OBJECTIVE: We hypothesized that prolonged second stage of labor increases the incidence of unintentional hysterotomy extensions at cesarean delivery.
STUDY DESIGN: A retrospective cohort of term pregnant women who underwent primary cesarean delivery after failed second stage of labor at Stanford University was assessed for hysterotomy extensions and other maternal and neonatal morbidities. Groups included second stage length of 1-3 hours and >4 hours. Data were analyzed with the use of chi-square and Fisher's exact tests.
RESULTS: Of the 239 women who were studied, the second stage of labor

The terminology we employ influences our perceptions. Thus, we believe in reviewing terminology regularly. The term "cesarean section" should be abandoned. A medical procedure is rarely named after a historic figure, and almost never after an ancient Roman law. Cesarean section has recently been amended to cesarean birth; however, the term is still inadequate. The word "hysterotomy" more appropriately describes the procedure we regularly perform. The adjectives used to describe cesarean birth should also be reevaluated. If we are to keep the term "classical" for the original fundal vertical ut

This systematic review aims to analyze the case reports, case series, or clinical studies describing the women with cesarean scar ectopic pregnancy (CSEP), and thus, to determine the efficacy and safety of different primary treatment modalities in the management of CSEP. A thorough search of electronic databases showed that 274 articles on CSEP were published between January 1978 and April 2014. Systemic methotrexate, uterine artery embolization, dilatation and curettage (D&C), hysterotomy, and hysteroscopy were the most frequently adopted first-line approaches. The success rates of systemic m

A few fetal diseases may benefit from surgical treatment before birth, but hysterotomy and subsequent delivery by cesarean section pose a risk to the otherwise unaffected mother. To assess maternal risk of mortality, morbidity, and reproductive potential after fetal surgery, we reviewed our experience with 17 highly selected women who underwent fetal surgery. Fifteen of these procedures were performed for one of two congenital anomalies: severe bilateral hydronephrosis and congenital diaphragmatic hernia. There were no deaths or serious maternal injuries. In the 14 women who continued pregnanc

BACKGROUND/OBJECTIVES: Prenatal myelomeningocele (MMC) repair has been shown to improve neurological outcomes. It has been suggested that decreases in the hysterotomy diameter during surgery can improve perinatal outcomes without altering neurologic outcomes. The objective of this study is to describe and compare the main maternal and fetal outcomes of fetuses undergoing open surgery for MMC repair, through the different modifications (standard-classical, mini-hysterotomy, and microneurosurgery).

OBJECTIVE: To identify risk factors associated with treatment failure among women with cesarean scar pregnancy (CSP).
METHODS: In a retrospective study, the medical records of patients with CSP treated at National Taiwan University Hospital, Taipei, Taiwan, from 1994 to 2015 were reviewed. The women were managed primarily with hysterotomy, evacuation, or methotrexate. Receiver operating characteristic (ROC) curve analysis and logistic regression analysis were used to evaluate the factors associated with treatment failure.
RESULTS: Among 90 patients, 44 underwent hysterotomy, 18 underwent eva

BACKGROUND: Spina bifida (SB) is a severe congenital malformation that affects approximately 150,000 infants annually, predominantly in low- and middle-income countries, leading to significant morbidity and lifelong disabilities. In Latin America, the birth prevalence of SB is notably high, often exacerbated by limited healthcare resources and poor access to advanced medical care. The implementation of laparotomy-assisted fetoscopic in-utero SB repair programs in Latin America targets reducing prematurity rates and enabling vaginal births while preserving the benefits of decreased need for hyd

Preventing blood loss at the time of a cesarean delivery during a scheduled, nonemergent cesarean hysterectomy for placenta percreta may reduce the need for crystalloid and blood product transfusion. Commonly a classical hysterotomy is created and this can result in as much as a 500-800 mL blood loss before the hysterotomy is closed. Our technique involves placement of 4 full-thickness interrupted sutures in a box pattern to create an unperfused area of upper uterine segment. Diathermy is used to open the uterus to the membranes in the center of the "box" without blood loss. A finger is then i

We report three cases of complete inversion of the uterus following delivery. One became chronic and this gave rise to the need for surgical cure, which consisted in carrying out a midline hysterotomy longitudinal and sagital, without reaching down to the cervix. The two halves of the body of the uterus which resulted were reduced easily one after the other. The hysterotomy was then sewn up correctly.

Uterine torsion is a rare obstetrical complication whose diagnosis remains challenging. We report a case of 180 degrees dextrogyre torsion at 36(+5) weeks of gestation complicated by partial abruption and in utero fetal death. Emergency cesarean section was performed through an unintentional posterior hysterotomy. Literature reports a few similar cases. Vertical hysterotomy should be advised in this context avoiding incision on lateral sides associated with increased risk of vascular or ureteral injury.

A 14-year-old female black and white colobus monkey (Colobus guereza) presented in labor with fetal arms visible protruding from the vulva. Manual manipulation for assisted delivery of the fetus was unsuccessful. Radiographs identified a large fetal skull and hysterotomy was required with ovariohysterectomy elected to follow. The fetus was confirmed to be deceased during hysterotomy, but the dam recovered from the procedure uneventfully. The detailed description of the anesthesia and surgical procedure in this case may aid other clinicians when presented with similar dystocia cases in this spe

Maternal cardiac arrest is a catastrophic event, with data regarding outcomes and feasibility of timely resuscitative hysterotomy remaining scarce. Our retrospective cohort study identified 31 pregnant or postpartum patients with in-hospital cardiac arrest at a tertiary care center over 15.5 years, an incidence of 2 per 10,000 deliveries (95% CI, 1.4-2.8/10,000 deliveries). Return of spontaneous circulation (ROSC) occurred in 87.1% of the patients (95% CI, 75.2-98.8%) within a median of 2 minutes, and 77.4% (95% CI, 62.6-92.1%) survived until discharge. Eleven instances of cardiac arrest (35.5

BACKGROUND: Müllerian anomalies increase the potential for complications during pregnancy termination. We present an unusual complication of abortion in a patient with a bicornuate uterus.
CASE: We present a case of failed medical and surgical abortion in a woman with a bicornuate uterus. A hysteroscopy was performed, but the pregnant horn could not be entered due to a cervical septum. Laparotomy and hysterotomy were required to complete the abortion.
CONCLUSION: Standard approaches to surgical abortion can fail in cases of uterine anomaly, and surgical versatility is required. Laparotomy an

AIM: The aim of this study was to investigate the effect of variation of the incision-to-delivery interval on neonatal wellbeing during cesarean delivery.
METHODS: In this prospective study, 71 women, hemodynamically stable and without severe comorbidities, undergone an antepartum cesarean delivery; acid-base status, umbilical arterial oxygen content and Apgar score were used as indicators of neonatal wellbeing. Umbilical cord blood gas, the Apgar score of each neonate, the interval between skin incision and delivery and the interval between hysterotomy and fetal extraction of 71 cesarean del

INTRODUCTION: This article describes a case of asymptomatic uterine torsion in the setting of macrosomia and polyhydramnios. While other causes have been reported in the literature, there are no existing cases of uterine torsion associated with fetal macrosomia and polyhydramnios.
CASE: This patient had an asymptomatic uterine torsion discovered incidentally during a scheduled repeat cesarean section. A congested vascular lower uterine segment was noticed on entrance into the abdomen. After exteriorization of the uterus, 180° torsion was discovered. If torsion is identified prior to hysteroto

INTRODUCTION: Urinary tract injuries (UTI) are a frequent complication of morbidly adherent placenta (MAP) management. In this study, we aim to characterize the type of UTI that occurs and to define if their incidence varies after establishing a fixed interdisciplinary group for the protocolized management of patients with MAP.
METHODOLOGY: All patients with confirmed MAP attended between 2011 and 2019 in our institution, were included. We analyzed the effect of a change in the surgical protocol including rigid ureteral catheters, vesicouterine dissection before hysterotomy and interdisciplin

BACKGROUND: Fetal surgery is part of modern fetal medicine, and some procedures, such as fetal spina bifida repair, are performed under general anesthesia. Fetuses are operated on in a time window when the developing brain is extremely vulnerable to external, potentially harmful factors. To date, little is known about the effect of fetal surgery on fetal brain development.
OBJECTIVE: This study aimed to assess the effect of fetal surgery on the developing fetal brain in the rabbit model.
STUDY DESIGN: This was a randomized, sham-controlled study in time-mated pregnant does at 28 days' gestat

BACKGROUND: The diagnosis of uterine dehiscence in the early second trimester by ultrasonography is rare and its effect on pregnancy outcome is unclear.
CASE: An asymptomatic woman presented for anatomy survey in the 19th week of pregnancy. Uterine dehiscence at the site of previous hysterotomy was diagnosed by ultrasound scan. She was admitted to the hospital for expectant management and eventually opted for termination of pregnancy in the 22nd week of pregnancy. Termination was performed by classical hysterotomy without any complications.
CONCLUSION: Given the increasing cesarean delivery

BACKGROUND: Extreme uterine torsion at term is a rare obstetric event and raises several critical management considerations.
CASE: At the time of repeat cesarean section at term for fetal malpresentation, 180 degrees torsion of the uterine corpus was diagnosed. At laparotomy, the gravid uterus would not yield to anatomic repositioning, necessitating delivery through a deliberate posterior transverse hysterotomy. No maternal abdominopelvic pathology or fetal abnormalities were demonstrated. Prophylactic bilateral shortening of the round ligaments was performed at delivery to prevent recurrent
