The vast majority of malignant melanomas occur in the skin. These are associated with sunlight exposure and thought to be due to ultraviolet radiation. Melanomas in rare instances (1.3-1.4 percent) may originate from extracutaneous tissues such as the leptomeninges.(1) They have also been reported arising from gastrointestinal, respiratory and genitourinary mucosa. BACKGROUND: Excision of the ileal pouch-anal anastomosis (IPAA) with construction of a conventional ileostomy is in general the ultimate procedure after unsuccessful salvage surgery. Conversion of the IPAA to a continent ileostomy (CI) is an alternative.
METHODS: The clinical outcome of 13 patients operated with conversion of IPAA into CI was evaluated. Median follow-up was 6 years. One patient was lost to follow-up.

A simple method for double contrast examination of the distal ileum in patients with a terminal ileostomy is described, using stomal intubation with a Foley catheter, syringe injection of barium, and air insufflation with smooth muscle relaxation. The retrograde small-bowel examination is a convenient method for studying the small bowel in patients with an ileostomy. Adhesions, recurrent Crohn's disease, ileostomy dysfunction due to prestomal narrowing, and obstruction due to neoplasm are demonstrable. The technique described is faster than the peroral examination or enteroclysis, does not cause patient discomfort, and produces a rapid examination of the entire small bowel in any patient with an ileostomy.

The use of a temporary loop ileostomy for fecal diversion in 15 patients with Crohn's disease is discussed. The indications for the use of ileostomy included both emergency and nonemergency situations.
