A 91-year-old man was referred for an asymptomatic saccular left external iliac aneurysm. Comorbidities included hypertension, hyperlipidemia, peripheral arterial disease, prior radical prostatectomy with bilateral pelvic lymphadenectomy for prostate cancer, left partial nephrectomy for small renal cell carcinoma, and fundoplication for Barrett's esophagus. Review of prior computed tomography scan of the abdomen revealed that the aneurysm had been present for the last 24 years (measuring 1.6 cm in 1999) with slow growth until 2023 (measuring 2 cm). On surveillance imaging the next year, the ma…

BACKGROUND: Isolated iliac artery aneurysms are less frequent than abdominal aortic aneurysms, with the same, predominantly atherosclerotic-degenerative etiology, but patients present more frequently with symptoms or at the stage of rupture. The majority of those affected are men over the age of 65. The ideal imaging technique for diagnosis, treatment planning and follow-up is computed tomography (CT) angiography. Below a diameter of 4 cm, the risk of rupture is 6.3%, and then increases significantly with increasing diameter. Today, treatment is recommended from a size of 3.5 cm at the earlies

Behçet's disease is a vasculitis of unknown etiology. Vascular involvement is rare, but may be inaugural in many cases. We report a case of Behçet's disease revealed by a symptomatic pseudo-aneurysm of the external iliac artery. The symptomatology was non-specific. Ultrasound Doppler and computed tomographic angiography were essential for diagnosis. The pseudo-aneurysm was managed by endovascular treatment. Corticosteroid and immunosuppressant therapy were administrated after surgery.

A 73-year-old woman presented with a 6-hour history of sudden onset lower abdominal pain. Her comorbidities included chronic obstructive pulmonary disease and hypertension. She was under surveillance for a known thoracoabdominal aneurysm. On presentation, she was hypotensive with a systolic blood pressure of 50 mm Hg and a pulse of 60 beats per minute. On examination, she had a pulsatile mass with bruit in her right lower abdomen. Pedal pulses were palpable in both feet. Blood gas analysis revealed a metabolic acidosis with a pH of 7.21 and a lactate of 7.1. Haemoglobin remained stable between

As endovascular treatment of abdominal aortic aneurysms has become established, there has been growing focus on treatment of the aneurysmal iliac artery. Isolated, large iliac aneurysms >30 mm pose a risk of rupture, but, in addition, 20% to 30% of abdominal aortic aneurysms are associated with iliac aneurysmal dilatation, which can compromise long-term outcomes. Endovascular solutions are evolving and until recently have utilized standard stent graft technology. The endovascular aortic sealing system was introduced as a new, effective method for the treatment of infrarenal aortic aneurysms. I…

Iliac vein aneurysms are rare, even among venous aneurysms. We report the first case of an external iliac vein aneurysm presenting with paradoxical embolism to bilateral lower extremities. Workup after embolectomy demonstrated a patent foramen ovale along with the aneurysm and an inferior vena cava obstruction, which was treated with balloon angioplasty and ultimately stenting. The patient then underwent interval lateral aneurysmectomy. At 4-year follow-up, she is free of recurrence.

The aim of the study was to investigate the effect of recent chemotherapy (Chx) on outcome of aorto-iliac aneurysm (AAA) repair. The 2005-2010 American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database was queried to identify vascular patients undergoing AAA repair within 30 days after Chx. Seventy-one patients underwent AAA repair within 30 days of receiving Chx, group A (71 ± 8.4 years, 77.5% males) and 20,024 patients underwent AAA repair without prior Chx, group B (73 ± 9 years, 79.2% males). The two groups did not significantly differ with respect to o…

The authors present a consecutive series of 27 patients (26 men, one woman), urgently operated for aortic or iliac aneurysm during the period 1986-1994. Mean age was 68 years. Free rupture was found in 16 cases, 8 times a rupture with important retroperitoneal haematoma and fissurisation in 4. There were three ruptures of iliac aneurysms. In two cases arterio-venous fistulisation occurred: one ilio-iliac, another aorto-caval. For two juxta-renal aneurysms, supra-renal crossclamping was necessary and nephroplegia was used. Every reconstruction was done by Dacron prosthesis: a straight tube (47%…

The computed tomographic and angiographic findings of an isolated external iliac artery aneurysm secondary to cystic medial necrosis in a patient without Marfan's disease are demonstrated. A review of the differential diagnosis and surgical treatment of iliac artery aneurysms is presented. The dramatic surgical sequelae in this patient underscore the importance of preoperative consideration of this rare diagnosis.

Isolated idiopathic iliac artery aneurysm in the pediatric population is extremely rare. Aneurysms in children may be secondary to connective tissue disease, arteritis, or mycotic causes. Presented here is the case of a 2 1/2-year-old female child who, despite the absence of these predisposing factors, presented with a single large iliac artery aneurysm which was successfully resected. The literature on this subject is reviewed. Only two other cases have been reported.

Iliac artery aneurysm rupture usually causes acute hemorrhagic shock requiring urgent intervention, whereas chronic contained rupture is rare and seldom reported. An 86-year-old male with hypertension presented with sudden lower back and left lower limb pain causing difficulty in walking. Four weeks later, his back pain improved; however, intermittent claudication persisted. Computed tomography and magnetic resonance imaging revealed a large retroperitoneal hematoma due to rupture of the left iliac artery aneurysm and occlusion of the iliac arteries. Despite treatment recommendations, conserva

CONCLUSIONS: In this small retrospective review, bifurcated-bifurcated aneurysm repair of aortoiliac aneurysms with preservation of perfusion to the IIA is technically feasible and safe with good short-term and midterm results in male patients.

We describe a case of lumbosacral plexopathy caused by an isolated aneurysm of the common iliac artery. The patient presented with worsening low back pain, progressive numbness and weakness of the right leg in the L2-L4 distribution. This had previously been diagnosed as sciatica. A CT scan showed an aneurysm of the right common iliac artery which measured 8 cm in diameter. Despite being listed for emergency endovascular stenting, the aneurysm ruptured and the patient died. It is important to distinguish a lumbosacral plexopathy from sciatica and to bear in mind its treatable causes which incl

The mean size of the iliac aneurysms was 4.5 cm (range 3.5 to 5.2 cm). Four aneurysms involved the hypogastric ostium in absence of any distal neck.
RESULTS: All the patients underwent initially successful endovascular treatment of isolated iliac aneurysms and were followed from 3 to …

INTRODUCTION: The etiology of lumbosacral plexopathy is often due to compression. One of the less common causes of this is aneurysm of the i … deficit improved considerably.
CONCLUSION: When a patient presents with lumbosacral plexopathy, an iliac aneurysm should be considered as a possible etiological factor. In such cases early diagnosis is essential, since surgical treatment will usually resolve the clinical problem.

Primary aortocolic fistula is a rare complication of infrarenal abdominal aortic aneurysm. Many case reports have been published along with … re sufficient arterial blood supply to the lower limbs. Any enteral bleeding of patients with aorto-iliac aneurysm or with a history of aorto-iliac prosthetic substitution has to be considered as an aortointestinal or an aortocolic fistula until the opposite is proven. This consideration is decisive for the prognosis of aortocolic fistulae. The operation treatme …
