GEN.S (Coulter), we compared the Mean Corpuscular Volume (MCV) to the Mean Spherized Corpuscular Volume (MSCV) assessed during the reticulocyte count procedure under hypo-osmotic conditions. A sub-group of patients with hereditary spherocytosis (HS) was singled out: in all of them, the MSCV became smaller than the MCV. As the cell volume normally increases in red cells derived from other patients in the same conditions, we decided to further study the reason for this particular behaviour of HS red cells. Whereas normal red cells are able to undergo an osmotic e

Effect of coupling fluid tonicity on pain and fragmentation outcomes in ESWL: a prospective comparative study.

To investigate the effects of coupling fluid tonicity on pain, stone fragmentation success and complications in extracorporeal shock wave lithotripsy (ESWL). This prospective, controlled clinical trial included 294 patients with renal pelvis stones (5-20 mm) allocated to three groups: hypotonic degassed water (n = 97), isotonic 0.9% NaCl (n = 99) and hypertonic 3% NaCl (n = 98). Primary endpoints were post-ESWL Visual Analogue Scale (VAS) pain scores and stone fragmentation success (< 4 mm residual fragments) on follow-up non-contrast computed tomography at 2-4 weeks. Baseline demographic and

Sites of resistance changes with inhibition of acid secretion in frog stomach.

The primary purpose of this study was to determine the sites of resistance changes from thiocyanate inhibition of secretion to better evaluate models of the gastric proton pump. The potential difference, resistance, and H+ secretory rate were measured with various tonicities of the secretory fluid. Thiocyanate (SCN) inhibition generally produces a resistance increase, but with nutrient Ba, the resistance is high and SCN decreases it. The latter effect is also seen with cimetidine. In the secreting fundus a hypotonic secretory solution has a small effect on resistance, but in the inhibited fund

[Vascular loading in the first 24 hours following severe head injuries].

The main goal at the acute phase of head injury is to prevent a decrease in blood pressure, which promotes cerebral ischemia. Volume loading is therefore frequently indicated. A normal or increased plasma osmolarity should be maintained. Thus hypotonic fluids should be avoided. Hyperglycaemia is also a risk factor for brain injury and glucose use has to be restricted in the first hours after trauma. Isotonic saline 0.9% is the first solution to be infused. Lactated Ringer solutions are mildly hypotonic as approximately 114 mL of free water is contained in each litre of the solution. Isotonic c

Optimal fluid management after aortic reconstruction: a prospective study of two crystalloid solutions.

To determine optimal fluid management after elective aortic surgery we compared postoperative administration of 5% dextrose Ringer's lactate solution (102 patients) with 5% dextrose half-normal saline solution (80 patients). For 72 hours after operation, intravenous fluids were titrated to maintain urine output between 50 and 100 ml/hr. The group receiving 5% dextrose Ringer's lactate required less intravenous volume per day (2005 +/- 138 ml [mean +/- standard error of the mean] vs. 2701 +/- 145 ml, p less than 0.05), gained less weight (0.8 +/- 0.2 kg vs. 3.2 +/- 0.2 kg, p less than 0.05), an

Expression of K+-Cl- cotransporters in the alpha-cells of rat endocrine pancreas.

The expression of K+-Cl- cotransporters (KCC) was examined in pancreatic islet cells. mRNA for KCC1, KCC3a, KCC3b and KCC4 were identified by RT-PCR in islets isolated from rat pancreas. In immunocytochemical studies, an antibody specific for KCC1 and KCC4 revealed the expression of KCC protein in alpha-cells, but not pancreatic beta-cells nor delta-cells. A second antibody which does not discriminate among KCC isoforms identified KCC expression in both alpha-cell and beta-cells. Exposure of isolated alpha-cells to hypotonic solutions caused cell swelling was followed by a regulatory volume de

Intrarenal distribution of blood flow and cortico-medullary sodium gradient after unilateral splanchnicotomy in the dog.

1. Intrarenal distribution of blood flow and cortico-medullary sodium gradients have been studied in anaesthetized dogs, previously subjected to unilateral splanchnicotomy. Experiments were carried out on hydropoenic, normal, isotonic and hypotonic volume expanded animals.2. Polyuria and natriuresis were present under all experimental conditions after denervation.3. Denervation resulted in an increase in medullary blood flow, particularly in hydropoenic and normal dogs. In isotonic and hypotonic volume expansion, the increases in the medullary circulation were smaller despite a greater increas

pH-dependent effects of chlorpromazine on liposomes and erythrocyte membranes.

Chlorpromazine (CPZ) is an amphipathic antipsychotic drug that binds to erythrocytes reaching in this way the central nervous system. CPZ is a basic molecule with pK=8.6. This paper reports on CPZ-induced lysis of red blood cells and liposomes. Haemolysis was tested under hypotonic conditions, in the pH range 5.0-10.0. Cell sensitivity towards CPZ increased with increasing pH. Increasing pH caused also a decrease in the critical micellar concentrations of CPZ. These results are interpreted in terms of a competition between repulsive electrostatic forces and attractive hydrophobic forces, that

Intravascular haemolysis complicating treated non-ketotic hyperglycaemic diabetic coma.

A patient with non-ketotic hyperglycaemic diabetic coma is reported with the complications of cerebral oedema, oliguric renal failure, transient cataracts, possible acute pancreatitis and intravascular haemolysis. The last association has not been reported before, and it may have been due to rapid infusion of hypotonic saline producing haemolysis locally at the infusion site.

Volume-sensitive chloride current in pigmented ciliary epithelial cells: role of phospholipases.

The whole cell recording technique was used to examine an outwardly rectifying chloride current activated by hypotonic shock in bovine pigmented ciliary epithelial (PCE) cells. Removal of internal and external Ca2+ did not affect the activation of these currents, but they were abolished by the phospholipase C inhibitor neomycin. The current was blocked by 5-nitro-2-(3-phenylpropylamino)benzoic acid, 4-acetamido-4'-isothiocyanostilbene-2,2'-disulfonic acid, and 4,4'-disothiocyanostilbene-2,2'-disulfonic acid (DIDS) in a voltage-dependent manner, but tamoxifen, dideoxyforskolin, and quinidine di

Perioperative crystalloid and colloid fluid management in children: where are we and how did we get here?

It has been more than 50 yr since the landmark article in which Holliday and Segar (Pediatrics 1957;19:823-32) proposed the rate and composition of parenteral maintenance fluids for hospitalized children. Much of our practice of fluid administration in the perioperative period is based on this article. The glucose, electrolyte, and intravascular volume requirements of the pediatric surgical patient may be quite different than the original population described, and consequently, use of traditional hypotonic fluids proposed by Holliday and Segar may cause complications, such as hyperglycemia and
